ADDENDUM TO FAR 52.212-1, INSTRUCTIONS TO THE OFFRORS – COMMERCIAL ITEMS.
GENERAL INSTRUCTIONS:

2. SITE VISIT: A site visit is scheduled for Tuesday, July 25, 2017 at 0900(CET). For additional information, see FAR Clause 52.237-1, Site Visit (APR 1984).
Prospective quoters interested in attending the site visit must notify the above point of contact via e-mail, no later than 24 July 2017 at 1000 (CET) in order to attend the site visit.
Unless already authorized to access the facilities, site visit attendees will be required to obtain a vehicle access for each location. Visitors may be required to present the following information: (1) vehicle registration, (2) valid driver’s license, (3) or car rental agreement (if applicable). Attendees are cautioned that the Government will not wait for anyone who is late or lost and may not hold another site visit.
3. All questions must be received no later than 28 July 2017 at 1500 (CET). All questions shall be submitted via e-mail to: latoya.orr@dla.mil, Contract Specialist; cc: jacqueline.verrine@dla.mil, Contracting Officer.

QUOTE PREPARATION INSTRUCTIONS:
1. Technical Capability. Quoters shall address each of the Technical Capability subfactors:
a. Qualifications: Quoters are advised that DFARS 252.222-7002, Compliance with Local Labor Laws (Overseas)(JUN 1997), applies to the solicitation and any resultant contract. The quoter shall demonstrate that it has the necessary facilities, equipment, contacts, etc., to perform at the specified locations within the required timeframe. Quoters shall also provide a valid copy of their business registration.
b. Equipment, Tools, and Vehicles: The quote shall include a list of the equipment, tools, and vehicles to be used in the performance of this requirement.
c. Safety and Security: The quoter shall submit one (1) copy of its organizational health and safety policy. The quoter shall provide certification that all employees have been properly trained on the correct use of all Personal Protective Equipment (PPE) to be used in performance of the work specified in the PWS.
The quoter shall submit substantiating documentation that demonstrates that all employees requiring access to the military installations identified in the PWS have, or will have submitted for local background checks as required by the PWS and that all background checks will be completed prior to commencement of the base performance period.
2. Past Performance. The quoter shall provide recent and relevant past performance in providing grounds and minor maintenance services. Recent past performance must be work performed for a duration of at least one (1) year of continuous performance within the last five (5) years of the solicitation issue date. Relevant past performance is work performed for “same or similar” services based on size, scope, complexity, and contract type.
a. Quoters shall provide the following information for each Past Performance submission:
-Title of the contract/project.
-Description of work completed.
-Name of Organization/Requiring Activity and a Point of Contact (POC).
-Length of contract/project (year/month/date – year/month/date).
-Dollar value of contract/project.
-Complete name, e-mail and phone number of POC at the Organization/Requiring Activity.
b. The Government may send a Past Performance Questionnaire to the POC for each relevant contract.
3. Price. Quoters shall submit complete pricing for the base and option year periods of performance identified above.
a. Price shall be in local currency (Euro).
b. Any escalation in option year pricing shall be identified.
QUOTE SUBMISSION INSTRUCTIONS:
1. Quotes must be received no later than the closing date and time specified in Block 8 of the solicitation.
Quoters shall submit signed, dated quotes to the following addresses: latoya.orr@dla.mil, Contract Specialist;
cc: jacqueline.verrine@dla.mil, Contracting Officer. Quotes shall be submitted via e-mail and ONLY electronic submissions will considered. Electronic files should not exceed 5MB. Confirmation of submittal prior to the RFQ closing date and time is quoter’s responsibility. Late quotes may not be considered for award.
2. The quote shall include the name(s), telephone numbers, and e-mail addresses of the person(s) authorized to conduct discussions/negotiations and to financially bind the company.
3. Quoters shall submit a signed first page of the solicitation (SF1449) and each amendment (SF30), if any.

ADDENDUM TO FAR 52.212-2, EVALUATION – COMMERCIAL ITEMS.
This addendum is provided in lieu of FAR Clause 52.212-2. The Government anticipates award of a firm-fixed-priced contract. Award will be made using the Lowest Price Technically Acceptable source selection process defined in FAR 15.101-2.
The Government intends to award without discussions. However, the Government reserves the right to conduct discussions if determined necessary. Therefore, quoters are advised that quotes submitted should include best and final pricing.
