Amendment issued to answer received questions, revise the solicitation, and extend the solicitation closing date.

A. Questions received:

Q1. Is it possible to inspect one site so that we can offer our best price?

A1. Yes, contact Roosevelt Terrell at Roosevelt.terrell@dla.mil . Please review the PWS for installation access. It will take at least five (5) days to process paperwork for access.

Q2. Can a site visit/survey to waste storage sites be arranged prior to submission of proposal?

A2. Please refer to question #1.

Q3. If no site survey, can pictures of waste and current waste storage sites be provided for assessment of current storage and packing conditions?

A3. Please refer to question #1.

Q4. If pictures cannot be provided, kindly describe the storage and packing condition for different waste items? This is to asset in identification of required scope for proper containment, packing, palletizing where required, labelling, segregation if required etc.. prior to waste loading on transporting vehicles.

A4. Please refer to question #1.

Q5. Can you provide the available Material Safety Data Sheets against each waste item? This is required for us to complete our internal Waste Profile Sheets.

A5. No. This type of information will be provided when a specific pickup of waste is needed.

Q6. If no MSDS are available for the waste items, can you provide us with a completed Waste Profile Sheet (i.e Either Attachment 5 of Solicitation or our attached herewith WPS)?

A6. No. This type of information will be provided when a specific pickup of waste is needed. See question #14.

Q7. Is the provision of manpower required for loading onto/into transporting vehicles the responsibility of bidder or the responsibility of the US Government?

A7. The USG will tailgate load the Contractor's conveyance.

Q8. Is the provision and operation of loading equipment at waste collection sites (i.e forklifts, cranes etc) in the bidder’s scope? Or is it the scope of the US Government?

A8. The USG will provide the material handling equipment (MHE) to tailgate load at the USG site being serviced. Any handling equipment needed past the tailgate load is the responsibility of the Contractor.

Q9. With respect to waste in liquid form in general, is liquid transfer facility from existing storage ((ie, Acid storage tanks if any, or IBCs (Intermediate Bulk Container) or Hand carried containers etc.. whatever may the current containment be)) to suitable transportation containers required?

A9. Due to the contingency nature of this contract, it is not possible to answer this question with 100% certainty. The Contractor should place his bid accordingly.

Q10. Reference Attachment 1, CLIN K2650 (CDE), are you referring to C-Bag and C1-Bag contents (ie. Breathing Masks, body suits etc..)? If no, kindly provide reference source for type of waste?

A10. Chemical Defense Equipment (CDE) disposed of under this CLIN could be any type of CDE being used by the USG during past or current operations.

Q11. Reference Attachment 1, CLIN K2530 “Medicine, Uncontrolled”, kindly clarify. Shall this be considered as bio-hazardous waste? Furthermore, what is the current packing type?

SP452017R0005

Q12. Reference Attachment 1, CLIN K6503 “Provide Certificate of Disposal”. Is this referring to Attachment 2 “Sample Certificate of Destruction” or are you referring to Kuwait EPA Disposal Certificates.

A12. This refers to the document in Attachment 2.

Q13. Reference Attachment 1, CLIN K6400. For unknown waste streams, what is the allocated time frame from sample collection for analysis to submission of analysis results along with filled hazardous profile forms?

A13. 21 days from issuance of the task order.

Q14. Reference Attachment 5, Hazardous Waste Profile Sheet. With regards to identified hazardous waste as listed in Bid Schedule, kindly indicate whether it is the responsibility of the Bidder to do necessary site analysis to complete Attachment 5 for each waste collection activity? Or will the US government representatives fill Attachment 5 and provide the same to successful bidder with each waste collection activity?

A14. It is the responbility of the USG to provide a current Hazardous Waste Profile Sheet (HWPS) with each waste stream. If the USG does not have a current HWPS, the appropriate CLIN may be ordered to have the Contractor complete a HWPS.

Q15. With regards to Transportation CLINs (i.e KT0400, KT0600, KT1300 etc..) listed in Bid schedule. Please Clarify what is meant by “EA” Each in Column D “U/M”? This is for us to understand and forecast the frequency and loading capacity/cycles of transportation vehicles. As an example, to clarify, Consider CLIN K0400 – Is the collection of 25 000 KG expected in one loading and transportation cycle from one site? Or multiple cycles?

A15. The transportation CLINs will be ordered in a 1:1 ration with the waste. 1 ea of the transportation CLIN means that the Contractor shall be required to transport 1 KG of waste to the TSDRF. Relative to the example (K0400), the collection of the example of 25,000 KG could be expected in one task order, but could potentially be ordered from multiple sites.

Q16. With regards to Transportation CLINs (i.e KT0400, KT0600, KT1300 etc..) listed in Bid schedule. Shall Unit price for transportation be against weight carried (Kg, Ton)? or number of expected trips cycle?

A16. The unit price for transportation will typically be ordered against the KG of the weight of the waste.

Q17. Please confirm if the Estimated Quantities in the Bid Schedule are already accumulated and ready for collection, or the same is estimated/forecasted to be generated over a period of 30 months? The reason we ask is because, the required number of visits for removal of the waste can be calculated if total quantity is available.

A17. The estimated quantities are expected to be generated during the performance period of the contract.

