ADMINISTRATIVE PROVISIONS

PART 1 GENERAL

1.1 SUBMITTALS

Submittals must be submitted by the Contractor within the number of days discussed at the pre-construction meeting. A list will be provided via the Submittal Register according to Section 01 33 00 SUBMITTAL PROCEDURES.

1.2 DESCRIPTION OF WORK

The work covered by these specifications consists of furnishing all plant, labor, equipment, materials, insurance, and duties for performing all operations in connection with providing power replacement panels, equipment and all necessary appurtenances for a complete and operational system in strict accordance with these specifications and applicable drawings and subject to the terms and conditions of the contract.

The drawings attached to the specifications show the general arrangement and approximate dimensions at the site. THE CONTRACTOR SHALL BE

RESPONSIBLE FOR ACCURACY REQUIRED FOR THE PROPER ACCOMPLISHMENT OF THE WORK.

1.3 PRINCIPAL FEATURES

Furnish and install complete replacement of all of the service conductors along with their associated above ground service panels, weatherheads and masts in (5)* Missle Alert Facilities (MAF) listed, and in all of the Launch Facilities (LF) that are listed. Also, replace the Manual transfer switch in all (5) MAFs (topside) as indicated. This work is on secure sites, inside and outside the fence. Additionally the work is in the barracks of the MAF and in a congested space below ground which must remain in use during and after working hours, including weekends, Federal Holidays and the workdays Federal Holidays are observed. The work to be performed is in a restricted area. Therefore, the Contractor shall conform to all requirements of MAFB B1 31-101 and AFMAN 31-108.

*NOTE: MAF O-01 is untypical from the other (4) MAFs in that replacement of the conductors and associated equipment for the LCEB run from the Utility Pole to the LCEB panel are excluded.

The work schedule will be prepared by coordinating with the Contracting Officer to help maximize the efficiency of the Contractor's schedule. Wing operational requirements, both real and exercise, however, take precedence over the scheduling of this work. The site will be worked as determined by the Contracting Officer/Government Representative, without exception. The schedule, once established, shall be considered firm. Certain types of work will only be permitted upon advanced coordination with the LCC Capsule Crew and the Government inspector/escort.

1.4 GOVERNMENT RESPONSIBILITIES IN THESE SPECIFICATIONS

Develop/process Entry Authorization List (EAL) for Contractor site access.

Contractor needs to be aware of the likely presence of lead based paint in this project. Refer to Section 01 57 19.05 32 TEMPORARY ENVIRONMENTAL CONTROLS for additional requirements.

1.5 CONTRACTOR'S USE OF PREMISES

The Inter-Continental Ballistic Missile (ICBM) MAFs and LFs are under the control of and within the vicinity of the Air Force Base Wing. The MAFs are continuously manned, secure installations. The LFs are unmanned, secure installations. Specific security and access regulations apply. The general work areas are shown on the drawings. All work areas are within the secured areas and require a Government inspector/escort at all times.

1.5.1 Owner Occupancy

These sites are operational facilities. Consequently the Contractor shall adhere to all applicable United States Air Force (USAF) regulations and procedures regarding security, site access, inspection, and safety.

Continued functional use of the site by the USAF is required and so the MAF and LF control and security functions shall remain in operation; therefore, temporary power shall be made available to the LCC and LF by the contractor when work prevents power from being supplied for more than four (4) hours.

1.5.2 Construction Operations

USAF controls the site, the 25-foot perimeter area outside the security fence, and the 100-foot access road easement. Property outside the above defined areas are not under USAF control and the Contractor shall not trespass, but shall first obtain written permission from the landowner, if necessary for equipment storage, stating the U.S. Government shall be held harmless from any damage caused by the Contractor during duty and non-duty hours for use of the private land. The Contractor shall be responsible for any negotiations concerning construction easement and storage of materials on private lands.

Contractor vehicles and trailers shall be allowed to park inside the security fence to deliver materials or equipment required to support the work under this contract. Vehicles and equipment trailers shall be allowed on site during work performance only and shall not remain parked inside the security fence overnight with the exception of one

(1) trailer when allowed by the FM. Contractor vehicles and equipment will be placed so as not to interfere with gates, emergency escape routes, antennae and other above ground structures or the helicopter pad. No equipment shall be parked and no materials stockpiled within 100 feet of the outer edge of the helicopter pad. Vehicles may be parked in the Air Force access area but any such parked vehicles must be clear of the boundary fence by a maximum distance of 25 feet and within a maximum distance of 50 feet from the centerline of the access road. Privately owned vehicles are not allowed on the site. Vehicles that are needed to conduct the work must be registered to the company

SECTION 01 00 50.00 Page 3 listed on the EAL, along with contractor's name on the placecard on the side of all vehicles.

