SPECIFICATION SHEET
Mountain Home AFB, ID
STEEL PAVING FORMS

PART I - General Contract Requirements
1. General Requirements
1.1. The 819th RED HORSE Squadron operating on Mountain Home AFB (MHAFB), ID requires 2,500 linear feet of 10’ steel paving forms.
1.2. The Contractor shall deliver the forms to the job site. The Contractor is providing the material for work to be completed by the Government. The materials shall conform to the specifications provided in Part II.
1.3. All equipment is to be delivered to MHAFB. See attachment for delivery location. Commercial gate coordinates: 43.0696, -115.8954 Delivery site coordinates: 43.0346, -115.8828
1.3.1. Specific drop-off location on site will be coordinated with the 819 RHS project manager, TSgt Jonathan Buck (jonathan.buck.3@us.af.mil).
1.4. All increases, decreases, or modifications to the requirements specified in this document must be directed by the Contracting Officer (CO).
2. Delivery Periods. The delivery of most materials will occur during standard business hours on Monday through Friday between 6AM and 6PM, however the times will fluctuate. The Government will provide notice to the Contractor for a material call no later than 48 hours prior to the anticipated delivery; the Government reserves the right to delay the delivery with 24 hours advance notice.
2.1. The performance period for this contract shall be between 31 March and 31 Jul 2023.
2.2. The Contractor shall be responsible for transporting the material to the construction site. The Government will make every effort to provide the Contractor the maximum amount of notice possible.
2.3. Notification of date and time of delivery will be made as early as possible for all materials. A minimum of 48 hours prior to the required delivery date, the 819 RHS Project Manager will contact the provider to coordinate the required quantities, date/time, and specific location of deliveries.
2.4. In the event of adverse weather events prohibiting safe delivery of materials, the 819 RHS Project Manager will contact the supplier to coordinate rescheduling of delivery within the following 48 hours.
3. Military Installation Requirements
3.1. All mud, dirt, debris, foreign objects, trash, or spills caused by the contractor (including Sub-Contractors) on a street or parking lot, used as access to the work site or staging area shall be cleaned off the same day or as soon as practicable or requested by the CO.
3.2. The Contractor shall immediately report to the CO to discuss any issues or incidents which may indicate potential imminent risk to Government personnel or the environment. Following the notification, the Contractor shall be available to the CO for instructions regarding remediation of the release. Contractor should be aware of “you spill, you dig” policies, which means if you are the cause of the spill then you are responsible for the cleanup. To report a release or spill, contact the CO immediately.
3.3. The Contractor shall follow existing security procedures and instructions applicable to MHAFB. The Contractor is required to provide any and all materials as required by security personnel. The Contractor is responsible for providing required documents in a timely manner to maintain the submitted work schedule. Delays caused by documents not being submitted early enough are the responsibility of the Contractor.
3.4. The contractor shall adhere strictly to existing security procedures and instructions applicable to accessing MHAFB. Contractor personnel attempting to access MHAFB shall hold a current, valid identification (ID) and/or limited to: material description and quantity. The U.S Government shall not be liable for delays resulting from pass coordination/approval. Contractor shall allot time into its schedule for obtaining base access.
4. Emergency Situations. Installation access may be restricted at any time. Restricted access is reserved for emergency actions, exercises, or increased installation security.
5. Contractor Responsibility.
5.1. The Contractor is responsible for the well being of their workers. The Government will not be responsible for supplying food or water to the Contractor’s workers. The Contractor should bring enough food and water for his/her workers to last the entire delivery period.
5.2. The Contractor shall provide the name and phone number of a supervisor that can make decisions for the Contractor and who is responsible for the performance of the work. The supervisor shall be designated in writing when the contract is awarded, and must always be available during work hours. The supervisor shall read, write, and speak fluent English. Additionally, the supervisor shall have sufficient technical knowledge with regard to the requirements prescribed in this document. The identified Government representative shall have the right to request a replacement for any individual not deemed acceptable to the Government at any time.

PART II – Material Specifications
1. General
1.1. The Contractor shall deliver 2,500 linear feet of 10’ steel forms and corresponding stakes and dowel adapters.
1.2. Partial delivery can be authorized by the 819 RHS Project Manager upon request by the contractor. Initial delivery must begin on 31 Mar 2023.
2. Steel forms
2.1. These steel forms will be used for cast-in place concrete of thickness up to 16” and must support such a lateral load.
2.2. Forms must be dual sided 12” x 16” smooth faces, 3/16” steel thickness with dowel holes spaced 15” on center, drilled 2.5” diameter, and positioned halfway up each side of the form, on dowel center.
2.3. Include dowel bar support system and hardware to keep dowels in place, with plastic adapters to accommodate 1” dowel bars.
2.4. Forms must have sliding design end connectors.
2.5. A minimum of three, channel design, stake pockets must be present in each form.
2.6. Steel stakes must be a minimum of ¾” diameter and 36” long.
2.7. Steel forms must be able to structurally support the weight of a Allen Engineering TRTP275 Tri-tube Roller Screed (9,500 lbs).