Q18. Kindly confirm if any of the waste is expected to be radioactive.

A18. No radioactive waste will be sent for disposal via this contract.

Q19. Kindly clarify the Invoicing Plan.

A19. See section 20 “Documentation requirements, acceptance, invoicing and tracking”.

Q20. Solicitation released on 15 August 2017 with a due date of 14 September 2017. Due to the Eid al-Adha for both employees and external vendors, it will be difficult to provide a substantial response to this bid in a manner keeping with the requirements and magnitude of this scope. Respectfully request a 1 month extension until 14 October 2017 to respond appropriately.

A20. We cannot extend the solicitation for one (1) month. We are extending the solicitation for five days.

Q21. Offerors are requested to propose the use of an in-country treatment facility. The DLA Disposition Services have no qualified facility listed for the State of Kuwait. As this contract/ a similar contract is currently running regarding hazardous waste disposal services in Kuwait, kindly provide the details of the facility currently being used?

A21. Please refer to http://www.dla.mil/DispositionServices/Offers/Disposal/HazardousWaste/HazWasteDisposal.aspx for a list of

PAGE 3 OF 7 PAGESCONTINUATION SHEET REFERENCE NO. OF DOCUMENT BEING CONTINUED:

facilities that are currently on the QFL.

Q22. Requires contractor to provide list of all intended interim and final TSDRFs in the Technical Proposal, however Offerors are not required to submit a Technical Proposal as per Part 12 Provisions, Addendum to 52.212-01 Instructions to Offerors - Commercial Items; Proposal Format and Content; page 39. Stating that only 2 volumes are required, i.e. - Volume I (Proposal Certifications and Price Schedule) and Volume II (Past Performance Information), is required to be submitted. Kindly clarify and provide updated documentation.

A22. See revised Note #9 from page 3 and revised paragraph (d) from page 40 of the solicitation which appear below.

Q23. Specifies that the driver identity will be confirmed via a current Commercial Driver's License. Such a license type is not issued in Kuwait, kindly provide the recognized Kuwait equivalent.

A23. Contractor must ensure that their drivers have the proper papers to operate their vehicle and permit to drive in Kuwait.

Q24. Specifies that Offerors must submit requests for additions to the Qualified Lists at solicitation closing date, and that a proposal will be nonresponsive if this is not done. However on page 40, Notes of Offerors, (d) 3. indicates that these request for additions to the Qualified Lists will not be used for evaluation purposes. This information is contradictory. Kindly clarify this point to understand if we must submit this for our proposal to be considered responsive or not.

A24. See revised Paragraph 15a of the solicitation below. Also, see question #22.

Q25. Section ends with: "…expiration [insert the period of time within which the Contracting Officer may exercise the option]". Kindly provide an update document with the period of time inserted within which the CO may exercise the option.

A25. The Contracting Officer may exercise the option by written notice to the Contractor within 7 days prior to contract expiration

Q26. Section contains: "…expiration [insert the period of time within which the Contracting Officer may exercise the option]...". Kindly provide an update document with the period of time inserted within which the CO may exercise the option.

A26. (a) The Government may extend the term of this contract by written notice to the Contractor within 10 days prior to contract expiration; provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 60 days before the contract expires. The preliminary notice does not commit the Government to an extension.

Q27. Indicates that offers submitted by e-mail are submitted at the contractor's risk, in the first paragraph of the same section it states: "You must submit your offer via e-mail." Kindly explain how to mitigate or eliminate the risk if the USG document clearly instructs offerors to ONLY submit via email and provide NO other way of proposal submission, yet DLA Disposition Services places risk on offeror because of submitting proposal via email. Kindly clarify as your instructions are contradictory and not fair.

A27. Kindly submit your offer via e-mail to james.e.smith@dla.mil and laura.welsh@dla.mil in sufficient time to be received prior to the solicitation closing time.

Q28. Kindly clarify the PPI requirements to submit: a minimum of three past performance projects + the Past performance of two Key Personnel (i.e. 3 Past Performance Projects + 2 Key Personnel = 5 PP items); OR can the past performance of two Key Personnel be submitted as part of the minimum requirement of three past performance projects (i.e. 2 Key Personnel + 1 Past Performance Project = 3 PP Items). Kindly clarify this matter.

A28. A maximum of three (3) reference surveys may be submitted for each offeror. A reference survey may be for either the firm, a principal subcontractor, or a key employee of the firm. Only the first three reference surveys received will be accepted. However, we will consider all recent and relevant CPARS evaluations in our past performance evaluations.

Q29. Offerors are required to add and to update any proposed disposal facility to the DLA Qualified Facility, PAGE 4 OF 7 PAGESCONTINUATION SHEET REFERENCE NO. OF DOCUMENT BEING CONTINUED:

kindly clarify with which volume is this to be submitted with as this is not indicated in the documentation.

A29. Information will not be contained in volume I or II. Please refer to question 22.

A33. The Contractor must attest that the waste requiring the certificate of disposal/destruction has been disposed of or destroyed per the requirements outlined in the contract.

A34. See question 22.

Q36. It's noted that USG will load the containers on to Contractor's Truck however please confirm who will be loading the Hazmat waste into the container? Will this be handled by USG or is this included in the contractor's scope of work?