The Contractor will be permitted to store construction materials at the site in a location coordinated with the Facility Manager (FM) (along with the approval of the Government inspector/escort). The Contractor shall be responsible for arranging for storage of all other construction equipment and materials during the course of the project.

Once equipment and supplies are brought inside the fence, Government inspector/escort personnel are required to accompany and advise Contractor personnel in placement and removal of such items. The Government shall not be responsible for loss of Contractor material, tools, or equipment.

The Contractor shall clean up the work area debris and shall ensure the security of equipment, tools, and work/contract materials is accomplished before the end of the workday.

Government representative may perform on-going verification of compliance of equipment used for construction. The equipment approved must be the equipment used.

1.5.3 SITE WORK SCHEDULE

Work hours shall be 0730 to 1600 Monday through Friday, exclusions are Saturday, Sunday, and the workdays that Federal Holidays are observed. The schedule shall include the sequence of work that will be performed and the dates on which work will be started and completed. Coordination with the Government Representative needs to occur by 0900, on the previous week's Tuesday, prior to the expected site work in order to ensure Inspector availability. Duration of the performance period shall be determined using the phasing system indicated on the drawings. Only one site may be worked at a time and must be finalized before moving to the next.

1.6 ENTRY AND SITE REGULATIONS

1.6.1 Entry

Access to the MAFs and LFs is controlled by the U.S. Air Force. All contracted personnel entering the sites shall be on an Entry Authorization List (EAL) created by the Government.

Contractor shall provide complete written, valid, and legible data that shall include legible photocopies or scanned electronic documents to be used to produce the initial EAL prior to their initial commencement of work at the site for this project. Twenty (20) working days are required to process, initially issue, add to, correct, and renew EALs.

Any delays caused by submission of incorrect, outdated, and invalid information will be at no cost to the Government. Missile Engineering shall provide to the Contracting Officer and the Contractor the expiration date for each EAL, when the date is made effective.

Contractor submitted data shall include the following for each employee (to include prime, subcontractor and delivery/testing services employees):

1) Project GHLN number, contract number, name of project, and

Contracting Officer.

2) Employers name and address (city and state)

3) One form of ID:

(i) Legible photocopy or electronically scanned Valid Driver’s license, State photo ID card, or official state/federal photo ID card with the individual’s name.

(ii) The same ID shall be shown for entrance onto the site once the EAL is effective.

4) All of the following for each company vehicle: make, model, year, color, plate state, and plate number.

Identification cards that expire during the construction period must be updated and photocopies or scanned copies must be sent to the Contracting Officer or his/her authorized representative 20 days prior to expiration or the person shall be dropped from the EAL. The Contractor/A&E is responsible for maintaining an accurate valid list.

SITE ENTRY

In order to gain entry onto the project facility, the Contractor/A&E personnel must be:

a. Listed on an effective EAL.

b. Identified by escort personnel.

c. Able to produce, for verification, the exact form of ID given for the EAL process.

d. Submit to physical and electronic search of person and materials.

e. Physically escorted on-site by an authorized escort official and remain under escort full time while on site.

f. Not in possession of any electronic device or cellular telephone at Launch Facilities (LFs).

Upon arrival, the escort will identify all Contractor/A&E personnel and check identifications. Processing onto and off the sites may require approximately thirty (30) minutes each and shall not be considered delay time. The processing onto and off of the site shall be during normal working hours, between 0730 hours (7:30 a.m.) and 1600 hours (4:00 p.m.).

VEHICLE IDENTIFICATION

Company Identification (logo) must be clearly, legibly, and identifiable at a minimum of 30-foot distance and displayed on each side of all vehicles and equipment brought onto or operated on site.

Vehicles and equipment without such identification may be denied access to the site.

