About this file

This document is a Justification for Sole Source Procurement under $250,000 for the Centers for Disease Control and Prevention (CDC) National Center for Injury Prevention and Control (NCIPC). The purpose is to justify a sole source contract with the University of Washington (UW) Institute for Health Metrics and Evaluation (IHME) to develop an innovative approach to improve non-medical cost estimates related to injury and morbidity. The contract will produce new U.S. health and economic burden estimates for NCIPC priority injury and violence prevention topics, such as traumatic brain injury, using Global Burden of Disease (GBD) study data. Key deliverables include draft journal articles describing the new estimates. The justification cites IHME's unique methodology and data access for generating quality of life estimates as the rationale for the sole source procurement. No competitive sources were identified that could meet the Government's requirements.

Rev. 5/14/19 burden annual estimates of the medical and non-medical burden of an NCIPC priority topic area for injury and violence prevention in terms of fatal/nonfatal, sex, age group, and injury mechanism and intent (e.g., self-harm cut/pierce injury). Priority topic area:

traumatic brain injury. Primary deliverable: Draft biomedical format journal article for peer review (1,200-3,000 words) describing new estimates. Author order for participating staff IHME and NCIPC/DIP/DAB based on level of effort. Option Period 1: US medical expenditures on injuries by mechanism and intent Replicate approach described in Dieleman et al. “US Health Care Spending by Payer and Health Condition, 1996-2016” (2020) JAMA (PMID: 32125402) to produce novel estimates of total medical spending on injuries by mechanism and intent (e.g., unintentional cut/pierce), payer (e.g., private, etc.), patient sex, age, and other characteristics (as possible), health care setting (e.g., nursing home, etc.). As possible: Incorporate multiple data years in addition to most recent available data years; differentiate fatal and nonfatal events; and incorporate prevalence (case count) data to produce unit cost estimates (e.g., per injured person). Primary deliverable: Draft biomedical format journal article for peer review (1,200-3,000 words) describing new estimates. Author order for participating staff IHME and NCIPC/DIP/DAB based on level of effort.

Option Period 2: GBD Economic burden of injuries and violence subject #2 Produce new comprehensive US health and economic burden annual estimates of the medical and non-medical burden of an NCIPC priority topic area for injury and violence prevention in terms of fatal/nonfatal, sex, age group, and injury mechanism and intent (e.g., self-harm cut/pierce injury). Priority topic area: will be determined based on most recent available GBD data and NCIPC prevention priority topics list, potentially will include: self-harm, burns, or drowning. Primary deliverable: Draft biomedical format journal article for peer review (1,200-3,000 words) describing new estimates.

Author order for participating staff IHME and NCIPC/DIP/DAB based on level of effort. Option Period 3: Improving non-medical cost of injury morbidity for WISQARS #2 Building from previous funded work to crosswalk underlying GBD data on the non-medical health burden injury morbidity (FY23), use GBD disability weight source data to improve injury morbidity burden estimates for priority CDC injury surveillance injury types in the NEISS-AIP data source. Primary deliverable: Draft biomedical format journal article for peer review (1,200-3,000 words) describing new estimates. Author order for participating staff IHME and NCIPC/DIP/DAB based on level of effort.

The vendor will have demonstrated research experience in: 1) Generating original estimates of nonfatal injury and disease quality of life values (such as disability weights, disability-adjusted life years, quality-adjusted life years, health utilities) from general population respondent samples, and 2) Access to and extensive experience with an existing comprehensive data source for such original estimates that can facilitate direct or calculated quality of life values for the broad range of injury types (mechanism, intent, body region, and/or nature of injury) that comprise the NEISS- AIP data source, including treated-and-released and transferred/hospitalized patients.

2. Proposed Vendor and Estimated Procurement Amount (if available):

a. Proposed Vendor: University of Washington Institute for Health Metrics

b. Estimated Procurement Amount:

3. Is the requirement on GSA Schedule? ☐ Yes ☒ No

If yes, provide schedule number: Click here to enter text.

D. Justification:

1. The justification for this non-competitive action is:

☐ In accordance with FAR 13.106-1(b)(1), “only one source [is] reasonably available” which states that, “For purchases not exceeding the simplified acquisition threshold. (i) Contracting officers may solicit from one source if the contracting officer determines that the circumstances of the contract action deem only one source reasonably available (e.g., urgency, exclusive licensing agreements, brand-name or industrial mobilization). (ii) Where a single source is identified to provide a portion of a purchase because that portion of the purchase specifies a particular brand-name item, the documentation in paragraph (b)(1)(i) of this section only applies to the portion of the purchase requiring brand-name item. The documentation should state it is covering only the portion of the acquisition which is brand- name.”

☐ Urgency

☐ Exclusive licensing agreements/Proprietary data

☐ Brand name required

☐ Industrial mobilization

☐ Other (e.g., compatibility, space issues, continuity, compatibility, etc.)

☒ In accordance with FAR 8.405-6 “Limiting Sources” which states that, “the only circumstances that may justify the action are – (A) An urgent and compelling need exists, and following the procedures would result in unacceptable delays; (B) Only one source is capable of providing the supplies or services required at the level of quality required because the supplies or services are unique or highly specialized; or (C) In the interest of economy and efficiency, the new work is a logical follow-up to an original Federal Supply Schedule order provided that the original order was placed in accordance with the applicable Federal Supply Schedule ordering procedures. The original BPA must not have been previously issued under sole-source or limited-sources procedures.”

☐ Urgency

☒ Only One Source

☐ Logical Follow Up

☐ Brand name required

2. Other facts supporting sole source procurement:

a. What can this product/service do that other products/services cannot do?

Market research by the NCIPC Economics Team has identified that collaboration with the University of Washington Institute for Health Metrics (IHME) to integrate Global Burden of Disease study (GBD) disability weights with WISQARS nonfatal injury data is a desirable next step, based on: Unique and preferred methodology: GBD study injury disability weights are elicited through vignettes and general population surveys and managed by the Institute For Health Metrics and Evaluation at the University of Washington. Cost-effective for CDC: GBD disability weights are publicly available and based on periodic survey data collected from thousands of general population respondents globally (qualify of life loss data, unlike medical cost data, are not typically treated as country-specific). With adaptation through funded collaboration, GBD disability weights can yield updated quality of life loss cost data for WISQARS Cost of Injury. Once a crosswalk between GBD injury classifications and NEISS-AIP is established, future GBD disability weights data can be integrated for updates to WISQARS Cost of Injury website.

b. What other products/services were considered and why did they not meet the need?

The services provided are used by a very small population of potential users which include government agencies, health foundations and other non-profit organizations.
