STATEMENT OF WORK

Naval Hospital Twentynine Palms, California 92278

ORGANIZATION: Naval Hospital Twentynine Palms (NHTP) located in Twentynine Palms, California is responsible for providing the U.S. Navy, other military branches, and their military families with reliable quality health care. In addition to maintaining the status as one of the nation’s premier care providers, NHTP is responsible for preparing for deployable support of operational forces and caring for their beneficiaries who remain behind

GENERAL REQUIREMENTS: The effort required hereunder shall be performed in accordance with this Statement of Work and with all other terms and conditions set forth herein.

1.0 GENERAL INFORMATION

This scope of work (SOW) covers the LOX storage tank, delivery, maintenance, back-up system and technical support for the entire system to NHTP for the base year plus four additional years immediately following the base year. The scope of work performed under these specifications includes the associated tank fees, inspections, training compressed oxygen for the eight H size tanks that comprise the secondary system and the liquid oxygen for the primary tank. NHTP has the requirements to supply medical grade breathing oxygen as needed to the emergency room, patient rooms, surgery and associated clinics as needed. This is accomplished by a large capacity tank of LOX that feeds into the hospital’s supply system. The LOX system that feeds into the hospital is provided and maintained by the contractor. The contractor is responsible for ensuring that the LOX is certified 99% pure at time of testing/certification prior to delivery. A copy of the certification shall be provided at the time of deliver.

1.1 This is a non-personal services contract to provide: Bulk Liquid Oxygen refills and/or tank swaps as well as a monthly tank rental fee.

1.2 Description of services/introduction: The contractor shall provide all personnel, equipment, supplies, facilities, transportation, tools, materials, supervision, and other items and non-personal services necessary to perform LOX storage tank, delivery, maintenance, back-up system and technical support as defined in this Performance Work Statement (PWS) except for those items specified as government furnished property and services. The contractor shall perform to the standards in this PWS.

1.3 Background: Contractor will deliver the following supplies under the defined periodicity in the table below: Supply Item Specifications on Replenishment One (1) 1500 gallon liquid oxygen storage tank Maintenance performed according to federal, military and state requirements. Eight H size Compressed Oxygen Cylinders Filled as necessary. Cylinders will require replacement with filled tanks if the tanks are used Routine Liquid Oxygen (LOX) Replenishment Filled within one week of order placement. LOX must be certified to be medical grade. Certification must be provided upon delivery of LOX. Annual amount of replenishment is expected to be below 500,000 cubic feet of medical grade oxygen. Emergent LOX Replenishment Filled within 24 hours. A copy of the certification shall be provided at the time of deliver.

DESCRIPTION/SPECIFICATIONS: Contractor will deliver the following supplies under the defined periodicity in the table below

replenishment is expected to be below 500,000 cubic feet of medical grade oxygen.

Emergent LOX Replenishment
Filled within 24 hours.

CERTIFICATION OF LOX: The contractor is responsible for ensuring that the LOX is certified 99% pure at time of testing/certification prior to delivery. A copy of the certification shall be provided at the time of deliver.

1. Vendor is responsible for providing and maintaining, one 1500 gallon storage tank, and eight H size compressed oxygen cylinders. The 1500 gallon tank is the primary delivery system. The eight compressed cylinders are the auxilary or backup system to the primary tank. Telemetry for the primary and auxilary systems are considered part of the delivery system. Telemetry monitoring equipment and other system support equipment that may be required to comply with applicable federal, state, military or The Joint Commission standards.

2. Vendor is responsible for providing any training on the storage tank and associated equipment as dictated by regulatory drivers.

3. Tank location will be located behind NHTP, BLDG 1145.

4. Vendor shall provide an atmospheric gasses inspection report semiannually. These reports will be between 5 to 7 months apart, and must be within the same fiscal year.

5. Vendor must be FDA certified to produce medical grade liquid oxygen.

6. Vendor must have other centrally located FDA certified sites to support facility requirement in the event of emergency situation, i.e. earthquakes, flooding, tornadoes, etc.

7. Must be able to provide a monitoring system to manage levels of medical gas.

8. Vendor must be certified to perform and complete FDA annual inspection for bulk liquid oxygen system.

9. Vendor must have trained and certified staff to perform any required maintenance and inspections at the local site or available within 4 hours of time of notification.

10. Vendor must be able to provide an onsite repair technician to make repairs to the existing bulk system.

11. Must have an emergency contact system in place in the event of catastrophic or emergency situations.

12. Personnel authorized to place refill orders are Safety Office, Material Management

13. Department and the Head or Facility Manager of the Facilities Maintenance Department.

1.5 DELIVERY INFORMATION: For all supplies noted above, the delivery dates will span the length of the contract. Contractor shall deliver routine orders within one week, Monday-Friday (with the exception federal holidays) between the hours of 0700 and 1530, from the time the order is placed. Emergent orders must be delivered within 24 hours.