The access road leading from the main road adjacent to the site is under military control. The area off to the sides of the access road is either private property, federal, state, or county property. Parking vehicles on the access road is prohibited unless permission has been obtained from the government. Vehicles may be parked in the Air Force access area but any such parked vehicles must be clear of the boundary fence by a maximum distance of twenty-five feet (25’) and within a maximum distance of fifty feet (50’) from the centerline of the access road. Permission to use additional offsite parking and storage areas must be obtained in writing from the landowner and a copy must be submitted to the Contracting Officer prior to the Notice to Proceed for Site Work.

1.6.2 Security

The Contractor shall:

Protect all Government property.

Comply with base physical security regulations and procedures.

The Government inspector/escort will be responsible for on-site security.

While work is in progress, the Contractor may be required to remove his personnel due to Alert status, real or exercise.

When work is being done in the LCEB, LCC, LFSB or LER, the Contractor crew must all stay together. If one person leaves for any reason, all must leave. Maximum number of Contractor's personnel permitted in each Launch Control Equipment Building (LCEB)Launch Facility Support Building (LFSB), Tunnel Junction (TJ), or elevator shaft will be five (5), and shall be three (3) in the LCC or LER.

During all on-site work, the Contractor personnel will be accompanied and continuously monitored by one (1) Government inspector/escort. Failure to remain within the area being monitored is a serious breach of security and safety regulations.

The Contractor will take all necessary action to ensure compliance. The Contracting Officer reserves the right to deny entry to or take action with any Contractor personnel who refuses to conform to security and/or safety regulations. Delays resulting from such entry denial shall not result in any cost to the Government or schedule delay. Also, the Government can take any action necessary due to the security breach at no additional cost to the Government.

1.7 SAFETY

The Contractor will comply with the most recent Occupational Safety and Health Administration (OSHA) regulations. OSHA Subpart 1926 and 1910 will be enforced.

The Contractor shall submit, within ten (10) working days prior to commencement of work, to the Contracting Officer, in writing, the name of the designated on-site person responsible for ensuring that all safety and accident prevention provisions of this specification are implemented and enforced. The Contractor shall ensure that all Contractor personnel receive safety training and briefings and that all safety inspections, practices, and procedures are complied with.

The Contractor shall indoctrinate each employee to ensure clear understanding of the following subject areas including, but not limited to:

The purpose of the accident prevention program (i.e., to minimize hazards and prevent injuries).

A review of the hazards inherent in the work environment and the precautions to be taken.

Location of first aid and other emergency facilities. (The Contractor shall maintain a complete first aid kit on site at all times).

A detailed description of what to do in case of injury, fire, or identification of a safety hazard.

Required safety equipment and its location on site. Safety equipment includes but is not limited to earplugs, goggles, face shields, safety harness/lanyards, protective clothing, and hard-hats. All personnel will wear safety equipment when required by the Contractor, Contracting Officer, or Government inspector/escort.

Review of daily cleanup procedures.

Location of company safety rules (posted or handout).

Identification of the Contractor employees responsible for safety/accident prevention.

In addition to normal safety practices, the Contractor will comply with the following requirements:

The safety of Contractor personnel on-site is the responsibility of the Contractor.

The Contractor and on-site superintendent shall attend an Air Force safety briefing conducted at the pre-construction meeting and comply with all safety procedures.

All personnel shall comply with Air Force on-site safety procedures and additional procedures as directed by the Contracting Officer or his designated representative when the Contractor's safety person is not immediately available.

Equipment shall be placed so as not to impede normal traffic in and out of the work area, or to interfere with emergency exit from work area and off-site.

Emergency flashlights shall be provided by the Contractor and shall be available in the crew's work area at all times.

Elevator load limit. The elevator load limits are posted (6000 lbs).

DO NOT exceed the maximum-posted limit. Close elevator doors when elevator is not in use.

All vehicles shall be in safe operating condition. Vehicles and equipment not complying shall not be allowed on site.

A number of hazards inherent to the work environment must be recognized, including:

Electrical circuits and devices pose serious and generally continuing hazards. The Contractor shall take every precaution to prevent personnel injury.

Mechanical devices (i.e., fans) can pose serious hazards when operated. Appropriate precautions shall be taken.

First Aid and Medical Requirements:

The Contractor shall comply with base accident response requirements.

These requirements are provided in a safety briefing by base personnel at the pre-construction conference. The Contractor shall keep these requirements on file at the working site for inspection by Air Force

The Contracting Officer or his designated representative shall coordinate emergency medical, ambulance, or airlift requirements.

All accidents (employee injuries and vehicle, building, equipment or property damage, etc.), regardless of severity, shall be reported immediately to the Contracting Officer or his designated representative. The Contracting Officer will notify the proper authority per Air Force regulations.

Emergency Operations. The Contractor shall provide, within ten (10) working days after award, a list of key Contractor personnel and telephone numbers to the Contracting Officer to affect early notification of any emergency that arises (during non-working hours) due to an incident related to this contract. It is the responsibility of the Contractor to:

Notify the Government inspector/escort of any emergency situation. The Government inspector/escort will follow local emergency procedures.

Ensure that all Contractor personnel are evacuated from the site and accounted for.

Assist in administering first aid, as required, and evacuate injured personnel.

Obtain emergency services as required.

Avoid movement of, and alterations to, any possible evidence of the cause of an emergency, incident, or accident. Removal of injured personnel or the need to prevent further damage takes precedence over this requirement.

1.7.1 Special Safety Concerns

1.7.1.1 Work in Confined Spaces

If hazardous materials are brought onto location, atmospheric monitoring needs to be performed to insure a safe environment is maintained.

Wear appropriate personal protective equipment.

1.7.1.2 Fire Regulations

Compliance with local and Air Force Fire Regulations is mandatory. Fire extinguishers rated and approved by the National Fire Protection Association and Base Fire Department of sufficient size, type, and quantity to cope with all known hazards, will be available and provided by the Contractor.

1.7.2 Welding Safety Precaution

1.7.2.1 Welding Permit

Before welding, the Contractor shall obtain a welding permit from Base Fire Department, within ten (10) working days after award, before any open flame or welding operation is conducted. The Contractor shall obtain any

SECTION 01 00 50.00 Page 8 necessary AF Form 592 – Welding, Cutting, and Brazing Permit (aka “burn permit”), at least three days prior to construction at the sites. Contact Missile Engineering at (307) 773-2438 for requirements, initiation and completion of permit. Contractor shall have the completed permit control number prior to any burning/welding operations. The welding permit shall be renewed every 30 days. The Contractor shall conduct his work such that after any welding operation a one-hour (minimum) fire watch in the area in which welding has occurred is conducted.

Welding downstairs will be accomplished in strict accordance with the following: GENERAL REQUIREMENTS FOR WELDING AIR FORCE TECHNICAL ORDER

(T.O.) 21M-LGM30F-12.

The prospective work area will be inspected by the Project Engineer or Government inspector/escort to insure that prescribed precautions have been taken.

The Contractor will perform atmospheric tests before, during, and after the operation to assure that no dangerous conditions of flammability or toxicity exist.

W A R N I N G Oxygen gas should not be released for purpose of clearing the air before, during, or after welding operations. Release of oxygen gas shall be for welding or cutting purposes only.

Prior to initiation of welding/cutting or open flame soldering operations, combustible material will be removed from the area.

Residue from spills and leaks will be cleaned up. Materials that cannot be removed will be protected against the possibility of ignition from operation.

The Contractor will post a fire watch person on any welding, cutting, open flame, heat or spark producing operation below grade and where flammable materials cannot be removed from within the welding area.

One fire watch person may perform for more than one welder provided he can properly monitor all operations assigned to him. Carbon dioxide and water type fire extinguishers shall be accessible and the fire watch person shall be trained in their use.

Ventilation for General Welding and Thermal Cutting shall be in accordance with ANSI A 49.1. During ventilation procedures, the Contractor shall exhaust contaminated air through an adequate duct system. Ventilation will be installed via flexible ducts connected to a negative air machine with a HEPA filter. This system will then be hooked up topside. The ventilation system shall have a minimum capture velocity of 160 cubic feet per minute (cfm) at two feet from the intake.

The Contractor will provide personal protective equipment for welders and helpers.

A fire-resistant shield of non-asbestos material, sheet metal, or similar material will be constructed to contain the work area.

Critical equipment, electrical cables, and lines in the welding area will be protected with non-asbestos blanket or other suitable material. All critical equipment will be inspected after welding is completed to insure that these parts have not been damaged.

Air duct openings will be covered with fire resistant covers to prevent

SECTION 01 00 50.00 Page 9 entrance of sparks. Covers will be used which will allow ventilating and cooling air to circulate adequately.

If oxygen and acetylene tanks are used, they will be located above grade and placed in a vertical position. They will be accessible to personnel when welding/cutting or soldering is in progress. Valves will be closed at the tanks during any work stoppage. Hose lines will be inspected daily for hose leaks. Ties will anchor hose lines so that the hose connections are not used to support the hose. Padding will be provided to protect hose lines at all corners or sharp projections.

All welding power source equipment will be located above grade. It will be accessible to personnel and will be shut off and disconnected during any work stoppage. Electrodes will be removed from holder when not in use.

Electrical welding cable will be entwined at least one turn per two linear feet from entrance to LCC to within six feet of electrode holder.

Welding leads will be isolated from frame of welder.

A minimum of two feet must be maintained between electrical welding leads and explosive devices and/or their circuitry.

Electrical welding grounding point will be located as close as practical to the weld point.

Inert gas shield arc, high voltage start, plasma-arc cutting, arc air gouging or transformer-type welder shall not be used except with specific approval.

1.8 OTHER CONTRACTOR REQUIREMENTS

In addition to those requirements outlined in this specification, the Contractor is responsible for the following:

Any power process, i.e., welding, core drilling, etc., in the elevator shaft, TJ, LFSB, LER, and LCEB areas requiring more than 20 amps shall require shutdown of the LCC. EQUIPMENT REQUIRING MORE THAN 40 AMPS SHALL NOT BE USED ON SITE UNDER ANY CIRCUMSTANCE. Contractor must provide own power for any processes requiring more than 20 amps. This power shall be located topside outside the structure at a minimum distance of 10'-0" from buildings. Any power process required in the LCC and LER area shall require LCC or LER shutdown. Contractor must notify Missile Engineering 72-hours in advance of any such work so that LCC or LER shut down may be scheduled. Note: The currently approved welder is Lincoln Electric, Class 300D. If another welder is to be used, the Contractor shall bear all costs for analysis and testing for higher headquarters approval. No performance period extension will be granted for headquarters approval.

The Contractor shall maintain, at each location, all Material Safety Data Sheets (MSDS's) pertinent to hazardous material used in the project in compliance with OSHA's Hazardous Communications Standard 29 CFR 1910.1200. MSDS's will be available to all contractors and Government inspector/escort at all times during construction. MSDS data will be available on all hazardous material used in the contract and all Contractor personnel will be made aware of the requirements.

The Contractor shall furnish and maintain, on site, the following:

Explosion proof flashlight and batteries (One (1) set per person).

Safety harness/lanyard, equipment lowering nets, and hoist.

Two (2) portable fire extinguishers rated at 2A 20 BC minimum.

One emergency first aid kit.

Safety glasses and hearing protection for all workers.

1.8.1 Concealed Work

All items of work to be concealed shall be Government inspected prior to concealment.

1.9 AVAILABILITY OF UTILITY SERVICES

Except where restricted in these specifications in other sections for certain tasks, all reasonable quantities of utilities including power up to 20 amps shall be made available to the Contractor. No temporary connections will be permitted, which may be of a semi permanent nature.

All connections shall be made by plugging into existing receptacles while work is in progress for electrical service. All extension cords must be GFI protected. All electrical cords shall be unplugged at the end of each work period. The Contractor must make arrangements for furnishing additional external electrical power for welding, drilling or additional lighting. The power generator must be located topside at a minimum distance of 10 feet from the building. Water connections, where required, shall be made with standard garden hose and hose bib fittings. This refers only to service water for contractual work requirements. All hoses will be disconnected and valves closed to hose outlets at the end of each work shift. All hoses and/or power cords will be removed at the close of work each day so that all doors, elevator doors, and blast doors may be operated in their normal manner during Contractor non-working hours. Electrical power plug-ins and water connections will be cleared by the Government inspector/escort before they are made. All power cords shall be GFI protected.

1.10 COORDINATION AND VERIFICATION OF DIMENSIONS

The Contractor shall be responsible for coordinating his work so as to minimize interference to the operational equipment and personnel in the building, as the facility will remain operational during construction.

Extreme caution shall be exercised to prevent damage to the equipment from dust, water, etc. It shall be the Contractor's responsibility to furnish drop cloths and whatever material may be required to keep equipment safe at all times from dust and debris. The Contractor, in a manner in accordance with Federal, State, and County rules and regulations, shall properly dispose of debris off site.

The Contractor shall visit the premises and thoroughly familiarize himself with the details of the work and conditions prior to starting work. THE

DIMENSIONS GIVEN ON THE DRAWINGS ARE FOR GENERAL REFERENCE ONLY AND SHALL

BE FIELD VERIFIED BY THE CONTRACTOR PRIOR TO COMMENCEMENT OF THE WORK.

Coordinate work of the various Sections of specifications to assure efficient and orderly sequence of installation of construction elements, SECTION 01 00 50.00 Page 11 with provisions for accommodating items installed later.

Verify characteristics of elements of interrelated operating interdependent responsibilities for installing, connecting to and placing in service, such equipment.

Coordinate space requirements and installation of mechanical and electrical work, which are indicated diagrammatically on Drawings. Follow routing shown for pipes, ducts, and conduits, as closely as practicable; make runs parallel with lines of building. Utilize spaces efficiently to maximize accessibility for other installations, maintenance, and repairs.

In finished areas (except as otherwise shown,) conceal pipes, ducts, and wiring in the construction. Coordinate location of fixtures and outlets with finish elements.

Execute cutting and patching to integrate elements of Work, uncover ill-times, defective, and non-conforming work, provide openings for penetrations of existing surface and provide samples for testing. Seal penetrations through floors, walls, and ceilings.

1.11 QUALITY OF WORKMANSHIP

The various trades involved in performing the work in this project shall be competent and highly skilled in their particular trade and their qualifications shall meet all State requirements. Only skilled craftsmen in the trades required for the job shall be performing the work. A journeyman level electrician, with a minimum of four (4) years of experience in the trade, currently licensed in the state, shall perform all electrical work. Contractor shall provide a copy of the electrician's certificate of licensing. All work shall result in the highest standards of workmanship, safety and quality. The Contracting Officer may, in writing, require the Contractor to remove from the project any employee deemed incompetent, careless, or otherwise objectionable to personal safety, equipment, or the quality of workmanship.

1.12 AS-BUILT DRAWINGS

As-Built drawings shall be prepared and submitted to the Contracting Officer as indicated on the Submittal Register. As-built drawings reflect work performed by the Contractor that differs from the contract drawings.

It is therefore the Contractor's responsibility to maintain the drawings in the most current status. Copies of the existing as-built drawings, showing the general arrangement of equipment, electrical, and so forth, will be available to the Contractor upon request to be used for additional information that may be required in performing his work. The Contractor shall be aware, however, that existing as-builts show approximate locations only, and actual site conditions may vary from those as shown on existing as-builts.

The Contractor shall maintain a set of redlined full-size, as-built construction drawings marked-up to fully indicate as-built conditions.

These drawings shall be maintained in a current condition at all times until completion of the work and shall be available for review by Government personnel at all times. The location as dimensioned from permanent features, general description, type of material, and approximate depth below finished grade of all new and existing exterior underground utilities encountered shall be indicated. All variations from the contract drawings for the required coordination between trades shall be indicated.

These variations shall be shown in the same general detail utilized in the initial contract drawings. As-built construction drawing shall be furnished to the Contracting Officer for use during the final inspection.

1.13 DAMAGE TO EQUIPMENT

Contractor shall notify the Government inspector/escort or the Missile Combat Crew (MCC) Commander immediately if any damage occurs to equipment during construction operations. The MCC Commander on site will direct all emergency procedures and/or action required after damage to any equipment.

Damage to areas shall be repaired by skilled craftsmen in the trade to the satisfaction of the Contracting Officer or his designated representative.

1.14 NOTICE OF FINAL INSPECTION

The Contractor shall, after notifying the Government inspector/escort, schedule a date with the Contracting Officer for final inspection one (1) week prior to the Contractor's projected completion of the site. A final inspection shall not be held until all discrepancies found during any pre-final inspections have been corrected. The Contractor shall notify and schedule any inspections with the appropriate state agency, and shall be responsible for ensuring that any state personnel who will require entry to the site for the inspection are added to the EAL in a timely manner prior to the date of the inspection. Contractor will not be permitted to work another site until each site has had a Final Inspection and all punchlist items have been completed.

