TRAINING MANIKIN PACKAGE

STATEMENT OF WORK

Description of Services: The contractor shall provide all equipment, tools, materials, software, and other items for use by clinic personnel in terms of the Training Manikin equipment package at the 366th Military Treatment Facility, Building 6000, at 90 Hope Drive, Mountain Home AFB, Idaho. No refurbished or previously used parts will be sent for use. Finally, the contractor shall provide a paid invoice report covering the total cost of all equipment purchased.

Salient Characteristics: Vendor is to provide in full the below requirements and all requirements be Section 889 compliant:

1 TOMManikin- Basic which features partial amputation to the left arm, full amputation to the right leg, with both being tourniquet responsive. Additionally, it must include ITTS Advanced Remote Tablet and Software enabling wireless control of up to 12 TOMManikin units with capabilities of bleeding, breathing, multiple voice tracks, pulse rates, pulse pressure and realistic responses to responder interventions. Furthermore, 1.2 gallons of onboard blood reservoir, 1400 Pelican accessories case, 12v/5 and 10 Amp NiMH rechargeable batteries each with its own smart charger, T handle tool, all air and fluid replacement connectors, and a TOMManikin Operator’s Manual.

1 TOMManikin- Female which has a gunshot wound (GSW) featuring a sucking chest wound (with anterior entry/posterior exit), junctional tourniquet/packable GSW to groin (anterior entry/posterior exit), and tourniquet-responsive/packable GSW right leg (through and through).

Additionally, it must include ITTS Advanced Remote Tablet and Software enabling wireless control of up to 12 TOMManikin units with capabilities of bleeding, breathing, multiple voice tracks, pulse rates, pulse pressure and realistic responses to responder interventions.

Furthermore, 1.2 gallons of onboard blood reservoir, 1400 Pelican accessories case, 12v/5 and 10 Amp NiMH rechargeable batteries each with its own smart charger, T handle tool, all air and fluid replacement connectors, and a TOMManikin Operator’s Manual.

Scope of Work. All equipment will need to be delivered to 90 Hope Dr. Building 6000, Mountain Home AFB, ID. The equipment will be inspected for damages prior to being signed for.

Licenses, Permissions & Certs Required: The contractor’s installation team must include service engineers on site who are fully qualified to solve technical problems, and are equipped with all critical system components.

Warranty: A minimum of 1 year warranty is required.

Hours of Operation: All installation, validation, and training will be performed on site during clinic operation hours, 0730-1630; Monday-Friday.

Government-Furnished Property and Services: Government personnel will be responsible for ensuring all adequate space is available for staging while unloading all new equipment.

Contractor Furnished Property & Services: The contractor shall be responsible for installation and adherence to industry regulations during the install process.

Responsibility for Damage: The Government shall not be responsible, in any way, for damage to the contractor's supplies, material, or equipment; or to contractor's employee’s personal belongings brought to or left on site.

Telephones: Government telephone facilities shall be made available for local calls to the contractor for official business use only.

Security Police and Fire Protection: The Government will provide general on-base security and fire protection services. Security Police telephone extension is 8-2256 (on-base) and 208- 828-2256 (from all other phones) for both emergency and routine calls. Fire protection emergency phone is 8-0911(from on-base phone) and 208- 828-0911 (from all other phones).

Utilities: All reasonable quantities of electricity will be made available to the Contractor without charge.

Toilet facilities: Use of existing toilet facilities in the immediate work area will be permitted.

The Contractor will ensure facility cleanliness is maintained at all times to the extent of cleaning up after Contractor personnel only.

Physical Security: The contractor shall be responsible for safeguarding all Government property while servicing components. This includes the building, and all personnel, appliances and products within it.

Base Passes: The contractor shall obtain a base pass from Security Forces at the proper base gate, coordinated through the Contracting Officer or designated representative.

Base Entry & Inspection: When driving a contractor vehicle, the contractor will enter Mountain Home AFB through the Grand View Highway gate. The contract vehicle will be inspected by Security Forces personnel prior to base entry. When driving a privately owned vehicle (POV), the contractor shall enter the base through the main gate Visitor Center on Airbase Rd. POVs may be inspected by Security Forces personnel on a random basis.

Weapons, Firearms, and Ammunition: Contractor personnel are prohibited from possessing weapons, firearms, or ammunition, on themselves or within their contractor-owned or privately-owned vehicles on Mountain Home AFB.
