About this file

This document contains a limited source justification for the acquisition of 13 CAE Healthcare ISTAN 200R patient simulators from the CAE Healthcare Electronic Catalog contract. The United States Air Force School of Aerospace Medicine requires the simulators for flight nurse and aeromedical technician training courses. The ISTAN 200R is the only simulator that meets all technical requirements, including being approved for use on aeromedical evacuation aircraft like the C-130, KC-135, and C-17. It supports functions such as blood pressure measurement, articulated limb movement, intravenous access, and mechanical ventilation. Market research identified it as the only option that fulfilled requirements for integrated monitoring on the Zoll defibrillator, full limb articulation, tetherless use, and flight approval. The total order value is $915,171.01. As the sole product meeting needs, this justification waives the competition requirement according to FAR 8.405-6(b)(1).

This document also contains information on federal contract opportunity SPE2DH16R0002 for medical equipment through the Defense Logistics Agency Troop Support Electronic Catalog program. The solicitation seeks proposals for equipment, maintenance, and spare parts. Included are items like hospital beds, surgical tables, laboratory analyzers, dental equipment, and medical simulation systems. Excluded are radiology, radiation oncology, furniture, and certain patient monitoring devices. The standing solicitation will remain open for five years and result in multiple long-term contracts under the medical equipment catalog.

LIMITED SOURCE JUSTIFICATION (LSJ)

1. Defense Logistics Agency (DLA) Troop Support, Medical Supply Chain, Hospital Supply Division, Electronic Catalog (ECAT), is the contracting activity.

2. This justification is for the acquisition of the CAE Healthcare ISTAN 200R patient simulator. The order will be placed against CAE Healthcare Electronic Catalog (ECAT} contract number SPE2DH14D0010 that expires on Sept. 11, 2019.

3. The United States Air Force, School of Aerospace Medicine (USAFSAM), Wright- Patterson Air Force Base, Ohio has the following requirement:

Item Description:

Model Number:

Quantity:

Unit Price:

Total Dollar Value:

Patient Simulator

CAE !STAN 200R

13 each $70,397.77 $915,171.01

4. This action is being conducted under the Multiple Award Schedule Program. The authority waiving the competition requirements is FAR 8.405-6(b)(1): only one awardee is capable of providing the supplies or services required at the level of quality required because the supplies or services ordered are unique or highly specialized. The CAE Healthcare !STAN 200R patient simulator is the only item on ECAT that can meet the clinical and technical requirements of United States Air Force, School of Aerospace Medicine (USAFSAM) needs.

Patient simulators are utilized in the en-route training for Flight Nurses and Aeromedical Technician courses, at the School of Aerospace Medicine located at Wright-Patterson Air Force Base Ohio. The minimum technical requirements are as follows:

A. Shall perform mechanical/automatic/manual blood pressures B. Shall have fully articulated movement in the wrists, elbows, knees, and ankles C. Shall have bilateral Intravenous insertion areas with permanent access options D. The unit shall be tetherless with an internal waste reservoir for intravenous fluids greater than or equal to 500ml E. Shall have a wide range of amputation emulation options for each limb F. Shall provide a compatible interface with the Zoll Propaq Monitor/Defibrillator to include temperature, invasive lines, and Defibrillation capability G. Shall monitor and record depth and rate of chest compressions in accordance with the American Health Association (AHA) guidelines.

H. Shall be approved for use on Aeromedical Evacuation aircraft (C-130, KC-135, C-

17) I. Shall support mechanical ventilation (Assist control/pressure control/(bag,valve mask (BVM) w/ Positive end-expiratory pressure (PEEP) of 12cmH20.

The GAE ISTAN-200R is the only human patient simulator to be approved for flight use in Aerovac aircraft which includes the C-130, KC-135 and the C-17 that meets all of the minimum technical requirements. This is critical to the USAF School of aerospace medicine's Expeditionary Education and Training Department, which is responsible for educating medical professionals in trauma, aeromedical evacuation and internal medicine.

The department runs educational programs for International Health Specialists(HIS), centers for Sustainment of Trauma and Readiness (CSTARS), Expeditionary Medical Support (EMEDS), Critical Care Air Transport Team (CCATT) programs and courses for flight nurses and Aerovac technicians.

5. The Contracting officer has determined the price of the GAE ISTAN 200R to be fair and reasonable based upon a favorable comparison to commercial and Federal Supply Schedule pricing at the time of CAE's initial ECAT submission. This order represents the best value to the Government and results in the lowest overall cost alternative (considering price, special features, and administrative cost). GAE will be issued a Request for Quote (RFQ) prior to the placement of the delivery order in an effort to obtain better pricing than was previously negotiated.

6. The USAF School of Aeromedical Medicine at Wright-Patterson AFB conducted market research between May 2017 and July 2017. The research consisted of attending a simulation market symposium as well as on-site visits with various industry leaders.

Three products were evaluated and they included the GAE !STAN 200R, the Gaumard HAL S3201 and the Laerdal SimMan 3G. The subsequent evaluation found the GAE Healthcare ISTAN 200R met all the essential characteristics. The Gaurmard HAL S3201 did not meet the minimum requirements (A, F, H & I) which included: The unit could not obtain mechanical automated monitoring of non-invasive blood pressure, the unit was unable to display temperature and invasive lines on the Zoll, Propaq, the unit was not flight approved for fixed wing aircraft and the unit could not be used on a ventilator. The Laerdal SimMan 3G did not meet the minimum requirements of (B & D) which included: The unit did not include a fully articulated wrist or ankle and the unit was not tetherless. (In order to meet the technical requirements of item I (shall support mechanical ventilation), Laerdal proposed adding model ASL 500. However, it was stipulated that the ASL 5000 must be hard-wired to the network with an Ethernet cable. By doing this, the unit would not be tetherless. DLA Troop Support completed market research in the ECAT in January 2018 and found a similar product manufactured by KForce Government Solutions (Trauma FX);

however, the trauma FX is not approved for use on Areovac aircraft to include the C-130, KC-135 and C-17. It is also geared to trauma situations and gunshot wounds.

7. ECAT vendors have the opportunity to add products that fall within the scope of the ECAT contract at any time; however, currently there are no other known ECAT vendors that would be able to offer a similar or equivalent product.

8. · In addition to the Services, DLA Troop Support will attempt to locate other acceptable sources inside and outside the ECAT program for future requirements as they arise.

As the Contracting Officer, I hereby certify that the information contained in this justification is accurate and complete to the best of my knowledge and belief.