1.15 HAZARDOUS WASTE

Any hazardous waste/material used or generated by the Contractor shall become the property of the Contractor. The use, generation and disposal of all hazardous waste/material will be the responsibility of the Contractor and shall be handled in accordance with all Federal and State hazardous waste and toxic substances control regulations.

1.16 HAZARDOUS MATERIALS

Contractor shall submit prior to contract start Material Safety Data Sheets (MSDS's) expected to be used during contract performance. Hazardous material shall be designed in the latest version of Federal Standard No.

313.

1.17 RECYCLING/REUSE OF EXCESS MATERIALS AND CONSTRUCTION AND DEMOLITION

(C&D) DEBRIS

The Contractor shall reuse/recycle excess materials and Construction and Demolition (C&D) debris to the maximum extent possible. C&D debris are those excess/residue materials resulting from construction or demolition activity that would normally be discarded to a landfill. C&D debris includes, but is not limited to soil, rock, vegetation, concrete, asphalt, and building materials such as windows, wood, metal, and roofing materials. The Contractor shall recycle C&D debris resulting from demolition, which shall become the property of the Contractor unless specified otherwise or as determined by the Contracting Officer.

Materials not to be returned to the Government and all Contractor-generated rubbish, if not recycled, shall be disposed of by the Contractor off site at his own expense, and in compliance with applicable environmental codes and regulations of governing authorities.

Reporting Tonnage of C&D Debris: Contractor shall track and report the tonnage of C&D Debris in two categories.

Tons of C&D that is or will be recycled or reused.

1.1.1 Project Description

The Electric Service at 55 MMIII Sites, consisting of (5) Missile Alert Facilities (MAF) and (50) Launch Facilities (LF), is old, dilapidated and in need of replacement. Thus the work shall include but is not limited to, replacement of the existing underground service conductors along with their associated above ground conductors and accessories at the (5) MAFs* and

(50) LFs. Also included is replacement of the service disconnects located on the service poles at the fence exterior, along with the service laterals at the (5) MAFs*. Additionally, the topside transfer switch shall be replaced at all (5) MAFs. Lastly, at the (50) LFs replacement of the service lateral located outside of the fence shall be included. The work shall also include any and all incidental related work.

*NOTE: MAF O-01 is untypical from the other (4) MAFs in that replacement of the conductors and associated equipment for the LCEB run from the Utility Pole to the LCEB panel are excluded.

1.1.2 Location

The work shall be located at the 5 MAF and 50 LF Minuteman III sites over the area of eastern Wyoming, western Nebraska and northern Colorado with distances from F.E. Warren AFB as shown below. The exact location will be shown by the Contracting Officer.

1.2 OCCUPANCY OF PREMISES

Building(s) will be occupied during performance of work under this Contract.

Before work is started, the Contractor shall arrange with the Contracting Officer a sequence of procedure, means of access, space for storage of materials and equipment, and use of approaches, corridors, and stairways.

1.3 EXISTING WORK

In addition to "FAR 52.236-9, Protection of Existing Vegetation, SECTION 01 11 00 Page 4

Structures, Equipment, Utilities, and Improvements":

a. Remove or alter existing work in such a manner as to prevent injury or damage to any portions of the existing work which remain.

b. Repair or replace portions of existing work which have been altered during construction operations to match existing or adjoining work, as approved by the Contracting Officer. At the completion of operations, existing work shall be in a condition equal to or better than that which existed before new work started.

1.4 DIG PERMITS

No excavation will begin without first obtaining an approved AF Form 103 - Base Civil Engineering Work Clearance Request (aka "dig permit"). The AF Form 103 provides coordination with all Government activities that may have buried cables/structures within the construction area. Items encountered and damaged within three (3) feet on either side of a marked line or around a marked point of items shall be repaired by the Contractor. Items encountered and damaged outside of three (3) feet on either side of a marked line or around a marked point shall be repaired by the Government.

In either case, the Inspector/Escort shall be notified immediately if any damage occurs. The Contractor shall initiate a request for an AF Form 103 at least fourteen (14) calendar days prior to any excavation; contact Missile Engineering at (307) 773-2438 for initiation of permit. Contractor shall review permit, keep signed copy on site and shall be responsible for maintaining utility markings during excavation.

1.4.1 NOTIFY CO

Notify the Contracting Officer at least 72 hours prior to starting excavation work.