All non-price factors shall be evaluated on an acceptable/unacceptable basis. In order to be considered for award, there must be an “acceptable” rating in every non-price factor/subfactor. In other words, a rating of “unacceptable” for any factor or subfactor will result in an overall rating of “unacceptable” and the quoter will be ineligible for award. The following ratings shall be used to evaluate quotes:
Factor I – TECHNICAL CAPABILITY:
Technical Capability Ratings Acceptable: Quote meets the requirements of the solicitation.
Unacceptable: Quote does not meet the requirements of the solicitation.
FACTOR II - PAST PERFORMANCE:
Past Performance Ratings Acceptable: Based on the Quoter’s performance record, the Government has a reasonable expectation that the Quoter will successfully perform the required effort, or the Quoter’s performance record is unknown (See note below).
Unacceptable: Based on the Quoter’s performance record, the Government has no reasonable expectation that the Quoter will be able to successfully perform the required effort.
NOTE: Quoters are authorized to provide information on any problems encountered on the identified contracts as well as any corrective action taken. The Government may obtain past performance information from sources other than the references provided. In the case of an quoter without a record of relevant past performance or for whom information on past performance is not available or so sparse that no meaningful past performance rating can be reasonably assigned, the quoter may not be evaluated favorably or unfavorably on past performance (see FAR 15.305(a)(2)(iv)). Therefore, the quoter shall be determined to have unknown (or “neutral”) past performance. In the context of acceptability/unacceptability, a neutral rating shall be considered “acceptable.”
FACTOR III – PRICE: Price does not receive an adjectival rating. The Government will evaluate each price using various techniques found in FAR 15.404-1 to determine that the price quoted is fair and reasonable.
a. Unrealistically Low Prices: Prices quoted may be reviewed to determine whether they reflect a lack of technical understanding of the requirements and to assess the inherent risk in accepting an unrealistic quoted price. Unrealistically low quoted prices, initially or subsequently, may be grounds for rejection for this solicitation.
b. Option to Extend the Term of the Contract: As part of the price evaluation, the Government will evaluate its option to extend the term of the contract (see FAR Clause 52.217-5 and 52.217-9).

1.0 INTRODUCTION: The Contractor shall provide non-personal services for labor and materials to support grounds keeping, minor maintenance & facility services as described and annotated within this Performance Work Statement (PWS). The work described herein includes the execution of services for DLA activities at Kleber Kaserne on premises and around Buildings 3209, 3222, 3224, 3227, 3228, 3229, 3230, Rhine Ordnance Barracks (ROB) building 320 and Sembach building 214. Based on four years of past performance, the monthly labor hours expended to satisfactorily complete services as detailed in subject PWS averaged 130 hours per month. The time expended on successful completion of all tasks is an estimate and is in no way a determination of the time required. Contractors shall provide a quote that will enable successful completion of all tasks as detailed in the PWS to the satisfaction of the US Government. Contractor shall be responsible for the management of their time expended in order to successfully complete all tasks.
2.0 SCOPE OF WORK: The following is the non-personal Performance Work Statement (PWS) for Grounds keeping, Minor Maintenance & Facility Services for DLA Installation Support Europe and Africa:
2.1 Non-personal grounds keeping services shall include but not be limited to: grass cutting; fertilization; weed control; dethatching; aeration; over seeding; watering; edging; planting (and plant replacement); raking;
trimming and pruning plants; mulching; cleaning; sweeping; preservation and painting of outside benches, tables, and planters; and disposal of related items such as clippings, trimmings, brush, branches and leaves.
2.2 Non-personal minor maintenance and facilities services shall include but not be limited to: parking lot and building entrance sweeping/power washing; exterior stairwell and basement window well cleaning; indoor plant maintenance or replacement; indoor plant watering and fertilizing; recycling of printer cartridges, alkaline batteries and other bulk office items, not to include daily office debris; moving office/furniture items; re-rolling or straightening of hallway carpets as needed to maintain a professional appearance; maintaining (clean and straighten) doorway entry mats; unlock and lock doors within DLA buildings to facilitate repair personnel access when requested by COR; replace drinking water filters; changing light bulbs; replacing door locks on an as needed/infrequent basis; hanging office pictures, blinds, shades, and other display items on an infrequent as needed basis; repair and touch-up paint office/common areas; exchanging fire extinguishers with U.S. Army Garrison; install conduit and provide cable management on an infrequent as needed basis; and other infrequent minor maintenance tasks in response to DLA Installation Support Helpdesk Service Requests. It is the incumbent Contractor’s responsibility to become familiar with all grounds and facility areas covered within this scope of work.
