About this file

This document is a Cost Proposal Form from the Ohio Department of Health (ODH) for an Environmental Monitoring contract covering State Fiscal Years (SFY) 2025-2029. The contract involves collecting environmental samples near a specified Nuclear Power Plant, with detailed requirements for sample collection, labeling, and shipping. The primary contract period spans SFY 2026 and SFY 2027 (7/1/2025 - 6/30/2027), with an optional two-year renewal term for SFY 2028 and SFY 2029 (7/1/2027 - 6/30/2029). Key deliverables include collecting environmental samples according to specified methods and locations, correctly labeling and shipping samples via overnight courier to the ODH Laboratory in Reynoldsburg, and submitting quarterly invoices within specified timeframes.

The cost proposal template shows zero dollar amounts for all line items and deliverables, suggesting that pricing details are to be completed by the potential offeror. The document allows for a cost narrative to be attached to explain and justify costs, with specific guidance on explaining hourly rates and connecting costs to the technical proposal. Samples must be shipped to the Radiochemistry Section at the Ohio Department of Health Laboratory, with strict requirements for submission timing and accuracy. Samples not meeting the defined timelines in Attachment A may be considered invalid and not eligible for reimbursement. The form provides a framework for potential suppliers to submit their environmental monitoring services proposal, with placeholders for organizational and contact information.
