COMBINED SYNOPSIS/SOLICITATION FOR FURNITURE

DLA DISPOSITION SERVICES MID-AMERICA, FORT SAM HOUSTON, TX

Please complete and return the enclosed bid schedule as your pricing submission. Note that prices may only be to two digits past the decimal place, and the Government will use standard rounding practices to round any unit prices submitted that are beyond two decimal places

Notes:

1. This procurement is a 100% Small Business set-aside. The manufacturing NAICS code is 337127, with a size standard of 500 employees. As the Small Business Administration has granted a class waiver to the "nonmanufacturer rule" (FAR 19.102(f)), a small business offeror 1) may furnish the supplies of a large domestic manufacturer, 2) may have no more than 500 employees, and 3) is not subject to usual limitations on subcontracting (FAR clause 52.219-14).

2. A site visit is strongly encouraged. The POCs to contact for site visits are:

The facility is available between 7:00 Am and 3:30 PM, Monday through Friday, by advance arrangement with the above POCs for those offerors wishing to do so.

4. Requirements are for:

- Labor for delivery, installation, and removal of replaced furniture IAW the PWS

- Manufacturer’s and/or offeror’s standard commercial warranty shall apply

- Preferred colors are noted in the bid schedule, as applicable

- Furniture shall be delivered within 60 calendar daysafter contract award

- Prices shall include all FOB Destination shipping, assembly, and installation charges

DLA Disposition Services San Antonio requires this furniture to be brand name or equal to the items identified in the bid schedule. The offeror shall provide pictures/brochures with its bid for mailto:Christopher.Atkinson@dla.mil mailto:Harland.valiquette@dla.mil mailto:Erin.Clore@dla.mil each item it proposes to furnish, and the relevant specifications for each item, in order for the Government to determine that they are the equal to the requested make/model/part number set forth in the bid schedule.

Upon completion of services, the contractor shall be required to clean up the work area and remove and and/or dispose of any furniture, supplies, tools, and other debris from the installation location. This shall be required after each workday of on-site performance. All contractors are required to display appropriate contractor identification while on Federal property. Any materials that are lost or damaged during the transfer shall be replaced by the contractor at their cost.

PERFORMANCE WORK STATEMENT (PWS)

1.0 DESCRIPTION OF SERVICES

The contractor shall provide all necessary personnel, equipment, tools, material, and supervision required to remove all old furniture and appliances currently located in the break room, conference room, offices, and related areas of Building 4199 to the warehouse, Building 4195, across the street for disposal by the Government, in preparation for unloading, assembling, and installing the replacement furniture and appliances set forth in the Bid Schedule in their respective locations. All trash and debris resulting from this work shall be removed and disposed of by the contractor. The work shall be completed within 60 days after award of the contract.

All contractor personnel are required to have approved background checks by Fort Sam Houston prior to commencing work. All details regarding this requirement may be obtained from the previously noted Government POCs.

2.0 GOVERNMENT FURNISHED PROPERTY AND SERVICES: Existing utilities will be made available without charge to the contractor. The contractor shall assume the risk of, and shall be responsible for, any loss or damage thereto as a result of contractor’s performance.

3.0 TIMES OF PERFORMANCE

The work to be performed under this contract shall be performed during the normal working hours of 7:00 AM – 3:30 PM, Monday through Friday, excluding Federal Holiday listed below, as designated by Presidential proclamation, or other DLA Disposition Services closure.

4.0 DLA DISPOSITION SERVICES ENVIRONMENTAL MANAGEMENT SYSTEM

(EMS)

DLA Disposition Services implemented EMS in accordance with ISO 14001:2004(E), DOD and DLA requirements at all DLA Disposition Services appropriate facilities. An EMS is an overall management system that includes organizational structure, planning activities, responsibilities, practices, procedures, processes and resources for developing, implementing, achieving, reviewing and maintaining the environmental policy of an organization. The DLA Disposition Services EMS is designed to ensure that all personnel, including contract personnel, whose work activities can cause real or potential significant environmental impact, are aware of how their work supports the EMS and are competent to perform their roles, responsibilities and authorities while supporting the DLA Disposition Services mission. The DRMS EMS program is explained in DRMS-I 6050.2, Environmental Management System (EMS) Core Instruction. Site supplements detail information required by EMS at a local level such as local, state and host environmental regulations as well as local emergency procedures and operating procedures unique to the location. Site supplements also contain information concerning the local interaction between the DLA Disposition Services tenant appropriate facility and the DOD/DLA host facility EMS program. Contractors and their personnel are required to be aware that each DLA Disposition Services appropriate facility has an EMS and to be aware of EMS policies to perform work and/or gain access to an installation. Each DLA Disposition Services appropriate facility maintains EMS awareness posters that identify the DLA Disposition Services Environmental Management Representative (EMR) and Host facility EMR for specific guidance concerning an individual facilities’ EMS program. Contractors shall request current copies of EMS information identified in this paragraph by sending an e-mail request to DRMSEMR@dla.mil.