2.3 Inspections shall include but not be limited to: the operational status of handicap lifts, emergency exit lights, light sensors, windows, doors, radiators, electronic locks, fire extinguishers, exterior lighting, general facility walk through, other facilities infrastructure items as directed by the Contracting Officer’s Representative (COR). Daily tasks include, but not limited to: visual inspections of grounds surrounding buildings 3222, 3224, 3227, 3228, 3229, and 3230 to identify and remove any debris, trash, snow/ice, and vegetation in order to maintain a welcoming and professional appearance. Other daily tasks include checking of basement humidity monitors; monitoring dehumidifiers and adjusting in accordance with readings noted as necessary to maintain basement areas between 40 and 55 percent relative humidity; replacing defective humidity monitors. Weekly tasks include, but not limited to: maintaining workplace environmental services such as venting basement areas three times per week (building 3227 and 3229), emptying dehumidifiers as required, weekly cleaning and disinfecting of dehumidifier containers. Quarterly visual inspections of grounds surrounding building 320 (ROB) shall identify any debris, trash, snow/ice, and vegetation in order to maintain a welcoming and professional appearance. Sembach building 214 will be visited as requested by COR. All inspection work does

not include weekends. Any deficiencies identified during inspections shall be reported to the COR within 24 hours for documentation and correction. Deficient items as listed in PWS shall be repaired by the Contractor.
2.4 Contractor shall use the most current recurring maintenance schedule, sample found in EXHIBIT 4, as maintained and provided by the COR. Additionally, the awarded Contractor must complete tasks in accordance with locally developed maintenance guides (EXHIBIT 5), manufacturer’s instructions, or U.S./host nation standards--whichever is most stringent.
2.5 The Contractor shall provide all necessary labor, supplies and materials, hoses, sprinklers, ladders, hand tools, vehicle(s), lawn mowers and other equipment to provide services, if and when required. In addition the Contractor shall provide transportation and supervision of Contractor employees involved in the execution of required services at the stated facilities.
3.0 SAFETY REQUIREMENTS: The Contractor shall be responsible for the safety of Contractor personnel as well as U.S. Government personnel impacted by the performance of services. The Contractor shall also be responsible for the safe operation and application of both Contractor-owned and government furnished equipment/property and material. Specific guidance is contained in the attached DLA Kaiserslautern Instruction SOP 6055.01 dated 2017.
4.0 HOURS OF OPERATIONS: Contractor personnel shall perform required work during normal duty hours, Monday-Friday 7:30 am - 4:30 pm CET, and shall be available during this time for all tasks outlined in this PWS. Unusual or emergency situation work may require performance outside normal duty hours (Monday – Friday) and/or on German or American Holidays. Notification for non-emergency work requirements necessitating weekend or other non-duty hour performance, such as VIP visits, maintenance of parking lot, and some landscaping shall be provided to COR/COTR no less than three working days prior to needed service delivery.
4.1 Holidays:

5.0 PERFORMANCE PERIOD: The contract term is for a one-year period with three (3) option years and shall commence in accordance with the terms of the contract. Invoices and payments throughout the contract term shall be submitted in WAWF.
6.0 PERSONNEL REQUIREMENTS: The Contractor shall work with, and report to the DLA Installation Support COR or Contract Officer Technical Representative (COTR). The DLA Installation Support COR or COTR shall in turn report to the DLA Troop Support Contracting Officer (KO) located at Kleber Kaserne, Kaiserslautern Germany, with whom contract authority resides. The Contractor assigned site supervisor, if required, shall conduct the overall management coordination on the contract for consigned work items as well

as management of onsite Contractor employees. The Contractor’s Representative and/or alternate(s) must be able to read, write, speak and understand English with moderate fluency; this person must be present when work is being accomplished as part of this PWS. The Contractor shall ensure that all employees present a professional appearance that is appropriate for their position. The KO, COR, or COTR reserves the right to determine the acceptability of any clothing worn. Employee identification shall not be substituted for station required passes or badges. Contractor employees shall conduct themselves in a proper, efficient, courteous, ethical and businesslike manner. All discrepancies shall be reported to the KO for appropriate action.
