As per FAR 15.206(c), this amendment is issued to all offerors that have not been eliminated from the competition. This solicitation is not being re-opened for proposal submission.

The following revisions to the solicitation are as follows:

1. Page 92-93 of the Solicitation is changed from:

Tab 1(C): Offeror's Subcontracting Plan for Small, Small Disadvantaged, Women-Owned, Veteran Owned Small Business, Service-Disabled Veterans Owned Business and Small HUBZone Business Concerns (if required). A Subcontracting Plan shall be submitted only by offerors who are large business concerns. Offerors must comply with FAR 52.219-9 (JUN 2020). Plan must meet a minimum of 2.5% for overall small business concern utilization for total projected sales (Refer to sections 1(C) and 8(B) of the Statement of Work).

Tab 1(I): The past performance evaluation result is an assessment of the offeror’s probability of meeting the minimum solicitation requirements. This assessment is based on the offeror’s record of relevant and recent past performance relating to pricing accuracy, contractual fill rate, and data submission accuracy. Contractor’s performance is validated through monthly, quarterly, and semi-annual post award audits. Additionally, The Federal Awardee Performance and Integrity Information System (FAPIIS), Past Performance Information Retrieval System (PPIRS), and Contractor Performance Assessment Reporting System (CPARS) data as well as reports generated by Corporate Data for Medical Intelligence Applications (CDMIA) will be reviewed as part of the criteria to evaluate the contractor’s past performance over each of the past 12 months. To be considered relevant, vendors must be commercial wholesalers that have past performance in similar size and scope to this acquisition. Offeror’s may provide any pertinent past performance data in Tab 1(I) of their proposal. (see Addendum #4).

Past performance information may be obtained from any other sources available to the Government, to include, but not limited to, Supplier Performance Risk System (SPRS), Federal Awardee Performance and Integrity Information System (FAPIIS), Electronic Subcontract Reporting System (eSRS), Experian (formally the Dunn & Bradstreet (D&B)), Contractor Performance Assessment Report System (CPARS), or other databases. These systems will be reviewed as part of the criteria to evaluate the contractor’s past performance over the past twenty-four months. To be considered relevant, vendors must be commercial wholesalers that have past performance in similar size and scope to this acquisition. Relevancy includes similarity, recency, types of items supplied, contract length, and dollar value.

The offeror may provide past performance information, as follows, for consideration in evaluating past performance: A list of contracts, commercial or government, performed within the past twenty-four (24) months preceding the closing date of the solicitation, limited to three (3) government and three (3) commercial contracts for the same or similar items. The offeror shall provide or include a point of contact (POC), address, telephone number, an estimated contract dollar value, the total contract length, and a sample listing of the items provided for under the contracts. For any government contracts, provide the government agency, a (POC), telephone number, appropriate contract number, estimated contract dollar value, the total contract length, and a sample listing of the items provided, if applicable. The Government reserves the right to limit the number of references it decides to contact. Also provide a listing of any problems or discrepancies (includes shortages, overages, damages, defects, or mis-shipments, etc.) experienced within the past twenty-four months (twenty-four months preceding the closing date of the solicitation) for the contracts or customers reported in the paragraph above. Include a brief description of how these problems were addressed and remedied. Offeror’s may provide this pertinent past performance data in Tab 1(I) of their proposal. (see Addendum #4).

PAST PERFORMANCE

There are three aspects to the past performance evaluation. The first is to evaluate whether the offeror’s present/past performance is relevant or not relevant to the effort to be acquired.

The second is to evaluate whether the offeror’s past performance is recent or not recent to the effort to be acquired. The third aspect of Past Performance is to determine how well the contractor performed on the contract criteria as indicated in the remainder of this addendum.

Past performance will be evaluated in accordance with FAR 15.101-2(b). Since this is an LPTA acquisition, all offers will be given an “acceptable” or “unacceptable” rating. Offers with no pertinent past performance shall be determined to have “unknown” past performance. In the context of acceptability/unacceptability, “unknown” shall be considered “acceptable.”

