About this file

This is a Justification for an Exception to Fair Opportunity (JEFO) document for a single-source acquisition through the Defense Logistics Agency (DLA) Troop Support, Medical Supply Chain, Capital Equipment Division.

The acquisition is for Karl Storz Endoscopic-America Video Tower Systems to be supplied to the Ear, Nose, and Throat (ENT) Clinic at Madigan Army Medical Center (MAMC), Joint Base Lewis-McChord, Washington. The order will be placed against existing Patient Monitoring and Capital Equipment (PMCE) Contract SPE2D1-20-D-0015. The required equipment includes Image1 S camera system modules, 4K camera heads, Advanced Image and Data Archiving (AIDA) Bella Image Capture Devices with Smart Screen, surgical carts, LED light sources, 27-inch 4K monitors, video connection equipment, accessories, software licenses, installation services, and project management services. The waiver authority cited is FAR 16.505(b)(2)(i)(B), justifying single-source procurement based on the supplies being unique or highly specialized.

The justification centers on system compatibility and operational necessity. The ENT Clinic requires equipment fully compatible with existing Karl Storz endoscopic systems and the proprietary StreamConnect platform already installed at MAMC, which serves as one of four Army hospitals training otolaryngology surgeons. The clinic performs comprehensive head and neck endoscopic examinations and must capture images and video for medical documentation integrated directly into the Electronic Medical Record (EMR). The document emphasizes that multiple vendors would create operational inefficiencies, staff training complexity, maintenance burdens, and compatibility issues that would negatively impact patient care. Market research conducted over two years identified Karl Storz Endoscopy-America as the only vendor capable of meeting all requirements and ensuring equipment interchangeability with existing infrastructure. The Contracting Officer, Anthony Giunta (FSD), and DLA Troop Support Competition Advocate, Maciej Okulicz-Kozaryn, have approved the justification.