6.1 CONTACT INFORMATION: The Contractor shall provide to the COR, within seven (7) days of the contract award, the names, addresses and home and/or mobile phone numbers of the Contractor Representative and the alternates with authority to act for the awarded vendor to the full extent of the PWS. Contact information shall be forwarded to the Contracting Officer by the COR/COTR for retention.
6.1.1 COR/COTR/CO: The designated DLA Installation Support COR/COTR shall be the Primary Point of Contact for the Contractor for day to day actions and shall coordinate all government actions as directed by the Contracting Officer for this contract. The COR/COTR has no authority to make any commitments or changes that affect price, quality, quantity, delivery, or other terms and conditions of the contract nor in any way direct the contractor or its subcontractors to operate in conflict with the contract terms and conditions. The Contracting Officer is the sole authority for the contract and is the only person with the authority to authorize work performed in relation to the awarded contract.
6.2 SECURITY REQUIREMENTS: Government badges are required to access US Army Garrison Rheinland-Pfalz (USAG-RP) Army Bases. The Contractor shall complete all applicable paperwork to obtain access passes to USAG-RP Army Bases where DLA facilities are located and work is performed as well as required paperwork for building access badges. These documents include form AE604-1B JUN 05 for Contractor entry into the Local National Screening Program (LNSP), Local Police checks and copies of employee identification cards or passports as appropriate to verify work permit requirements. The government shall provide a Base Pass using form AE190-16A and identification security badges for building access using a locally generated DLA ACS BADGE Request form. A signed non-disclosure agreement or escort may be required to access buildings 3209, 3222, 3224, 3227, 3228, 3229, 3230, 320 (ROB), and 214 (Sembach). All badges shall be distributed to and be retained by the Contractor employees until surrendered upon contract completion or termination of employment. DLA ACS Badges shall be signed for by the Contractor. The government shall provide and the Contractor must sign for electronic door keys as needed for the performance of duties.

7.0 REQUESTING SERVICES: Normal service delivery, during regular hours shall comply with tenants of the PWS. Service that requires delivery outside of normal hours shall be requested electronically via email no less than three working days in advance. The Contractor shall confirm the outside of normal hours request within 24 hours of receiving the request either via E-mail or via telephone. Emergency service where a three-day lead time for electronic notification is not feasible shall be done by telephone using a number provided by the Contractor or in-person. The Contractor shall provide a communication medium (for example, answering service or machine) to receive 24-hour work request notification calls or emergency calls when available telephone lines are occupied or out of the service area. The Contractor shall send an acknowledgement email or telephone call to the COR within 24 hours of receipt.
8.0 MEETINGS: The Contractor shall, at a minimum, be required to attend monthly meetings and provide weekly proposed schedules. The following details the minimum required expectations:
8.1 The COR/COTR and the Contractor shall attend, at a minimum, monthly progress meetings the last Thursday of every month unless previously coordinated between COR/COTR and the Contractor due to holiday or vacation. During regular monthly meetings the Contractor shall present clear information, on a typed document, in English, on the task requirements completed (Para 10.0), work to be performed and any pending future work expected within the monthly performance period.
8.2 The KO, COR/COTR and Contractor shall meet quarterly, and monthly, if warranted, to clarify a contract or service delivery point. A request to meet with the KO, COR/COTR and Contractor to resolve an outstanding service deficiency is not restricted to a monthly basis. All contract issues requiring KO resolution shall be promptly brought to the Contracting Officer’s attention for a meeting to expeditiously address.
8.3 The Contractor will provide the COR a written weekly proposed schedule stating the contractors objectives for the week; this shall be submitted via email or in person before 11:00 the first work day of the week. The Contractor and the COR/COTR shall discuss work priority and assess service delivery quality. The Contractor shall make necessary adjustments to work priority as directed by the KO/COR/COTR.
9.0 MATERIALS AND SUPPLIES: The Contractor shall provide all materials, tools, vehicles and supplies required in the performance of this contract except as otherwise noted. All materials, supplies and repair/replacement parts shall be new, clean, non-counterfeit, free of defects, corrosion, damage, and fitted for size/design. The characteristics of materials, supplies and repair/replacement parts shall be for the use intended and of an equal or better quality, capacity and efficiency as the originals, as well as be produced by manufacturers regularly engaged in the manufacture of these products. Materials acquired shall become U.S.
