Water Treatment Service Contract

Gallup Indian Medical Center, Gallup, NM 87301

Statement of Work

Gallup Indian Medical Center (GIMC) 516 East Nizhoni Blvd., Gallup, NM.

GIMC Bldg. 2000 - 3 Hurst Steam Boilers.

GIMC Bldg.2000 - closed loop hot water system.

GIMC Bldg.5001 – 2 Trane screw type Chillers, 2 Cooling towers.

GIMC Bldg.2018 – 2 Rite boilers, Hot water boilers.

All the equipment is listed above need chemical treatment for it to last a longer life.

The chemicals used protect from corrosion, rust, legionella, The Chemical controllers that GIMC utilize is the Industrial Water Engineering controllers for the 3 Hurst boilers and the 2 Trane chillers cooling towers.

Industrial Water engineering only company that can be used to do monthly services.

The chemical controller and chemicals are Industrial water Engineers product.

Scope of work:

• The contractor shall perform monthly service visits to rigorously test and analyze the water in the boiler and cooling tower. Following these assessments, the contractor will treat the closed loops, cooling towers, steam boilers, and water softener systems that supply the boilers with superior-grade chemicals as required.

• Contractor shall make adjustments on digital monitor controls and other related components

• Contractor shall work with the Utility System Operators and perform training on water treatment when ask by Utility System Operator Supervisor.

• Chemical treatment reports are turn-in to the Utility System Operator Supervisor:

• Contractor shall be responsible in monitoring all Water Testing and Analysis Services performed and keeping all records on a monthly basis.

• Contractor shall monitor the cooling system piping needs for the correct amount of chemicals to be added and monitoring of microbiological growth, adjustments on the controllers.

• Contractor to monitor the water supply from the city for softness before it’s fed into the Boilers adjust the water softner controls.

• Chemical treatment – includes Boiler water, steam, Boiler feed, water cooling tower and other related components:

BW-560 mix chemical for steam boilers BW-5055 mix chemicals for condensate return water and Deaerator tank.

CW-8800 RTC- Cooling Tower.

Safety Brom 6300 -Cooling Tower.

• Chemicals – all materials used must be high quality

• Chemical waste must be disposed of properly according to EPA and other applicable regulations

• Contractor shall provide all tools, materials, supplies, transportation, equipment, personnel, project management and all other necessary items for a complete the monthly service.

• Contractor shall develop and maintain a comprehensive safety program. All contractors’ personnel onsite must use Personal Protective Equipment (PPE) that meets OSHA requirements

• Contractor shall comply with Hospital’s Interim Life Safety Measure (ILSM) and Infection Control Risk Assessments (ICRA) while doing business on IHS property. All work areas shall be maintained in a clean and orderly condition with NO piling of debris

• Contractor shall provide Influenza (Flu) Shot at their own expense to their personnel and provide Flu Shot documentation to COR or designee upon issuance of Notice to Proceed. This is in compliance with IHS mandatory Influenza policy.

• All contractors’ personnel shall be accompanied by IHS employee(s) from the Boiler Plant or Maintenance personal.

• All site work shall be performed between 8:00AM and 5:00PM, Monday – Friday.

No on-site work shall be permitted outside that workweek or on Federal Holiday observances. Any deviation from these hours shall be authorized in writing by the Contracting Officer

• Contractor to be licensed to perform work in the state of New Mexico. Contractor is to have an EIN Number and Registered with SAM.gov. Contractor is required to have qualified workers that are primarily trained in water treatments.

• 72 hours prior notice must be given to Gallup Facilities Management Staff before conducting any water testing or inspections:
