U.S. Bureau of Reclamation is the owner and operator of Folsom Dam and Reservoir. U.S. Bureau of Reclamation is referred to as Reclamation, BOR, or USBR through these specifications depending on the name of the reference document developed by Reclamation. References titled BOR or USBR are to reference documents. All references titled Reclamation are to reference the agency.
1.1.2 Central California Area Office (CCAO)
Central California Area Office is referred to as CCAO throughout the specifications. CCAO is an area office of Reclamation's California-Great Basin Region 10, and the operator of Folsom Dam and Reservoir.

1.4 SUBMITTALS
Government approval is required for submittals with a "G" designation;
SECTION 01 00 00 Page 5 submittals not having a "G" designation are for information only. When used, a designation following the "G" designation identifies the office that will review the submittal for the Government. 1.5 SITE USE CONDITIONS
1.5.1 Workdays And Hours
Regular workdays are Monday through Friday, excluding Government holidays.
Normal work hours are between the hours of 7:00 a.m. and 6:00 p.m. local time subject to conformance with Folsom Municipal Noise Standards.
Operations producing high-intensity impact noise must meet requirements specified in Section 01 14 01 WORK AND SITE ACCESS AND RESTRICTIONS. The Government anticipates that the Contractor will perform work outside normal work hours and is prepared to approve work outside normal work hours subject to meeting the requirements in paragraph WORK OUTSIDE NORMAL HOURS.
Limit the number of haul trucks and privately owned vehicles that need to be screened between 6:00 a.m. to 8:00 a.m. and 3:30 p.m. to 5:30 p.m to prevent overflow traffic into the Folsom Dam complex, creating traffic congestion at Folsom Dam Road and Folsom Lake Crossing. Typically, it takes 30 minutes to screen each multi-axle vehicle (haul truck) and 15 minutes to screen a Contractor owned vehicle. The screening time varies based on the weather on a given day. Typical screening times are based on the availability of canine and handler; screening by Sheriff without screening canines could take twice as long. During a typical day, a screening canine can screen about 50 Contractor vehicle or 18 haul trucks.
Beyond that, the canine services are less effective and screening may need to be performed by Sheriffs. On very hot days, the screening canines can screen about 75 percent of the number of vehicles they could normally screen. The scheduled hours for truck loading, unloading, and hauling operations must follow State, local, and City ordinances.

1.5.2 Work Outside Normal Hours
Work outside normal Monday through Friday working days and hours requires justification and Contracting Officer approval. Submit application 30 calendar days prior to such work to allow arrangements to be made by the Government for approval and allow Reclamation and the Government to inspect the work in progress.
Work either started, or scheduled to be finished, or both, outside normal work hours are permitted subject to conformance with the Folsom Municipal Code noise standards, and subject to approval of a work plan for work outside normal hours. During periods of darkness, the different parts of the work must be lighted in a manner approved by the Contracting Officer.
1.5.3 Cleaning
Comply with Federal, State, local laws and regulations, and EM 385-1-1.
Conform to most stringent requirement in cases of conflict between specifications, regulatory requirements, and EM 385-1-1.
Before bringing on site, clean construction equipment to remove dirt, vegetation, and other organic material to prevent introduction of noxious weeds, and invasive plant and animal species. Cleaning and inspection of equipment and vehicles shall meet requirements of Bureau of Reclamation "Inspection and Cleaning Manual for Equipment and Vehicles to Prevent the Spread of Invasive Species" availible at:

Cease work in areas where waste is hazardous until waste materials are investigated by the Government. If waste is hazardous meeting the definition of 40 CFR 261.3, the Contracting Officer can order delays in time of performance or changes in work, or both. If such delays or changes are ordered, an equitable adjustment will be made in the Contract following applicable clauses of the Contract.
1.5.4 Other Site Restrictions
Normal Reclamation project operations will not be altered for construction activities.
Discharge from the outlet works and penstocks will be required throughout the year.
1.5.4.1 Lake Level Fluctuation
Refer to Section 01 14 01 WORK AND SITE ACCESS AND RESTRICTIONS for historical lake level data and operational information. Contractor assumptions of lake levels lower than the operated levels described above are at Contractor's sole risk.
1.5.4.2 Main Dam Spillway Bridge
Refer to Section 01 35 26 GOVERNMENTAL SAFETY REQUIREMENTS paragraph EQUIPMENT LOADING DIAGRAM for Main Dam spillway bridge loading limits.

1.5.4.4 Safety
The Contractor must comply with the requirements of USACE EM 385-1-1, and applicable federal, state, and local laws, ordinances, criteria, rules and regulations. Safety requirements are provided in Section 01 35 26
GOVERNMENTAL SAFETY REQUIREMENTS.
1.6 SECURITY
Folsom Dam and facility is designated as a National Critical Infrastructure (NCI) facility according to Presidential Decision Directive 21 (PDD-21).
Security requirements are defined in Section 01 35 50 SITE SECURITY.
1.6.1 Site Access
Entrance to the project site is limited to authorized personnel and vehicles only. Contractor employees will be given access only to those areas where they have been approved to work. No other site access is allowed. The Contractor will not be given keys to any of the facilities.
Reclamation and the Sacramento County Sheriff's Department maintain a security post and tower as shown on the drawings. Do not impede staff access.
1.6.2 Controlled Access Areas
Areas of the federal lands at Folsom Reservoir are controlled access. Some areas have more restrictive controls than other areas. Contractor personnel must always be badged or escorted while in these areas.
1.6.2.1 Critical Areas
Critical areas are identified as a Control Access Area requiring Personnel Identification Verification (PIV) or cards and successful completion of background investigation for unescorted access.
a. Critical Areas includes the following:
(1) Tainter (spillway) gates and controls including the area immediately upstream on the reservoir side.
(2) Anti-Scaling gate and equipment at Wing Dam and Main Dam interface.
(3) Crest of the Wing Dams.
(4) Auxiliary Spillway Control Structure.
b. Unescorted Access
Contractor must designate individuals requiring unescorted access (escort privileges) into Critical Areas. Those individuals are subjected to a Tier 2 background investigation by the Office of Personnel Management (or equivalent).
c. Escorted Access
Other Contractor personnel can enter Critical Areas if they have been issued an approved Contractor's badging system picture identification
SECTION 01 00 00 Page 8 badge and are escorted by an approved escort. Escorted Contractor personnel must be monitored continuously by approved Contractor escorts at a ratio of escorts to non-escorts identified in Section 01 35 50
SITE SECURITY.
1.6.2.2 Non-Critical Areas
Non-critical areas include the Industrial Area including the maintenance shops and warehouse, and other areas not specifically identified as Critical Areas.
1.6.3 Personnel Identification Badges
A periodic check must be made by the Contractor's Security Manager on the status of these badges. Each Contractor's superintendent or foreman must be responsible for monitoring the status of each badge issued to their employees. Only actively employed personnel must retain a badge.
1.6.3.1 Personnel Entry Approval
Delay or failure to obtain entry approval will not affect the Contract price or time of completion.
1.6.3.2 Contractor Vehicles
The Contractor is responsible for making sure that construction equipment and delivery vehicles, including haul trucks, are free of mud, soil and debris when leaving the site and entering the public highway. Required tracking control best management practices are provided in Section 01 57 23
TEMPORARY STORM WATER POLLUTION CONTROL.
1.7 GENERAL REQUIREMENTS RELATED TO OPERATION OF FOLSOM DAM
Conduct operations so as not to cause interference with normal operations and maintenance of the Folsom Dam Facility.
1.7.1 Reservoir Operations
Folsom Dam will remain in operation during the entire construction period.
Reservoir operations will not be adjusted to accommodate construction needs. No construction activity must interfere with reservoir operation.
Notify the Contracting Officer if any construction activity will potentially interferes with operation of Folsom Dam.