Government Property when repaired, installed and completed. Plants, seeds, grasses, fertilizers, topsoil and groundcover bedding shall comply with Host Nation environmental and agricultural regulatory guidelines.
Paints, cleaners and other grounds keeping type solvents or salts shall similarly comply with Host Nation regulatory mandates for application, storage, use and disposal.
10.0 TASK REQUIREMENTS: The Contractor shall maintain and repair all items (task requirements) specified in this instrument and annotated in a report that shall be presented to the COR at the monthly meeting as outlined in Para 8.1. Any items found by the Contractor requiring repair, and not listed in the PWS, shall be brought to the attention of the COR for a determination as to the applicability of this, or other contract specification sections. The Contractor shall instruct his personnel to report all potential hazards, unsafe conditions, and/or structural and grounds deficiencies found during performance of their work immediately to the COR/COTR.

10.1 SPECIALIZED CONTRACTOR PERSONNEL, MATERIAL AND INVOICING: The need for specialized contractor personnel and/or work required to be subcontracted or specially ordered shall be discussed in advance with the COR. Such specialty work might include landscape design, repair/replacement of flower containers, bedding borders, outdoor fountains, fountain pumps, associated solar panels used for pump/lighting energy, picture frames and special wall mounting hardware and fixtures. The Contractor shall notify the COR when said items are required, provide the COR a list of materials required, and provide material sourcing in conjunction with the COR. Above listed specialty work shall be paid for by the COR using a Government Purchasing Card, and the contractor shall be responsible for pickup within the local area and placement on site, unless delivery is part of the procurement cost, after the COR purchases the necessary material. The Contractor is solely responsible for procurement and payment of material and specialty service normally required for maintenance of lawns, grounds, and building interiors as described within the specification. Material shall include but not be limited to minor amounts of topsoil, mulch, leaf bags, fertilizers, seeds and seeding material, interior and exterior plants, interior pots, shrubs, hoses, watering cans and nozzles, bedding plants, weed controllers, oils, gas, belts, sharpening and tune up service, spark plugs, water filters, humidifier filters, sanitizing tablets, picture hooks and hardware, salt, miscellaneous paints, sealants and hardware for outside furniture, material for constructing or repairing hallway display boards, and any materials needed in the normal performance service outlined in this contract. Contractor should pay specific attention to site conditions at the time of site walk and shall be specific in assumptions within proposals.
11.0 GROUNDS AND BUILDING MAINTENANCE: The Contractor shall be required to perform the following grounds keeping, minor maintenance, repair work and facilities inspection work necessary to maintain the buildings specified herein. Refer to Para 2.4 for required parameters.
11.1 LANDSCAPING/MOWING: The Contractor must ensure the lawn and grassed areas are mowed when the grass has reached an average height of nine (9) cm. Before mowing the lawns, remove all stones, debris, trash and branches. After each mowing session remove any resultant debris within and outside the area worked, including grass cuttings visible on the lawn surface, walks and parking areas. Trim grass around trees and bushes, along sidewalks, hedges, fences, posts, picnic tables, sports fields and playground equipment, buildings and other bordering structures after each mowing to the same height as the surrounding grass. All materials and debris removed shall become the property of the Contractor and be disposed of immediately. The Contractor may dispose of material at the recycling facility at Kleber Kaserne, Kaiserslautern if the material is of a type that can be accepted by the recycling facility. Acceptable material includes paper, plastics, toner cartridges, mixed trash, biological items and bulk items such as office chairs. Material is accepted at the recycling facility at no cost. Materials not able to be accepted at on base recycling point shall be disposed of at German public facilities as part of grounds service and shall include normal alkaline batteries. Hazardous items such as paints and solvents may be turned in free of cost to USAG-RP HAZMAT collection points.
11.2 FERTILIZING: The Contractor shall fertilize the lawns/flower beds twice a year during the spring months (March to April) and again in fall months (October to November). Fertilize all lawns with slow-release (Langzeitduenger) fertilizer; minimally 50g/sqm (or better depending on conditions).
11.3 WEEDING: Flower beds, borders, plant containers and sidewalks shall be maintained weed free.
Weeding shall be completed once a week during the peak summer months of June, July and August and other months at a reduced frequency but not less than monthly as necessary to maintain a neat grounds appearance.
