1. DESCRIPTION OF WORK:

This Project includes a base contract in addition to two option items. The base contract involves construction and maintenance activities such as road grading with ditch/drainage maintenance; drainage dip construction; culvert removal and replacement; new culvert installation; new gate installation; cattle guard maintenance and wingwall replacement; and cattleguard removal and replacement. The base contract involves four National Forest Service System Roads (NFSRs). The road details are as follows:

· NFSR 76

· 29.7 miles total

· Maintenance level 3, double lane: miles 0.00-9.65

· Maintenance level 2, single lane: miles 9.65-29.70

· Road needs include reconditioning (with ditch and drainage structure maintenance), cattle guard removal/replacement, cattleguard maintain and replace wingwalls, culvert installation.

· NFSR 17

· 4 miles total

· Maintenance level 2, single lane: miles 0.00-4.00

· Road needs include reconditioning (with ditch and drainage structure maintenance), culvert/fish barrier installation.

· NFSR 440

· 10.55 miles total

· Maintenance level 3, single lane: miles 0.00-10.55

· Road needs include reconditioning (with ditch and drainage structure maintenance), culvert removal and replacement.

· NFSR 440M

· 1.94 miles total

· Maintenance level 2, single lane: miles 0.00-1.94

· Road needs include reconditioning (with ditch and drainage structure maintenance), excavation and embankment (drain dip construction).

Option items #1 and #2 contain furnishing, hauling, and placing with compaction of aggregate spot surfacing along the routes listed in the base contract. Spot surfacing sections will be flagged by Forest Service representatives. All culverts, gates, and cattleguards to be removed are to be disposed of off forest land.

The contractor shall have a Traffic Control Plan submitted to the CO for approval a minimum ten (10) days prior to commencing operations. A Traffic Control Plan shall be approved prior to beginning any work, see Specifications for more details. If road closure is needed, coordination with COR is required fourteen (14) days before intended closure.

The contractor is responsible for utility locates. Damages to any utilities shall be the responsibility of the contractor. Locates are available by calling 811.

The contractor for this work shall furnish all materials, supplies, equipment, transportation, labor, quality control, and supervision necessary to complete these tasks. Mobilization shall be paid once for cost to mobilize and demobilize equipment to and from work site within Carson NF. Costs incurred while performing the work or traveling between locations on the Carson NF will not be paid. If a road has not been accepted by the COR prior to Contractor moving equipment to another road; the Contractor will incur any cost associated with returning equipment to road for additional items that may be required for acceptance.

All equipment shall be pressure washed prior to entering Forest Service land to ensure it is clean of seeds and other debris for weed control, subject to inspection. A 48 hour notice shall be given to COR to inspect equipment prior to mobilization onto NFS lands. No fuel storage on site is permitted. Fuel trucks must be equipped with spill containment kits and portable berms that meet all state and federal regulations.

All work in the base contract shall be accomplished within one-hundred twenty (120) days of the issuance of the initial base contract Notice-to-Proceed.

All work in the option item(s) shall be completed within 60 days of the issuance of the option item Notice-to-Proceed.

The contractor should receive CO approval of work schedule at least fourteen (14) days prior to starting work. Contractor shall notify CO seven (7) days prior to work commencing and pre-construction meeting are required.

2. PROJECT LOCATION:

The project routes are located on the Camino Real District, of the Carson National Forest, in Taos County, New Mexico.

The project area can be accessed as follows:

· NFSR 76

· Beginning of road/work coordinates: 36.126686N, 105.509912W

· At around mile 24.02 of SH-518 turn onto NFSR 76.

· NFSR 17

· Beginning of road/work coordinates: 36.262207N, 105.362178W

· At around mile 24.02 of SH-518 turn onto NFSR 76 and continue for about 16.07 miles, the begininng of NFSR 17 can be accessed on the right.

· NFSR 440

· Beginning of road/work coordinates: 36.269753N, 105.572639W

· At around mile 6.78 of SH-518 turn onto NFSR 438 and continue for about a tenth of a mile, the beginning of NFSR 440 can be accessed on the right.

· NFSR 440M

· Beginning of road/work coordinates: 36.250031N, 105.551375W At around mile 6.78 of SH-518 turn onto NFSR 438 and continue for about a tenth of a mile, then NFSR 440 will be on the right. Continue on NFSR 440 for about 2.17 miles and the beginning of NFSR 440M is on the left.

3. TRAFFIC CONTROL

The Contractor shall provide, erect and maintain all necessary barricades, suitable and sufficient lights, danger signals, signs, and other traffic control devices, and shall take all necessary precautions for the protection of the work and safety of the public. Roads closed to traffic shall be protected by effective barricades, and obstructions shall be illuminated during the hours of darkness. Suitable warning signs shall be provided to properly control and direct traffic.

The Contractor shall erect warning signs in advance to any place on the project where operations may interfere with the use of the road or trail by traffic and at all intermediate points where the new work crosses or coincides with an existing road or trail. All road barricades, warning signs, lights, temporary signals, flagman and operators and equipment, and other protective devices, except for special devices, shall conform with Part VI of the Manual on Uniform Traffic Control Devices for Streets and Highways, published by the Federal Highway Administration and applicable safety codes.