1.5 SALVAGE MATERIAL AND EQUIPMENT

All material salvaged is the responsibility of the Contractor. Salvaged materials shall be removed off-site at the end of each day..

Contractor shall maintain property control records for material or equipment designated as salvage. Contractor's system of property control shall be approved by the Contracting Officer. Contractor shall be responsible for storage and protection of salvaged materials and equipment.

The Contractor shall meet with the Contracting Officer and the Real Property Accounting Officer during the Pre Construction Meeting and the Project Closeout Meetings to modify and include any necessary changes to the DD Form 1354. The Contractor shall provide the Interim DD Form 1354 that uses the appropriate division of the RPUIDs/ Category Codes to represent the final constructed facility and include all associated cost.

Coordinate the Contractor's Price and Payment structure with the structure of the RPUIDs/ Category Codes.

Divide detailed asset breakdown into the RPUIDs and related construction Category Codes and populate associated costs which represent all aspects of the work. Where assets diverge into multiple RPUID/ Category Codes, divide the asset and provide the proportion of the assets in each RPUID/ Category Code. Assets and related RPUID/ Category Codes may be modified by the Contracting Officer as necessary during course of the work. Coordinate identification and proportion of these assets with the Government Real Property Accounting Officer.

3.1 GENERAL REQUIREMENTS

Prepare for approval a Project Schedule (3064) to be in accordance with the Progress Report (3065) outlined in Section 01 20 00.00 20. Provide a schedule that is a forward planning as well as a project monitoring tool.

3.1.1 Approved Project Schedule

Use the approved Project Schedule to measure the progress of the work and to aid in evaluating time extensions. Make the schedule cost loaded and activity coded. The schedule will provide the basis for all progress payments. If the Contractor fails to submit any schedule within the time prescribed, the Contracting Officer may withhold approval of progress payments until the Contractor submits the required schedule.

3.2 PROJECT SCHEDULE SUBMISSIONS

Provide the submissions as described below. The reports required for each submission are contained in paragraph SUBMISSION REQUIREMENTS.

3.2.1 Initial Project Schedule Submission

Submit the Initial Project Schedule for approval within 10 calendar days

SECTION 01 32 01.00 10 Page 2 after NTP. The schedule shall demonstrate a reasonable and realistic sequence of activities which represent all work through the entire contract performance period. The Initial Schedule shall be at a reasonable level of detail as determined by the Contracting Officer.

3.2.2 Periodic Schedule Updates

Based on the result of the meeting, specified in PERIODIC SCHEDULE UPDATE MEETINGS, submit periodic schedule updates. These submissions will enable the Contracting Officer to assess Contractor's progress. If the Contractor fails or refuses to furnish the information and project schedule data, which in the judgment of the Contracting Officer or authorized representative is necessary for verifying the Contractor's progress, the Contractor shall be deemed not to have provided an estimate upon which progress payment may be made.

3.3 PERIODIC SCHEDULE UPDATE MEETINGS

Conduct periodic schedule update meetings for the purposes of reviewing the Contractor's proposed out of sequence corrections, determining causes for delay, correcting logic, maintaining schedule accuracy and determining earned value.

3.3.1 Status of Activities

Update information, including percent Complete shall be subject to the approval of the Government prior to the meeting. As a minimum, address the following items on an activity by activity basis during each progress meeting.

3.3.1.1 Percent Complete

Update the percent complete for each activity started, based on the realistic assessment of earned value. Activities which are complete but for remaining minor punch list work and which do not restrain the initiation of successor activities may be declared 100 percent complete.

To allow for proper schedule management, cost load the correction of punch list from Government pre-final inspection activity(ies) not less than 1 percent of the total contract value, which activity(ies) may be declared 100 percent complete upon completion and correction of all punch list work identified during Government pre-final inspection(s).

3.4 WEEKLY PROGRESS MEETINGS

a. Meet weekly with the Government (or as otherwise mutually agreed to) between the meetings described in paragraph PERIODIC SCHEDULE UPDATE MEETINGS for the purpose of jointly reviewing the actual progress of the project as compared to the as planned progress and to review planned activities for the upcoming two weeks. The then current and approved schedule update shall be used for the purposes of this meeting and for the production and review of reports. The Contractor's Project Manager and the Authorized Representative of the Contracting Officer shall attend. The weekly progress meeting will address the status of RFI's, RFP's and Submittals.