The past performance evaluation results are an assessment of the offeror’s probability of meeting the minimum past performance solicitation requirements for each of the past 12 months. This assessment is based on the offeror’s record of relevant and recent (for each of the past 12 months) past performance information relating to the following:

1. Pricing Accuracy – historical data will be collected to measure if the offeror has provided acceptable pricing accuracy for support similar to that required by this solicitation.

2. Contractual Fill Rate (delivery/quality) – historical data will be collected to measure if the offeror has met an acceptable fill rate percentage for similar requirements.

3. Data Submission Accuracy – historical data will be collected to measure if the offeror has provided an acceptable level of data submission relating to the NPV or similar program for the data submitted as a contractor via the ACCEPTANCE STAGE report and the VALIDATION STAGE report. Accuracy is based on empty fields as well as fields that include bad data. These reports are generated by Corporate Data for Medical Intelligence Applications (CDMIA).

To be considered relevant, vendors must be commercial wholesalers that have past performance in similar size and scope to this acquisition.

Note: Contractor’s performance is validated through monthly, quarterly, and semi-annual post award audits. Additionally, The Federal Awardee Performance and Integrity Information System (FAPIIS), Past Performance Information Retrieval System (PPIRS), and Contractor Performance Assessment Reporting System (CPARS) data, Historical Fill-Rate Data, as well as reports generated by Corporate Data for Medical Intelligence Applications (CDMIA) will be reviewed as part of the criteria to evaluate the contractor’s past performance over each of the past 12 months.

All data may be validated by the government.

Offeror’s may provide any pertinent past performance data in Tab 1(I). If nothing is provided, performance will be validated by reports generated from FAPIIS, PPIRS, CPARS, and CDMIA.

Proposals will be evaluated in accordance with FAR 15.101-2, FAR 15.305, and DFARS

215.305. This assessment is based on the offeror’s record of relevant and recent past performance information that pertain to the products and/or services outlined in the solicitation requirements. However, the comparative assessment in FAR 15.305(a)(2)(i) does not apply.

In order to be considered for award an offeror must meet or exceed the acceptability standards for the following non-cost factors: Past Performance and Compliance with the terms and conditions in the solicitation.

Sources of Past Performance Information for Evaluation are as follows:

-Past performance information may be provided by the offeror, as solicited.

-Past performance information may be obtained from questionnaires tailored to the circumstances of the acquisition

-Past performance information may be obtained from any other sources available to the Government, to include, but not limited to, Supplier Performance Risk System (SPRS), Federal Awardee Performance and Integrity Information System (FAPIIS), Electronic Subcontract Reporting System (eSRS), Experian (formally the Dunn & Bradstreet (D&B)), Contractor Performance Assessment Report System (CPARS), or other databases, if applicable.

An offeror with SPRS scores that are excessively low will not meet acceptability standards.

The offeror may provide past performance information, as follows, for consideration in evaluating past performance: A list of contracts, commercial or government, performed within the past twenty-four (24) months preceding the closing date of the solicitation, limited to three (3) government and three (3) commercial contracts for the same or similar items. The offeror shall provide or include a point of contact (POC), address, telephone number, an estimated contract dollar value, the total contract length, and a sample listing of the items provided for under the contracts. For any government contracts, provide the government agency, a (POC), telephone number, appropriate contract number, estimated contract dollar value, the total contract length, and a sample listing of the items provided. If the contracts/accounts submitted were performed by a predecessor company, subsidiary, subcontractor, or other affiliate entity (see FAR 2.101), the offeror must clearly demonstrate that the predecessor, subsidiary, subcontractor, or affiliate will perform major or critical aspects of the resultant contract in order for the submitted contract/account to be considered.

Past Performance will be evaluated in accordance with FAR 15.305 and DFARS 215.305.

This assessment is based on the offeror’s record of relevant and recent past performance information that pertain to the products and/or services outlined in the solicitation requirements. However, the comparative assessment in FAR 15.305(a)(2)(i) does not apply.

