Statement of Work
Title: Public Health Surveillance for Chronic Conditions via Internet Panels
BACKGROUND
Public health surveillance uses a variety of sources, e.g., surveys, medical records, billing records, sales records, and vital statistics. Population-based surveys are a mainstay of public health surveillance. These can be conducted by telephone, personal interview, and – increasingly – via internet. Several companies in the United States maintain internet-based respondent panels which can be tapped rapidly. These panels are created and then weighted to simulate population-based samples. Importantly, internet-based panels may be able to reach respondents who are reluctant to participate in traditional telephone surveys and/or respondents in population subgroups for whom it is difficult to derive stable estimates from population-based samples unless oversampling of those subgroups occurs. Data from panels can also be weighted to provide estimates that may be representative of particular subgroups or the overall adult population. In addition, preselection of participants may allow data to be obtained more quickly and flexibly than is typically possible in traditional public health surveys. This is particularly important when CDC urgently needs to collect timely information, as when dealing with an outbreak or other public health emergency.
PURPOSE
The purpose of this study is to collect data from individuals who participate in an internet panel, with attention to population subgroups defined by age, race/ethnicity, and/or geography. Questions will focus on chronic diseases or conditions and risk factors and behaviors related to those conditions as well as medical care and management approaches. The specific questions to be asked and population subgroups to be addressed will be determined in consultation with CDC.
SCOPE OF WORK
The Contractor shall conduct 3 internet-based surveys, each in up to 10 states to be identified by CDC. Each survey will include approximately 25-30 individual questions related to chronic diseases and conditions (e.g., conditions such as pregnancy, diabetes and cancer; risk factors and behaviors such as tanning and toothbrushing; and medical or management factors such as care-seeking and participation in lifestyle change programs). Once it has been determined which questions and populations will be addressed, the Contractor will perform the data collections. Depending on the specific needs of each survey, the Contractor may use existing panels of construct new panels. The Contractor will provide CDC with results that are designed to be valid, stable, and representative of the adult population of each participating state as well as results representative of the subpopulations of interest within each state, with approximately 400-500 adult respondents per state. The Contractor will additionally provide CDC with a dataset and corresponding codebook of data collected for each target population group.
The specific tasks include:
Task 1: Define specifics of surveys Contractor shall work with CDC Technical Monitor to determine the states, questions, and subgroups to target; and analysis plan. Contractor will develop a data management plan (DMP) in accordance with CDC Data Policy for each dataset. (DMP will be finalized, if needed, when data collection and analysis are complete.)
Task 2: Conduct surveys Contractor shall collect information.
Task 3: Analyze Data The Contractor will provide CDC with unweighted results as well as results that are weighted and/or sampled to be representative of the adult population of each participating state as well as results representative of the subpopulations of interest within each state.
Task 4: State Datasets and Codebooks Contractor shall deliver final reports summarizing the data findings, both weighted to represent the target population and unweighted. Results shall be cross-tabulated by demographic factors such as age group, sex, and race-ethnicity. Datasets shall be provided in CSV or Excel (or other statistical programming format as determined by discussion between CDC and the contractor) with codebooks of data collected.

(a) Section 508 of the Rehabilitation Act of 1973 (29 U.S.C. 794d), as amended by the Workforce Investment Act of 1998 and the Architectural and Transportation Barriers Compliance Board Electronic and Information (EIT) Accessibility Standards (36 CFR part 1194), require that when Federal agencies develop, procure, maintain, or use electronic and information technology, Federal employees with disabilities have access to and use of information and data that is comparable to the access and use by Federal employees who are not individuals with disabilities, unless an undue burden would be imposed on the agency. In order to facilitate the Government's determination whether proposed EIT supplies meet applicable Section 508 accessibility standards, offerors must submit an HHS Section 508 Product Assessment Template, in accordance with its completion instructions. The purpose of the template is to assist HHS acquisition and program officials in determining whether proposed EIT supplies conform to applicable Section 508 accessibility standards. The template allows offerors or developers to self-evaluate their supplies and documentation detail - whether they conform to a specific Section 508 accessibility standard, and any underway remediation efforts addressing conformance issues. Instructions for preparing the HHS Section 508 Evaluation Template are available under Section 508 policy on the HHS Web site http://hhs.gov/web/508.
In order to facilitate the Government's determination whether proposed EIT services meet applicable Section 508 accessibility standards, offerors must provide enough information to assist the Government in determining that the EIT services conform to Section 508 accessibility standards, including any underway remediation efforts addressing conformance issues.
(d) Respondents to this solicitation must identify any exception to Section 508 requirements. If a offeror claims its supplies or services meet applicable Section 508 accessibility standards, and it is later determined by the Government, i.e., after award of a contract or order, that supplies or services delivered do not conform to the accessibility standards, remediation of the supplies or services to the level of conformance specified in the contract will be the responsibility of the Contractor at its expense.
