STATEMENT OF NEED
Brand Name or Equal Comprehensive Optical Biometer
BACKGROUND:
The Department of Veterans Affairs, Corporal Michael J. Crescenz VA Medical Center, has a requirement for a new Comprehensive Optical Biometer for its Ophthalmology Department, used to calculate the power of intraocular lenses (IOLs) that are implanted during cataract surgery.
DELIVERY:
The Contractor must comply and provide all necessary information to VAPHS. Otherwise, the delivery may be refused and require redelivery at no additional cost to the government.
All items must be FOB Destination.
Normal business hours for acceptance of deliveries are 7:30am-3:00pm, Monday thru Friday excluding Federal Holidays. Any delivery service performed for emergency orders may be delivered other than normal business work hours and must be approved ahead of scheduled delivery.
The Government recognized U.S. Holidays are:
New Year’s Day, Martin Luther King, Jr. Day, Presidents Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Juneteenth, Thanksgiving Day, and Christmas Day. If the holiday falls on Sunday, it is observed on Monday.
Deliveries must be coordinated in advance with POC once a ship date is established. The POC, Biomedical Engineering and/or Logistics Representative shall be the only VA entity signing for freight received on this requirement. Any damage to VA property by the contractor/vendor must be repaired within 90 days at the contractor/vendor’s expense.
The VA-issued purchase order number must be included in the packing slip. All items included in the requirement shall be shipped and delivered to site at same time. If deliveries must be broken into several shipments due to size, this must be coordinated in advance with the POC. Failure the coordinate shipment with POC, may result in failure to deliver on site and cause re-scheduling of delivery/freight at no charge to the government. The packing slip must include the VA-issued purchase order number.
Items must be delivered in manufacturers original sealed containers with brand name marked thereon or maintain coverings in place and in good repair.
No smoking, eating, or drinking inside the hospital at any time when delivering/assembling any equipment.
OPERATOR TRAINING:
Contractor shall provide technical training for biomedical engineering personnel. The cost for this training and all training related expenses such as travel, lodging, and other expenses are included in the contract price. The training shall cover the preventative maintenance, troubleshooting, and general maintenance of the equipment. Training shall allow for classroom instruction as well as individual hands-on programming, troubleshooting and diagnostic exercises. Competency assessment shall be provided with the training.
The training program shall include at a minimum following: Technical Training, Operational Training, and Hardware/Software trouble shooting. Training shall occur onsite during normal business hours 08:00am
– 4:30pm, Monday through Friday. Competency assessment shall be provided with the training.
Scheduling of all training shall be coordinated with the Corporal Michael J. Crescenz VA Medical Center after delivery is complete.
WARRANTY:
Contractor shall provide at a minimum a 1 -year manufacturer’s warranty. Any recalls to the equipment, the vendor agrees to replace within three work business days without any additional cost to the government. The warranty shall include all travel and shipping costs associated with any warranty repair.