1.8 DELIVERY, STORAGE AND HANDLING
Transport and handle manufactured products following with manufacturer's instructions. Remove and replace damaged items with new items. Protect materials subject to adverse effects from moisture, sunlight, ultraviolet light, or weather during storage at jobsite. Store curing compounds, sealants, adhesives, paints, coatings, sealers, joint compounds, grouts, and similar products at the temperature and environmental conditions recommended by manufacturer.
1.9 SCRAP MATERIAL
Materials specified to be removed and become the property of the Contractor are designated as scrap. The Contractor, by signing this Contract, hereby acknowledges that they have made due allowance for value, if any, of such

1.10 WRITTEN GUARANTEES AND GUARANTOR'S LOCAL REPRESENTATIVE
Prior to completion of the Contract, obtain and furnish to the Contracting Officer's representative written guarantees for equipment / appliances furnished under the Contract. Furnish with each guarantee: The name, address, and telephone number of the guarantor's representative nearest to the location where the equipment / appliances are installed, who, upon request of the Using Service's representative, will honor the guarantee during the guaranty period and will provide the services prescribed by the terms of the guarantee.
1.11 PRICING OF CONTRACTOR-FURNISHED PROPERTY
At the request of the Contracting Officer, promptly furnish and must cause any subcontractors to furnish, in like manner, unit prices and descriptive data required by the Government for property record purposes of fixtures and equipment furnished and installed by the Contractor.
1.12 EXISTING UTILITIES
Immediately notify the Contracting Officer in writing if utilities not shown on the plans are encountered within the construction limits of the entire project. If the encountered utilities will interfere with construction operations, the Contracting Officer will determine the necessity for removal or relocation. If such utilities are removed or relocated as directed by the Contracting Officer, an equitable adjustment in the Contract price must be made pursuant to the Contract Clause CHANGES to cover the cost of any additional pertinent work or delay.
It is anticipated that pole lines, signs, pipelines and private improvements that would interfere with or are to be replaced by new construction will be removed to new locations by the owners (or by the State of California or its agents) before construction operations. Notify the Contracting Officer at least 30 calendar days in advance of the date on which work will be started requiring the removal of such utilities or private improvements. Take care to preserve and protect any existing utilities and improvements from injury or damage during construction operations. Assume full responsibility for reimbursing the owners for any damage to their properties, utilities, or improvements, or interference with their services caused through construction operations. If such damage be found to have been caused without the Contractor's fault or negligence an equitable adjustment in the amount due under the Contract will be made under the applicable CONTRACT CLAUSES.
1.13 TEMPORARY UTILITIES FOR CONSTRUCTION
Make arrangements for, and be responsible for costs of: power, power distribution, potable water, sanitary facilities and waste disposal, communication, and heating fuel and fuel storage, in a manner satisfactory to the Contracting Officer. Make the arrangements with the utility owners (such as City of Folsom, AT&T, SMUD) for usage, distribution and/or connection, metering and direct payment.
During the construction period, at the Contractor's expense and subject to the Contracting Officer, the Contractor must provide and maintain the necessary temporary connections, distribution lines, and meter bases required to measure the amount of each utility used, for the purpose of
SECTION 01 00 00 Page 10 determining charges. The utility companies will provide and install the meters and make the final hot connection after inspection and approval of the Contractor's temporary utility installation. The Contractor must not make the final electrical connection.
Remove temporary utilities and restore site after completion of the contract.
1.13.1 Water For Construction Purposes
The Contractor is responsible for obtaining water for construction purposes and providing means of conveying water to the point of use. Obtain water from offsite municipal or commercial sources or possibly from Folsom Reservoir as approved by US Bureau of Reclamation and USACE Contracting Officer. The contractor is responsible for permitting, contracts, infrastructure and cost associated with obtaining water.