11.4 FOLIAGE AND LANSCAPING DEBRIS: The Contractor, during the months of November through March, shall clear and ensure the entire work site, excluding the planted bed areas, are clear of foliage and plant

debris to prevent safety hazards caused by layers of wet leaves or other plant debris. Each cleaning shall be done in a way that ensures walkways, paved parking areas, steps, entrances and basement window wells are free of leaves and debris. Foliage, pine cones or sticks raked from grassy areas can be retained on site in the wooded area between B3222 and B3228 and/or in the area adjacent to the DLA mower storage garage. Foliage and debris shall remain on site for no more than 14 days unless authorized by the COR to allow for decomposition and more efficient loading in Contractor provided disposal bins or vehicles. All materials and debris removed shall become the property of the Contractor and shall be disposed of immediately. Foliage and debris collected at building 320 (ROB) must be removed within seven (7) days unless authorized by the COR. Bulk removal of autumn leaves shall be accomplished during the months of October and November, contingent upon leaf shedding, using Contractor procured bins to maximize leaf removal efficiencies. The Contractor must remove debris from fire escapes at building 3227 and 3230, and ensure all storm drains surrounding DLA Kleber Kaserne facilities are free/clear of debris on a weekly basis.
11.5 INDOOR PLANTS: The Contractor shall water, fertilize and if necessary replace indoor plants in the public hallways, as required upon authorization by COR.
11.6 OUTDOOR FLOWER BEDS AND CONTAINERS: During the months of April thru October, the Contractor shall keep the flower beds clean and free of weeds. The Contractor shall water, fertilize, clip, trim and shape bushes and hedges as required, in order to keep them in a healthy and attractive state. Plant containers requiring replacement shall be identified by the Contractor and referred to the COR. Planter replacements shall be approved and paid for by the COR using a Government Purchase Card. Plant replacement in bedding areas and areas of low ground cover shall be reviewed and approved by the COR prior to procurement and payment by the Contractor as part of regularly expended material and supplies.
11.7 SWEEPING: The Contractor shall sweep the entrance steps and the surrounding area to all buildings, not to exceed three meters distance, three times per week; police the grounds for trash and litter three times per week; sweep common basement areas once per month; inspect and remove insect webs and alike from the corners and wall crevices of basement storage areas in B3227 and B3229 on a quarterly basis; and clean two Contractor occupied garages, and the Installation Support storage area located in B3227 on a quarterly basis.
11.8 WINTER MAINTENANCE: During inclement winter weather the Contractor shall shovel and salt the walkways and building entryways as well as the staircases of DLA occupied buildings on Kleber Kaserne.
Sidewalk areas measure 136 meters in length and vary in width from 1.2 to 1.5 meters; sidewalks include areas between B3222 and B3227, B3222 and B3228 and B3222 and B3229. Entrance areas include the handicap ramps on B3230 (front and side), B3227 (north) and B3228 (north); steps on B3227 (north and south), B3228 (east, north and south), B3229 (east and west) and B3230 (west); entries include B3209 (north and west); and stairwells B3227 (east and west) and B3229 (south). Sidewalks and steps must be completely shoveled and salted no later than 0730 CET Monday through Friday if snow or ice has accumulated overnight. Areas shall also be serviced once every 2 hours during the day (0800-1600 CET) during periods of persistent snow accumulation. The Contractor shall also place government provided salt containers at DLA building entries at the beginning of winter, refilling with salt as required. Salt containers shall be returned to on-site storage in the third week of April.
11.9 INSPECTIONS: The Contractor shall perform weekly inspections of two exterior handicap lifts to verify operational status; bimonthly inspection of 45 emergency exit lights; monthly inspections of building common areas and hallways to check for infrastructure deficiencies including windows, window handles, door operation, door thresholds, door closure fixtures, flooring, baseboards and radiator knobs; quarterly inspection

of 85 common area and storage room light sensors; semi-annual inspection of all common area fire extinguishers; yearly inspection of 10 Intrusion Detection Alarm boxes. Inspections are limited to checking date indicators, checking asset operational lights, pushing a button, turning a key, flushing and like minor tasks to check for operational function and/or a visual inspection of overall conditions. Result of inspections shall be provided in writing using COR provided inspection sheets and copies given to COR at scheduled monthly meeting.
