About this file
This is a tax compliance form issued by the Georgia Institute of Technology Office of Planning, Design & Construction for suppliers responding to Solicitation/Contract No. 1068-27, an Indefinite Delivery Indefinite Quantity (IDIQ) contract for Construction Manager Cost Management Services. The solicitation seeks qualifications from construction management firms specializing in CM at Risk projects and cost management services to be performed in Atlanta, Georgia. The scope of work includes developing planning estimates, cost models, program estimates, design estimates, detailed itemized estimates, estimate reviews, scope and budget reviews, value engineering studies, CM/Contractor cost reconciliation, and change order cost evaluations across multiple project categories. Qualified respondents may be shortlisted for interviews, and attendance at the mandatory pre-qualification meeting is required. Bid documents are available for download in the Supporting Documents tab.
The tax compliance form must be completed by all suppliers and submitted to the Georgia Department of Revenue at compliance-state-con@dor.ga.gov for processing in accordance with the Georgia Procurement Manual. Required information includes the supplier's name, physical location, Federal Identification Number, state tax registration details, affiliate information, and the contact person responsible for tax issues. The Georgia Department of Revenue will verify whether the supplier qualifies as a "prohibited source" under O.C.G.A. 50-5-82 or has outstanding tax issues. Missing, incomplete, or erroneous data may delay or prohibit contract award verification, and no prohibited source may receive a contract award. Suppliers are strongly encouraged to verify their tax status and resolve any outstanding tax liabilities or missing tax returns prior to submission.