If water is obtained from Folsom Reservoir, the contractor is subject to the following restrictions and requirements:
a. Up to two locations can be used for water removal from Folsom Reservoir.
b. The locations are subject to the approval of the Contracting Officer.
the contractor shall coordinate with the Government Contracting Officer and Government Environmental Manager and Archeologist for clearance and appropriate documentation before the sites could be used.
c. Meter the volume of water used from the reservoir with totaling meters.
d. Follow the guidelines and criteria described in Section 01 57 19 Paragraphs Prevention of Fish Entrapment, Screen Construction Criteria and Water for Construction Activities.
i. Report the amount of water draw from Folsom Lake monthly to the Bureau of Reclamation Central California Area Office.
Water for use in concrete, grout, shotcrete and other permanent work must meet the specific water quality requirements for that application. Water obtained from municipal sources will be at the Contractor's expense.
1.13.2 Temporary Electrical Light And Power
1.13.2.1 Safety
Provide construction power facilities in accordance with the safety requirements of the NFPA 70 and EM 385-1-1. The Contractor, or their delegated subcontractor, must enforce the safety requirements of electrical extensions for the work of subcontractors. Work must be accomplished by skilled electrical tradesmen in a workmanlike manner, as approved by the Contracting Officer.
1.13.2.2 Temporary Light And Power
Provide temporary light and power service as required for the work and to inhibit vandalism. Provide safety switches and wiring into buildings and required extension cords, lighting outlets, power outlets (grounded type), lamps and other equipment and accessories necessary for adequate temporary lighting and power for construction purposes. Remove temporary lighting
SECTION 01 00 00 Page 11 and power and their connections at completion of the work.
1.13.2.3 Construction Equipment
In addition to the requirements of EM 385-1-1, temporary wiring conductors installed for operation of construction tools and equipment must be either Type TW or THW contained in metal raceways, or may be multiconductor cord.
Temporary wiring must be secured above the ground or floor in a workmanlike manner and must not present an obstacle to persons or equipment. Open wiring may only be used outside of buildings, and then only following the provisions of the National Electrical Code.
1.13.2.4 Circuit Protection
In addition to the present requirements in EM 385-1-1 and NFPA 70, 15- and 20-ampere receptacle outlets used for obtaining power during construction must have ground fault circuit interrupters (GFCI) for personnel protection. Block and brick saws must also be equipped with GFCI. The Contracting Officer may allow an exception to this requirement for circuits for concrete vibrators or circuits operating at other than 60 Hertz normal (in both cases an assured grounding program as described in NFPA 70, except utilizing the daily inspection frequency of the grounding means of such equipment, may be permitted). The assured grounding program must not be permitted as a substitute for usage of GFCI'S except as described above.
Generator-powered 15- and 20-ampere, 60 Hertz receptacle outlets must have GFCI'S, and be properly grounded. A testing means must be provided that impose a measured fault of 5 milliamperes, plus or minus 1 milliamperes, and result in tripping the GFCI unit.
1.14 SURVEYING
The Government has established primary control to be used for establishing work lines and grades. Primary control consists of benchmarks and horizontal control points in the work vicinity. The Government will provide complete listing and identification of primary control within 30 days after issuance of Notice to Proceed. The Contractor must check and verify primary control and resolve discrepancies with Government before beginning work. Preserve and maintain primary control points until otherwise determined. Government can reestablish damaged or destroyed primary control points and backcharge reestablishment cost to the Contractor.
1.14.1 Protection of Monuments, Stakes, and Marks
Preserve and protect all survey monuments and related marks. When removal is necessary, accurately reference the monuments or related marks, subject to the approval of the Contracting Officer.
a. Replace and reset all survey stakes, control points, monuments, benchmarks, or reference stakes disturbed or destroyed during the work to the satisfaction of the Contracting Officer at Contractor's expense
b. Reset primary or secondary control monuments removed as soon as the work requiring the removal is complete. Alternatively, set other control points so as to reestablish the control network.
c. Recheck the position of monuments, control points, or other marks that are subject to movement due to the passage of equipment or other forces at regular intervals, but not less than monthly and submit results to
SECTION 01 00 00 Page 12 the COR.
