SUBJECT: Solicitation Number 19KE5020Q0022 Ground Ambulance and First Responder Services
The Embassy of the United States of America invites you to submit a quotation for 24- hour ground ambulance and first responder services.
Your quotation must be submitted in a sealed envelope marked "Quotation Enclosed" to the Contracting Officer -Aisha O’Neal on or before 10:00 am on September 4, 2020. No quotations will be accepted after this time.
In order for a quotation to be considered, you must also complete and submit the following:
1. SF-1449 (blocks 17, 24 and 30). Block 24 should list the total value of the quote for the base year and the option year.
2. Section 1, Pricing
3. Section 5, Representations and Certifications and ensure compliance with FAR
52.229-11 is completed with IRS Form W-14 found at www.irs.goc/w14
4. Additional information as required in Section 3
The U.S. Government intends to award a contract/purchase order to the responsible company submitting a technically acceptable quotation at the lowest price. We intend to award a contract/purchase order based on initial quotations, without holding discussions, although we may hold discussions with companies in the competitive range if there is a need to do so.
All contractors must be registered in the SAM (System for Award Management) Database https://beta.sam.gov/ prior to contract award pursuant to FAR provision 52.204-
7. Therefore, prospective offerors should register prior to the submittal of quotations/proposals.

This is a firm-fixed price type of contract.
II. PRICING
VALUE ADDED TAX. Value Added Tax (VAT) is not applicable to this contract and shall not be included in the CLIN rates or Invoices because the U.S. Embassy has a tax exemption certificate from the host government.
PRICING SCHEDULE:

DESCRIPTION/SPECIFICATIONS/WORK STATEMENT
I. PERFORMANCE WORK STATEMENT
The purpose of this firm fixed price purchase order is to furnish 24-hour ground ambulance and first responder services to eligible U.S. Embassy Nairobi mission personnel per attachment A performance work statement. It will be necessary for the Contractor to have a staff of medical professionals to receive phone and radio command medical orders from appropriate authorized Government representative(s), thereby being able to render life-saving techniques to patients.
2. DEFINITIONS.
“Advanced Life Support (ALS)” means Advanced Life Support Unit
“Ambulance activation” means the request by a Member in case of a Serious Medical Incident for an ambulance and includes mobilization of a ground ambulance with the appropriate first responders.
“Ambulance Service Providers” means the ambulance companies that may respond to an ambulance activation and are located nationwide.
“Calendar Day” means the twenty-four-hour period from midnight to midnight. Saturdays, Sundays and all holidays are considered calendar days.
“CO”: Contracting Officer
"COR" means the Contracting Officer's Representative, appointed in accordance with Section G of this contract.
“Commencement Date” means the date on which the service begins.
“COR”: Contracting Officer’s Representative
“COVID19 Response” means a Serious Medical incident that may be a Rescue or Transfer that is due to a highly infectious disease and in this instance in reference to COVID19.
"Government" means the Government of the United States of America unless specifically stated otherwise.
“Inter-facility transfer” means a Serious Medical incident at the place or work to a health facility or between two health facilities.
“Intervention” means all activities related to a single event of a Serious Medical Incident.
“Member” means the employees, dependents of employees, contractors, and visitors to designated sites of the US Embassy in Nairobi who are eligible to use the Services.
“Membership Fees” means the fee for subscription to the Membership.
"Ordering Officer" means the Contracting Officer of the U.S. Embassy, Nairobi.
“Rescue”: means a Serious Medical incident at the home or outside of work including road accidents.
“Serious Medical Incident” means a medical condition which requires ambulance transport to a health facility.
“Services” means the medical emergency services provided to the Member under “Description of
Services”.
“US Embassy Nairobi” means the US Embassy - Kenya.
3. Management
3.1. Contracting Officer’s Representative. The Contracting Officer’s Representative (COR) is responsible for validating requests for services, and providing instructions to the Contractor in a timely manner.
3.2 Authorized Service Requesters. Any service request will list the names and phone numbers of all authorized order(s). Service requesters are only authorized to order service within the boundaries established in the contract and must seek approval from the Contracting Officer before requesting for any services required, but not authorized in the contract.
3.3 Project Manager. The Contractor shall designate a Project Manager who shall be the Contractor’s point of contact. The Project Manager shall be responsible for managing the
Contractor’s work under this contract, including delegating requests to service providers along with any instruction required, and ensuring a smooth and effective operation. The Contractor’s Project Manager contacts are:

4.0 Service Scope.
4.1 Service Specifications. The Contractor agrees to provide all services specified in the contract for any person determined to be eligible by U.S. Embassy Nairobi mission personnel, regardless of race, creed, color, or national origin of the person for whom services are being ordered.
Additionally, the Contractor warrants that sub-contracting shall not be used as a means of circumventing this provision.
4.2 Service Request. Requests for services will be written or made via telephone by the appropriate authorized Government representatives. A list of authorized representatives will be provided upon award of the contract. Offerors are advised that the Advanced Life Support (ALS) (or ALS capable comparable) vehicles must be made available to the Embassy at any given time. In event a mass casualty, offeror is expected to provide ALS vehicles commiserate with level of need (as required).
4.3. Work Equipment. U.S. Embassy will not be responsible for any loss, theft, or missing equipment belonging to the Contractor. The Contractor shall ensure all stretchers, restraint straps, wheelchairs, and blankets, etc. are permanently stenciled or taped, making such property readily identifiable as belonging to the Contractor. The Contractor further agrees that his/her technicians and/or attendants will make all reasonable efforts to pick up any of his/her equipment immediately after completing the trip. Failure to do so will in no way require the Embassy to assume financial responsibility.
4.4. Location of Work. The services shall be provided within and throughout Nairobi and its surrounding counties. For the purpose of this solicitation, local trip is defined as the U.S.
Embassy’s primary service area which includes the following neighborhoods; Gigiri, Runda, Runda Mumwe, Kiambu, Nyari, Thigiri Ridge, Hillview, Muthaiga, Kurura, Parklands, Westlands, Shanzu, Loresho, Spring Valley, Kileleshwa, and Lavington. Long distance is defined as any area that is not within these listed neighborhoods. The Contracting Officer will rely on the Embassy medical staff on areas of any disputed mileage.
5.0 Detailed Work Statement.
5.1 Access to emergency specialists. The Contractor shall provide access to emergency specialists to assist in medical assessments, dispatch of Ambulance Service Providers or other specialists and coordination with hospitals as required during the provision of services.
5.2 Emergency Contacts. The Contractor shall provide a dedicated hotline to request ambulance activation including dispatch and transport to medical facilities in case of a Serious Medical Incident.
5.3. Service Activation. The Contractor shall arrange for the ground ambulance activation, dispatch, retrieval and transfer to a receiving facility in Kenya.
5.4. Remote Locations. The Contractor shall make reasonable best effort when the beneficiary is in a hard to reach or remote location and/or the situation is insecure.
5.5. Monitoring Services. The Contractor shall, during an ambulance activation, actively monitor through real-time location updates from the point of dispatch of an Ambulance Service Provider through to pick up a Member and until the admission of the Member at a receiving health facility.
When required, contact parties involved including the US Embassy in Nairobi, international and local insurers and health facilities to facilitate the provision of these Services.
5.6. Patient Assistance. The Contractor shall ensure Ambulance Service Providers shall assist the patient as necessary; including, but not limited to entering and exiting vehicles; entering, locating, and checking in at appropriate clinic areas upon arrival at destination.
5.7. Medical Information. The Contractor shall assist to obtain information that may relate to the Serious Medical Incident, to assist in any future medical care. This should be performed on a reasonable efforts’ basis in compliance with any legislation or regulation in force.
5.8. Response Time. The Contractor shall ensure standard emergency transports that do not require specialized staff are expected to be provided within 15-20 minutes of notification. Transportation requiring specialized staff (i.e. RN) is expected to be provided within 30 minutes of notification.
Patients wait time for a pre-scheduled trip should not exceed 15-20 minutes.
i. The Contractor shall report any/all delays as soon as the driver is aware of the situation.
ii. Ambulance services shall be available upon request twenty-four (24) hours a day, three hundred sixty-five (365) or sixty-six (366) days per year, including all National Holidays.
6.0 Personnel Requirements
6.1. Contractor Personnel. Upon award of the contract, the Contractor shall supply the Embassy with the contact information for a dedicated project manager as well as a phone number and email address for a 24-hour manned 1st Level Customer support center.
6.2. Personnel Training. The Contractor shall ensure all contracted personnel have completed training in accordance with the standards published by the Nursing Council of Kenya (NCK) and Kenya Council of Emergency Medical Technicians (KCEMTs).
6.3. Training Evidence. The Contractor shall provide all assurances and evidence of the training program successfully completed by the Emergency Medical Technician.
6.4. Training Certifications. The Contractor shall ensure all certifications and licenses, are recognized by government or public entity where the emergency ambulance service is operated or by which it is governed.
7.0 Reporting Requirements
The Contractor must provide the following reports:
7.1. A written incident report is sent to the COR within 24 hours of incident.
7.2. Standard emergency transport requiring no staff provided and response times report.
7.3. Standard emergency transport with staff provided and response times report.
7.4. Monthly Report. The following items are provided to the COR by the 15th of each month.
i. Total cost billed to Embassy
ii. Total quantity of trips ordered
iii. Total quantity of individuals transported
iv. Total quantity of destinations
7.5. Quarterly Report. Contractor provides aggregate quarterly report by 15 December, 15 February, 15 May, and 15 July.
7.6. Annual Report. Contractor provides annual report to COR by 1 July of each contract year.
8.0 QUALITY ASSURANCE AND SURVEILLANCE PLAN (QASP)
This plan provides an effective method to promote satisfactory contractor performance. The
QASP provides a method for the Contracting Officer's Representative (COR) to monitor
Contractor performance, advise the Contractor of unsatisfactory performance, and notify the
Contracting Officer of continued unsatisfactory performance. The Contractor, not the
Government, is responsible for management and quality control to meet the terms of the contract.
The role of the Government is to monitor quality to ensure that contract standards are achieved.

C. The contract will be for a firm- fixed price for a base period of one-year period from the date of the contract award, with four one-year options.
The anticipated period of performance for contract services will be 28 September 2020 through 27 September 2025. Contingent upon need and subject to the availability of funds, the government reserves the right to exercise four – 1-year option periods. The exercise of each option year is at the sole discretion of the government.

(d) Comptroller General Examination of Record. The Contractor shall comply with the provisions of this paragraph (d) if this contract was awarded using other than sealed bid, is in excess of the simplified acquisition threshold, as defined in FAR 2.101, on the date of award of this contract, and does not contain the clause at 52.215-2, Audit and Records-Negotiation.
(1) The Comptroller General of the United States, or an authorized representative of the
Comptroller General, shall have access to and right to examine any of the Contractor’s directly pertinent records involving transactions related to this contract.
(2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR subpart 4.7, Contractor
Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.
