STATEMENT OF WORK
Parker Indian Health Center Bandwidth
BACKGROUND – Indian Health Service (IHS) Parker Indian Health Center is the main facility of the Colorado River Service Unit located in the town of Parker, Arizona. The 17-bed JCAHO-accredited hospital that provides the Indian community with general medical care and pediatric services. Specialty outpatient services are provided on-site by the Indian Health Services, by contract specialists or by off-site referrals. Obstetric patients are generally sent for delivery to local private facilities. Patients requiring surgical and more complex hospitalizations are referred to Phoenix Indian Medical Center, about 150 miles distant. An Emergency Department which is operated 24 hours a day is also provided for tribal members.
As the main facility of the Colorado River Service Unit, the Parker Indian Health Center also provides support services to the Peach Springs Health Center, Peach Springs, Arizona;
Chemehuevi Health Station, Chemehuevi Valley, California; Supai Health Station, Supai, Arizona and the Irene Been Health Clinic in Moapa, Nevada.
DESCRIPTION – Contractor to provide internet services that utilize a dedicated high speed Ethernet based line of 100 megabytes per second. The contractor will provide the infrastructure to support the capabilities on a continuous basis and to support the operation on a 24 hour basis 365 days per year health care facility and outlaying health clinics in a tristate area (Arizona, California and Nevada.)
JUSTIFICATION - The IHS Parker Indian Health Center, Parker, Arizona, through its information technology is utilized to provide end users with accessibility to EHR (Electronic Health Record) for patients and also to transmit data to offsite facilities within the IHS intranet and also global internet.
IMPACT – without the increased Bandwidth, the Parker Indian Health Center and its subordinate clinics is bogged down in the transfer of information and this creates a problem with overall efficiency and the provision of quality health care for our patient population.

Base year is for a period of 12 months effective October 01, 2022 through September 30, 2023.
At the discretion of the Indian Health Service the period of performance may be modified by written notice to exercise an option up to an additional four (4) option years.

The point of contact (POC) shall correspond with the Contractor regarding delivery issues, issuance of modification to add funds and ensuring all administrative paperwork is as complete as appropriate. The POC does not have the authority to change orders or issue changes that would affect the contract price, terms and conditions or period of performance. The POC shall be responsible for the following:

a. Monitoring the contractor’s technical progress, including the surveillance and assessment of performance and recommending to the Contracting Officer changes to the delivery order;
b. Performing technical evaluation, as required;
c. Performing technical (daily and final) inspection and acceptance as required by this delivery order;
d. Assisting the contractor in the resolution of technical problems encountered during performance;
e. The POC shall certify receipt and acceptance of service;