Past Performance will first be evaluated to determine if the offeror has relevant or non-relevant and recent or not-recent past performance for each of the areas of Pricing Accuracy, Contractual Fill-Rate, and Data Submission Accuracy.

The NPV (GEN-IV) Past Performance Evaluation Criteria are as follows: an offeror must receive an acceptable for each criterion for which it has relevant and recent past performance in order to receive an overall acceptable past performance rating:

1. Pricing Accuracy- at a minimum offeror has provided accurate prices 97.9% of the time for each of the past 12 months for support similar to that required by this solicitation.

2. Contractual Fill-Rate (including but not limited to delivery/quality): at a minimum offeror has met fill rate requirements 90% of the time for each of the past 12 months for similar requirements.

3. Data Submission Accuracy: at a minimum, offeror submitted accurate data 90% of the time for each of the past 12 months relating to the NPV or a similar program for data submitted as a contractor via the ACCEPTANCE STAGE report and the VALIDATION STAGE report. Accuracy is based on empty fields as well as fields that include bad data.

These reports are generated by Corporate Data for Medical Intelligence Applications

(CDMIA).

To be considered relevant, vendors must be commercial wholesalers that have past performance in similar size and scope to this acquisition.

PAST PERFORMANCE

Past Performance will be evaluated in accordance with FAR 15.101-2 Lowest Price Technically Acceptable Source Selection Process, FAR 15.305, and DFARS 215.305. This assessment is based on the offeror’s record of relevant and recent past performance information that pertain to the products and/or services outlined in the solicitation requirements. However, the comparative assessment in FAR 15.305(a)(2)(i) does not apply.

The past performance evaluation results are an assessment of the offeror’s probability of meeting the solicitation requirements. This assessment is based on the offeror’s record of relevant and recent past performance information that pertain to the products outlined in the solicitation requirements.

There are two aspects to the past performance evaluation. The first is to evaluate whether the offeror’s present/past performance is relevant or not relevant to the effort to be acquired.

Relevancy includes similarity, recency, types of items supplied and contract length in comparable size and scope to this acquisition. An effort will be considered recent if performance occurred within the twenty-four months period preceding the closing date of the solicitation.

Past Performance on the relevancy of the contractor’s experience will be evaluated and rated on a relevant/recent or not-relevant/not-recent basis.

Acceptable: The offeror’s past performance effort involved recent (each of the past 24-months) and similar size and scope of effort based on the past performance criteria above.

Unacceptable: The offeror’s past performance record involved not-recent (each of the past 24-months) and little or none of the size and scope of effort based on the past performance evaluation criteria above.

The second aspect of the past performance evaluation is to determine how well the contractor performed on the relevant contracts. The evaluation will include whether the offeror timely delivered requirements and met quality standards, as well as problems encountered, and the effectiveness of any corrective action taken in addressing those problems. For purposes of this aspect of the evaluation, performance that occurred within the 24-month period preceding the closing date of the solicitation will be considered. Performance data outside of that 24-month period will not be considered.

Past Performance on how well the contractor performed on relevant contracts will be evaluated and rated on an acceptable/unacceptable basis.

Acceptable: Based on the offeror’s performance record, the Government has a reasonable expectation that the offeror will successfully perform the required effort, or the offeror’s performance record is unknown.

Unacceptable: Based on the offeror’s performance record, the Government does not have a reasonable expectation that the offeror will be able to successfully perform the required effort.

Note: In the case of an offeror without a record of relevant past performance or for whom information on past performance is not available or so sparse that no meaningful past performance rating can be reasonably assigned, the offeror may not be evaluated favorably or unfavorably on past performance (See FAR 15.305 (a)(2)(iv). Therefore, the offeror shall be determined to have unknown past performance. In the context of acceptability/unacceptability, “unknown” shall be considered “acceptable”.

All other terms and conditions remain unchanged.