11.10 BASEMENT VENTING AND DEHUMIDIFIERS (B3227 & B3229): This work shall include but not be limited to opening and closing five windows to vent basement as required to achieve optimal lower humidity readings; daily (Monday through Friday) checking thirty-two basement relative humidity meter readings;
turning thirty-two dehumidifiers on or off as required; emptying dehumidifier water containers as required; and weekly placing of disinfectant tablet in portable dehumidifier tanks.
11.11 MINOR MAINTENANCE SUPPORT: Minor maintenance shall include but not be limited to straightening hallway carpet runners; hanging pictures; hanging shelves; replacing Government provided entry mats; fixing mats in place with double backed tape to prevent slippage; transporting Fire Extinguishers to USAG-RP DPW for replacement; replacement of defective temperature and dehumidifier meters; replacement of defective portable room dehumidifiers (Government provided); clean/replace dehumidifier filter and tank cleaning/disinfecting; quarterly changing of water cooler filters (material reimbursement); cleaning and painting park benches located on the grounds; pickup and transport of office print cartridges and other bulk office recyclables to appropriate government recycling or off base locations. Recycling of office waste paper and plastics is the responsibility of the custodial Contractor and not part of this PWS. Unless otherwise stated, minor maintenance is on an as needed basis as requested by the COR.
11.12 MISCELLANEOUS: Work tasks not specifically stated in sections 9.0 through 10.12 and EXIBIT 1, 4 and 5 but implied as related to work described in sections 1.0 through 2.5 are addressed in this section.
Miscellaneous work shall include but not be limited to set up and tear down of DLA’s portable public address (PA) system for courtyard functions, set up and tear down of agency lawn tents and chairs, opening and closing storage areas and building doors for other contracted workmen and as approved by the COR, and other minor maintenance and beatification work on an as needed basis as requested by the COR. Miscellaneous work is on an infrequent, as needed or seasonal basis. Additionally, the Contractor is responsible for all repairs of damaged grounds or facilities due to Contractor personnel fault such as neglect, improper maintenance, or failure to adhere to checklists/manufacturer’s instructions/or other codes and guidance.
12.0 RECORDS: The Contractor shall maintain all inspections records, repair records, invoices and material receipts in a format that the information can be readily available to the Facility Manager/COR, Contracting Officer and DLA staff as needed. The maintenance of the stated records shall be developed jointly between the Contractor and the COR prior to the start of on-site repairs or maintenance services.

13.0 WARRANTIES: The Contractor shall make maximum use of the provisions of manufacturer’s warranties for Government property as applicable. The Contractor shall maintain a file(s) of warranties on grounds equipment that are under warranty and under the management of this contract. These files shall identify the item, part and model numbers, the nature and expiration date of the warranty, and the name and address of the firm to contact concerning the Government’s entitlement under the warranty. Further the Contractor shall maintain maintenance records and files on all grounds keeping GE.
13.1 WARRANTY ENFORCEMENT: In the event of an equipment failure or other problem covered by the warranty, the Contractor shall verify expiration date and immediately inform the COR of the status.
14.0 PERFORMANCE STANDARDS: The following standards are the performance objectives under this contract.
14.1 QUALITY ASSURANCE: The Government shall evaluate the Contractor’s performance under this contract using the method of surveillance as specified in a Quality Assurance Surveillance Plan (QASP) which shall be provided to the vendor at the time of award. The COR shall provide a monthly QASP report to the Contractor to document performance. All surveillance observations recorded as a defective performance shall be reviewed by the COR and Contractor at the monthly meeting as stipulated in section 7.1 for written corrective action development. Uncorrected deficiencies shall be forwarded to the Contracting Officer for follow-on action.
14.2 QUALITY CONTROL: The Contractor shall establish and maintain an inspection and surveillance system to ensure that the work performed conforms to the contract requirements. The Contractor shall document and maintain a file of all scheduled and performed inspections and surveillances, inspection and surveillance results, and dates and details of corrective and preventive actions. The quality inspection and surveillance file shall be the property of the Government and maintained in a centralized location within DLA Kleber. The file shall be turned over to the KO within five calendar days of termination of the contract.
15.0 PERFORMANCE OBJECTIVES/RESPONSE TIMES: The Contractor shall respond and commence performance within 48 hours after receipt of the service request. Exhibits below provide general information for proposals; a site visit to review existing site conditions prior to providing a proposal is required.