1.14.2 CONTRACTOR SURVEY DATA
Submit Surveying Plan describing work layout and surveying method for each separate feature, including a surveying schedule. Project surveys must be performed by a licensed surveyor in the State of California following EM 1110-1-1005. Prior to commencement of survey work, submit in writing the name and license number of the person who will perform the required surveys. Surveys must provide detailed topographic information including, but not limited to, elevations and locations of embankment hinge points, edge of pavement and aggregate surfacing, slopes, and other surface features. The accuracy of all surveys must be third order or better.
Include accuracy checks of Government-established primary controls.
a. Pre-Construction Existing Embankment Survey
Survey cross sections before construction begins along the entire work limits. These pre-construction survey cross sections must be used to reflect existing ground before any construction activities begin and must also be used as the original ground surveys when checking measurement for pay quantities as described in Section 01 20 00 PRICE AND PAYMENT PROCEDURES. Cross sections must be surveyed perpendicular to the dam centerline alignment line shown on the drawings. Take the Pre-Construction Survey Cross Sections at maximum 50 foot intervals, and at location of any geometric change. In addition, perform a profile survey of the top of the existing dam crest and adjacent abutments.
Cross sections along the dam centerline alignment shown on the drawings must include the entire embankment where the existing embankment will be excavated and reconstructed and continue out to a point at least 10 horizontal feet past the completed work on both the upstream and downstream slope. Elevations must be taken at inflection points but at not more than 10-foot intervals to the nearest 0.1 feet. Horizontal locations must not deviate from the alignment of the cross-section line by more than 1 foot. The survey data must be provided to the Contracting Officer on Compact Disk containing survey control and cross section points defined in the form of Northing and Easting coordinates and their elevations (for example X, Y, and Z).
Provide the Contracting Officer the survey data files, one set of the cross section data, plotted as viewed from the right abutment toward the left abutment, with a 5 horizontal by 2 vertical grid, on a standard D size drawing sheet, at a scale of 1:10, horizontal and vertical. Prepare any needed layout drawings. Standard cross section plotting paper with pre-printed red grid is not acceptable.
b. During Construction Embankment Survey
Submit to the Contracting Officer survey cross sections performed after completion of dam crest excavation and other project excavations to serve as a basis for measurement and payment quantities of activities related to activities in Section 31 23 16.01 EXCAVATION FOR LEFT AND RIGHT WING DAM. Cross sections must be taken at the same locations as the pre-construction embankment survey.
c. Post-Construction Embankment Survey
Submit to the Contracting Officer survey cross sections performed after
SECTION 01 00 00 Page 13 completion of embankment fill placement and compaction to serve as a basis for measurement and payment quantities of activities related to activities in Section 35 73 13.01 EMBANKMENT FOR EARTH DAMS FOR LEFT AND RIGHT WING DAM. Cross sections must be taken at the same locations as the pre-construction embankment survey.
1.14.2.1 Field Notebooks
In addition to Contract Clause 52.236-17, LAYOUT OF WORK, licensed professional land surveyors must perform Contractor surveys. Submit waterproof Survey Field Notebooks written in a legible, sequential manner.
No erasures are to be made. Record original field notes, computations, and other surveying data in fieldbooks. Record survey data in accordance with recognized professional surveying standards. Field notes must be reduced and checked with each page initialed by the reviewer. Initial reductions must be made in black pencil; corrections by reviewing personnel must be in red pencil. Corrections by ruling or lining out errors will be permitted.
An electronic data collection device can be used, provided that the resulting field data is submitted on 8 1/2- by 11-inch paper. Written data collector information must contain the offset distance from the location line/preliminary line/base line, the elevation reading, the station, and a brief feature description. Label information to indicate each facet of work and include all computations and coordinates. Each notebook must bear the signature and seal of a registered land surveyor following the statement: "I certify that the data in this field notebook has been reviewed and checked and is true and correct."
Rejection of part or all of a fieldbook can necessitate resurveying.
1.14.2.2 Data Storage
Submit Surveying Electronic Data optical disc using the current version of Microsoft Windows© Operating System. The file must contain coordinates, elevations, and station values relating to control points on the location line/preliminary line/base line and cross sections along the line. The information must be IBM PC compatible, and presented in the American Standard for Information (ASCII) format. Data on the disk must conform to the following format:
a. An 80-column format with 15 columns devoted to Northing coordinates, Easting coordinates, elevations, and location line/preliminary line/base line station values. Reserve the remainder of the 80 columns for additional designations.
b. Show the Northing (Y) and Easting (X) coordinates and elevation values to three places to the right of the decimal and be right justified.
c. Enter a point identification abbreviation in the field immediately right of the station value. Typical points and identifications are angle point (AP); back tangent (BK TAN); forward tangent (FWD TAN);
angle split (A SPLT); point of curve (PC); point of tangent (PT); and other significant features such as top of rock, centerline of road, railroad bridge, edge of pavement, corner of building, and similar locations.
1.15 PLANNED UTILITY OUTAGES AND STREET CLOSURES
Utility outages and street closures to be of as short a duration as possible and to be scheduled as far in advance as possible with the

Contracting Officer, in no case less than 21 days before the outage or closure. Obtain from the Contracting Officer in writing a statement or schedule giving the permissible times of outages or closures for particular installations and the maximum time allowed for each. Strictly observe such schedules and the Contractor will be held responsible for any violations.
Power outages to be restricted to off-duty hours and weekends. Other utilities can be connected and streets closed during normal working hours with the outage duration and street closure as short as possible. Outages and street closures are to be scheduled as far in advance as possible with the Contracting Officer and in no case less than 21 days before the outage or closure.
1.16 GENERAL SAFETY REQUIREMENTS
1.16.1 General
For information regarding specific Safety Requirements see Section 01 35 26 GOVERNMENTAL SAFETY REQUIREMENTS. Refer to EM 385-1-1, (see Contract Clauses,, ACCIDENT PREVENTION) and the Occupational Safety and Health Act (OSHA) Standards for Construction 29 CFR 1926; General Industry Standards 29 CFR 1910; and the National Fire Protection Association Codes are applicable to this Contract. In case of conflict the most stringent requirement of the standards is applicable.
1.16.2 The Prime Contractor's Superintendent
The Prime Contractor's superintendent must take an active role in enforcing the safety requirements by participation in safety conferences, hazard analysis, tool box meetings, walk-through inspections, correction of violations, and similar tasks, and including that of the subcontractor's work.
1.16.3 Project Coordinator
Contractor must appoint a Project Coordinator who will serve as the prime Contractor's main point of contact for ensuring information on day-to-day tasks between Main Dam, Left Wind Dam, and Right Wing Dam are understood, coordinated, and potential conflicts are mitigated. The Project Coordinator must have, at minimum, 5 years of project management experience on large projects involving more than one prime Contractor. Experience must also indicate a working knowledge of the federal Government changes clause, pricing, cost and pricing data, P6 scheduling experience related to fragmentary network development and how to effectively plan, scope, and effectively negotiate potential changes on behalf of the project team.
Duties must include effectively manage, control, and coordinate sub-contractor's work related to potential change orders. Project coordinator must be a full-time position, not a shared or side duty.
1.16.4 Violations
If recurring violations or gross violation indicate that the safety performance is unsatisfactory, corrective action must be taken as directed, and at the discretion of the Contracting Officer the retention or some part thereof can be withheld from the progress payment until corrective action has been completed.

1.16.5 Elevated Work Areas
Workers in elevated work areas in excess of 6 feet above an adjoining surface require special safety attention. In addition to the provisions of EM 385-1-1, the following safety measures are required to be submitted to the Contracting Officer's Representative. Prior to commencement of work in elevated work areas, submit the Fall Protection and Prevention Plan as noted in Section 01 35 26 GOVERNMENTAL SAFETY REQUIREMENTS, depicting all provisions of their positive protection system including, but not limited to, details of guard rails.
1.16.5.1 Protection
Positive protection for workmen engaged in the installation of structural steel must be provided by safety nets, tie-offs, hydraulic man lifts, scaffolds, or other required means. Walking on beams or girders is prohibited without positive protection.
