1.0 REQUEST FOR PANIC ALARM SYSTEM

NMCSD Inpatient Mental Health has requested a proposal for a Panic Alarm System that will cover both 1 North and 1 West Units at Building 1 NMCSD. This system will alert Mental Health staff members of patient and staff emergencies and safety events occurring between the 2 units. Each Unit will have the ability to alarm by zone within their own area and also alarm as a zone to each other’s areas for overall situational awareness. In addition, the system will have the capability to alarm to the Outpatient AOP clinic located adjacent to 1 North and 1 West Units provided Outpatient AOP has the same Controller and Alarm panel installed and cabled as necessary.

2.0 STATEMENT OF WORK

Brizo will provide, install, program, test and commission the Panic Alarm System for Inpatient Mental Health 1 North and 1 West. This system will be configured so that each of the 2 areas will have their separate Panic Alarm panel which will display their respective zones for each transmitter designated to either a person or in a temporary mounted position under a desk within a room. Each panel will be programmed so each individual transmitter will be a zone on their respective panel for immediate identification of the room or the person who requires assistance. This system will alarm in the nurse’s stations on both Units. It does not include connecting to the existing siren in 1 North.

2.1 CLIN - 001 Unit 1 North

Unit 1 North will have 21 transmitter FOBs which will be either temporary fixed position; mounted under a desk or be portable; attached or handheld by a person. The configuration will be set up and programmed in 1 North so that each of the 21 transmitters will be associated with a zone/person or a zone/room so an alarm transmitted by a specific FOB will display the assigned person or assigned location to that zone for display.

In addition, if an alarm is activated in 1 West, the alarm will be indicated as a 1 West zone on 1 North’s Alarm panel. Should Outpatient AOP in Building 1 deploy the same Alarm panel upgrade, 1 North and 1 West would also each have a zone for AOP on their panel and vice versa for overall situational awareness for response purposes between the 3 Units. Each area will be aware of their own individual zones and the other area’s alarms will be displayed as an individual zone on each of the 3 Units’ Alarm panels.

2.2 The major components included will be as follows:

a) Security Control Receiver Panel - 32 Zone - (Qty 1)

b) Controller - (Qty 1)

c) Repeater - (Qty 2)

d) Transmitter Devices - (Qty 21)

2.3 CLIN 002 - Unit 1 West

Unit 1 West will have 21 transmitter FOBs which will be either temporary fixed position; mounted under a desk or be portable; attached or handheld by a person. The configuration will be set up and programmed in 1 West so that each of the 21 transmitters will be associated with a zone/person or a zone/room so an alarm transmitted by a specific FOB will display the assigned person or assigned location to that zone for display.

In addition, if an alarm is activated in 1 North, the alarm will be indicated as a 1 North zone on 1 West’s Alarm panel. Should Outpatient AOP in Building 1 deploy the same Alarm panel upgrade, 1 West and 1 North would also each have a zone for AOP on their panel and vice versa for overall situational awareness for response purposes between the 3 Units. 3.0 PROJECT TASKS

The major implementation tasks involved are as follows:

a) Order Equipment/Materials/Receive Equipment/Materials

b) In House Staging, Integration and Testing of Equipment; RMA if necessary

c) Installation of Cabling

d) Installation of Equipment

e) Programming and Testing of Equipment/System On-Site

f) Programming and Testing of Transmitters

g) Labeling Panels

h) Remove Old Panic Alarm Equipment - Unit 1 North ONLY

i) Training

j) Operational Acceptance

4.0 CHANGE ORDERS

Any changes requested regarding quantity or quality of work and materials will result in a change order and may have a cost and/ or schedule change associated with the change. In the event any unforeseen work is necessary that is found during the process of installation and integration, the additional work required as a result will be discussed with customer. Should there be any costs associated with the work to be performed notification via a formal change order will be provided to the customer and approved by the customer prior to any work being performed. If there are any changes to the initial design concept or any changes to the scope of work to be performed made by the customer, these changes will also be performed through our formal Change Order process. The customer may then accept the work to be completed or decline depending upon the nature of the work that is to be performed. A Modification to the contract would then be approved.

5.0 COMPLETION AND ACCEPTANCE OF THE SYSTEM

Upon completion of the installation and implementation, Brizo will test the equipment per the Statement of Work in Paragraph 2.0, hereinafter referred to as the “System” and validate that it is operational per the manufacturer’s specifications. At such time, the customer will formally accept the equipment by completion of the attached Acceptance Certificate.
