STATEMENT OF WORK
1.0 INTRODUCTION
1.1 The contractor shall provide, in accordance with this statement of work, either professional on or off-site document destruction, burring/destruction of hardware (i.e. computers, hard drives, etc…) services for Naval Medical Center San Diego (NMCSD), California 92134. All equipment, vehicles, materials, supervision, and other incidental services necessary to perform this service as defined in this Statement of Work.
1.2 During the term of this contract, the contractor shall handle, on behalf of the Government, classified documents, Personally Identifiable Information (PII) documents, and Health Insurance Portability and Accountability Act (HIPAA) related documents, as well as confidential and personal information which will be covered for document destruction.
1.3 During the term of this contract, all of the contractor’s employees are to go through screening processes to include credit, criminal, and background checks to ensure proper handling or Government documents that are going to document destruction.
2.0 SERVICE
2.1 Destruction service to take place by contractor on behalf to the Government handling classified Documents, PII documents, and HIPPA related documents, as well as confidential and personal information which will be destroyed by the contracting company who handles such service on-site as a document management destruction service.
2.2 Contractor must sign in Hazardous Material Cage Log prior to picking up shred bins and sign out from the log after completing work. File type of destruction to take place is termed “Complex File” – fifteen (15) 96-gallon bins containing documents will either be shredded on-site or picked up from NMCSD for off-site shredding, Hazardous Material Cage once a week every Wednesday, between 09:00 – 14:00.

2.3 An additional type of destruction termed “Purge Files” may occur four times a year when NMCSD requires destruction of a greater than normal volume of documents. This will require an additional destruction of sixteen (16) or forty (40) 96-gallon bins containing documents will be shredded on-site or picked up from NMCSD for off-site shredding.
2.4 Multiple 96-gallon bins to accommodate large volume of paper for high volume document traffic will be shredded on-site or picked up from NMCSD for off-site shredding by the contractor. Empty bins will be replaced for NMCSD to continue to fill. They will be obtained via the contractor, locked for transport, and not unlocked until serviced or requested by NMCSD.
2.5 A certificate of destruction is a requirement necessary back to NMCSD for proof of destruction for audit purposes.
2.6 The contractor is not required to provide service on the following Federal holidays:

2.7 If pick up day falls on a holiday, please adjust to next business day for pick up.
3.0 TITLE TO DOCUMENTS
The Government retains ownership to all documents that are to be under destruction and require a proof of destruction for audit purposes.
4.0 GOVERNMENT PERSONNEL
NMCSD employees will not perform maintenance or attempt repairs to equipment, i.e. 96 Gallon Bins while such equipment is under purview of this contract with the contractor.
5.0 PARTS AVAILABILITY
To ensure minimal equipment downtime, the contractor shall maintain and or replace the 96-gallon bins on a timely manner during the Monday, Wednesday, and Friday time frame 09:00 - 14:00 hours. Bins must be kept clean, locked, and in full operation order at all times.
6.0 LABOR
All compensation for labor, parts and materials must be included in the contract price.
7.0 PARTS/SUPPLIES QUALITY
Parts and supplies provided under this service contract shall be guaranteed to be equal in all respects, including performance, interchangeability, durability, and quality to the Original Equipment Manager parts when new or as presently recommended by the contractor.
8.0 TRAVEL AND RELATED EXPENSES
The contractor is responsible for its related trip expenses including round trip travel, mileage, and all other necessary expenses related to travel. The contractor requires valid DBIDS pass for entry to Naval Medical Center San Diego prior to start date of contract. POC: RSCS Celestin Dashka, 619-532-6971.
52.217-8 OPTION TO EXTEND SERVICES (NOV 1999)
The Government may require continued performance of any service within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor within 15 days of contract expiration.

52.217-9 OPTION TO EXTEND THE TERM OF THE CONTRACT (MAR 2000)
(a) The Government may extend the term of this contract by written notice to the Contractor within 30 days; provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 60 before the contract expires. The preliminary notice does not commit the Government to an extension.
(b) If the Government exercises this option, the extended contract shall be considered to include this option clause.
(c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed 5 years.