1.16.5.2 Guard Rails
Perimeter guard rails must be installed at surfaces or openings more than 6 feet above an adjoining surface and on perimeters. Rails must be designed to protect all phases of elevated work. Rails around elevated surfaces must not be removed until the permanent protection shown on the drawings is installed. Rails must be designed by a licensed engineer to provide adequate stability under any anticipated impact loading. As a minimum, the rails must consist of a top rail at a height of 42 inches, a mid-rail and a toe board.
1.16.6 Fire Prevention
Cutting or welding will be permitted only in areas that are or have been made fire safe. Where possible, combustibles must be located at least 35 feet horizontally from the work site. Where such location is impracticable, combustibles must be protected with fire blankets and/or protective welding screens to prevent slag from running out of the work area. Edges of covers must be tight to prevent sparks from going under them. This precaution is also important at overlaps where several covers are used to protect a large pile. Do not allow any welding/cutting or open flame operations in an area of the facility that is protected by an automatic detection system, if the system is out of service. First priority of work must be to return the detection system to operational condition. Return the fire detection system back to an operational status (if possible) during periods that the facility is unoccupied, and at the end of the work day. Post a fire guard for a 24-hour period (or certify to the Fire Department that the facility is safe) after welding, cutting, and open flame operations in a facility when: (a) fire detection system cannot be returned to service; (b) fire detection system does not exist. Other fire prevention precautions must follow the latest National Fire Codes.
1.16.7 Inspections
Construction sites are subject to fire and safety inspections without notice. Any violation of fire and safety standards will result in a work stoppage at the expense of the Contractor.
1.16.8 Recordkeeping / Reporting Requirements
Record and report accident exposure and experience incident work. (This
SECTION 01 00 00 Page 16 includes exposure and experience of the prime Contractor and sub-contractor(s)). As a minimum these records must include exposure work-hours and a log of occupational injuries and illnesses. (OSHA Form 300 or state equivalent as prescribed by 29 CFR 1904). Refer to EM 385-1-1, 01.D.04.
1.16.9 Accident Reporting
In addition to the requirements for reporting accidents following EM 385-1-1, Section 1, submit at the 50 percent point and 100 percent of project completion, a written summary of worker's compensation claims filed by workers on the project. The report must include subcontractors. The main report covering the Prime Contractor claims must be certified as "correct and true" by the Contractor's compensation insurance carrier. The same certification is required for subcontractor reports.
In addition, report near misses on SPK FORM 385-1, Immediate Mishap Notification. Include incidences where procedures or controls were changed due to recognition of conditions or circumstances that could inflict injury or property damage if the cause is not corrected.
1.16.10 Reporting Damage, Spills and Security Issues
Any of the following must immediately be reported to US Bureau of Reclamation (Reclamation) personnel with simultaneous notification to the Contracting Officer. The emergency contact for Reclamation personnel will be provided to the Contractor prior to the start of construction:
a. Any damage including suspected or potential damage to existing facilities including but not limited to utilities.
b. Any spills involving materials of a type and quantity which could cause a State Water Pollution Prevention Program violation per Section 01 57 23 TEMPORARY STORM WATER POLLUTION CONTROL or which cause a delay in access / egress or impede construction or public traffic.
c. Any security breach with the exception of individuals who inadvertently and momentarily enter the edges of a zone they are unauthorized to enter provided such individuals immediately exit such zone upon immediate notification from security staff and they are and were, at all times under the observation of security staff while in the unauthorized zone.
1.16.11 Emergency Contacts
Prior to starting field work under this Contract, provide to the Contracting Officer's Representative, the names and contact information of at least two individuals, both of whom have the authority and capability of acting on behalf of the Contractor and either of whom is capable of being at the site within one hour of contact. Each contact must be capable of moving equipment, accessing any part of the work area and representing the Contractor to emergency responders. Availability must be twenty-four (24) hours per day seven (7) days a week (24/7) including holidays.
1.17 TIME EXTENSIONS FOR UNUSUALLY SEVERE WEATHER
1.17.1 Time Extension Determination Procedure
This provision specifies the procedure for the determination of time
SECTION 01 00 00 Page 17 extensions for unusually severe weather following the CONTRACT CLAUSE, entitled "DEFAULT (FIXED-PRICE CONSTRUCTION)".
In order for the Contracting Officer to award a time extension under this clause, the following conditions must be satisfied:
a. The weather experienced at the project site during the Contract period must be found to be unusually severe, that is, more severe than the adverse weather anticipated for the project location during any given month.
b. The unusually severe weather must actually cause a delay to the completion of the project. The delay must be beyond the control and without the fault or negligence of the Contractor.
1.17.2 Adverse Weather Delay Workdays
The following schedule of monthly anticipated adverse weather delays is based on National Oceanic and Atmospheric Administration (NOAA) or similar data for the project location and will constitute the base line for monthly weather time evaluations. The progress schedule must reflect these anticipated adverse weather delays in all-weather dependent activities.

1.17.3 Record
Upon acknowledgement of the Notice to Proceed (NTP) and continuing throughout the Contract, the Contractor must record on the daily CQC report, the occurrence of adverse weather and resultant impact to normally scheduled work. Actual adverse weather delay days must prevent work on critical activities for 50 percent or more of the scheduled workday. Refer to ER 415-1-15.
1.18 EQUIPMENT DATA FORM
In conjunction with paragraph, UAI 31.105-101 EQUIPMENT OWNERSHIP AND OPERATING EXPENSE SCHEDULE, submit CESPK FORM 450 for necessary equipment to perform work requiring adjustment of Contract price and must submit these forms with the modification proposals. A sample form is attached at the end of this Section.
1.19 SOIL DENSITY TEST (USING METERS CONTAINING RADIOACTIVE MATERIALS)
If the Contractor proposes to use meters containing radioactive materials to obtain unofficial results for their own convenience, adhere to the following requirements:
a. Following 06.E of EM 385-1-1, obtain a service permit to use, store, operate, or handle a radiation producing machine or radioactive materials on the project site. Obtain the service permit from the
SECTION 01 00 00 Page 18 appropriate through the Contracting Officer. The Contractor must notify the Contracting Officer during the pre-work conference if a radiation producing device will be utilized on the project site in order to determine the permit application requirements, and allow a lead time of 45 days for obtaining a permit.
b. The Contractor is responsible for providing a copy of any Nuclear Regulatory Commission (NRC) licenses per the Code of Federal Regulations 10 CFR "Energy" for radioactive sources brought onto the project site by the Contractor or subcontractors. Provide these licenses to the Contracting Officer before the radioactive sources is allowed on the project site.
1.20 SPECIAL CONSTRUCTION PROCEDURES
1.20.1 Cooperation With Others
During the life of this Contract, other Contractors will be performing operations in the general area where work under this Contract is being performed. The Contractor is to cooperate with others wherever necessary for the better prosecution of the work. As far as practicable, persons working in the vicinity are to have equal rights to the use of transportation facilities and grounds within the limitations specified in the CONTRACT CLAUSES. The obligation of the Contractor under this Contract includes jointly planning and scheduling the work, on a cooperative basis, with other Contractors in order to minimize delays and interferences. The Contractor is to arrange operations as to not interfere with other work in progress. In case of dispute or disagreement regarding use of transportation and storage facilities and rights of access, the decision of the Contracting Officer will govern.
1.20.1.1 Anticipated Concurrent Construction Activities
The following are anticipated construction activities that would need Contractor coordination. This is not an exhaustive list. The Contractor must coordinate with the Contracting Officer's Representative for additional activities encountered.
a. Right Wing Dam Toe July 2023 to December 2023.
b. Folsom Auxiliary Spillway Control Structure May to October 2024.
c. Temperature Control Shutters March 2024 to February 2026.
d. Folsom Auxiliary Spillway Control Structure May to October 2025.
1.20.2 Construction Emergency Action Plan