5.0 INDEMNIFICATION

The Government assumes no liability for any damage to the property of the Contractor, to the property of any person, or public property or for personnel injuries, illness, disabilities or death to the Contractor, Contractor’s employees, and any other person subject to the Contractor’s control or any other person including members of the general public; caused in whole or in part by, (a) the Contractor’s breach of any term or provision of this contract; or,

(b) any negligent or act of omission of the Contractor, its employees or subcontractors in the performance of this contract. The Contractor also shall to hold the Government harmless and indemnify the Government for any and all costs, including those that arise from violation of environmental regulations, Department of Transportation (DOT), or any similar state enforcement programs under which the government incurs environmental clean-up or response costs, judgments, action, debt, liability costs and attorney’s fees or any other requests for monies or any other type of relief arising from or incident to the processing, transporting, and disposal of any subsequent operation performed upon, exposure to or contact with any component, part, constituent or ingredient of this item, material or substance, whether intentional or accidental.

6.0 CONTRACTOR’S PERSONNEL

Conduct of Personnel: While on duty working in Government buildings and on Government property, personnel employed by the contractor shall be prohibited from sleeping, using Government telephones or utilizing Government facilities for recreational activities. Personal items in office areas, such as desk accessories, radios, supplies, pens, pencils, plants and food shall not be disturbed by the contractor’s employees at any time.

Contractor’s employees shall be subject to Government standards of conduct including sexual harassment. Clothing, personal grooming, and personal cleanliness standards for Government working environment shall apply.

Personnel Appearance: Contractor personnel shall present a neat appearance and be easily recognized. This shall be accomplished by wearing clothing bearing the name of the company or by wearing an appropriate badge containing the company name and employee name. The military uniform, or any part thereof, shall not be worn by contractor’s employees while performing work under this contract. Additionally, employees shall be fully attired.

Standards of Conduct: All personnel employed by the contractor in the performance of this contract, or any representative of the contractor entering Offutt AFB and DLA Disposition Services Offutt compound/facilities, shall obey all regulations of installation and DLA Disposition Services Offutt and shall be subject to surveillance to ensure that no violations occur. The contractor shall be responsible for maintaining satisfactory standards of employee competency and conduct and taking such disciplinary actions with respect to contractor employees as may be necessary. The contractor shall ensure that all personnel employed in the performance of this contract shall be United States citizens; or non-United States citizens who legally reside in the United States. No contractor employee shall be permitted on DLA Disposition Services Offutt Compound when investigation reveals employee’s presence would be detrimental to the security of the installation or to the accomplishment of the work prescribed. The Government reserves the right to refuse to permit any contractor employee to perform services under this contract who is not in compliance with requirements of this contract (for example, employees not meeting stated minimum qualifications). In such cases, the KO/COR shall advise the contractor of the reason for requesting an employee’s removal or withdrawal of authorization to enter DLA Disposition Services Offutt Compound.

Disorderly Conduct: Disorderly conduct, such as use of abusive or offensive language, quarreling, intimidation by works or actions, or fighting shall not be condoned. Contractor employees shall not participate in disruptive activities which interfere with normal and efficient Government operations.

Intoxicants: The contractor shall not allow any employees, while on Offutt AFB or the DLA Disposition Services Offutt premises, to possess, sell, consume, or be under the influence of intoxicants, illicit drugs, or any other incapacitating agent which produces similar effects. The KO/COR shall require the contractor to remove from the job site, any contractor employee found under the influence of intoxicants, illicit drugs, or incapacitating agent during his tour of duty.

Criminal Actions: Contractor employees shall be subject to prosecution for criminal actions. These shall include, but are not limited to, the following:

a. Falsification, unlawful concealment, removal, mutilation, or destruction of any official documents or records, or concealment of material facts by omission from official documents or records.

b. Theft, vandalism, immoral conduct, or unauthorized use of Government property.

c. Unethical or improper use of official authority or credentials.

d. Misuse of weapons, including possession of weapons on Offutt AFB on DLA

Disposition Services Offutt.

e. Violations of State laws, such as traffic offenses, which have been assimilated and are enforced through the U.S. Magistrate Court System.

f. The contractor’s employees shall be on Offutt AFB on DLA Disposition Services Offutt premises only for contractual duties and not for other non-contractual purposes.

Cleanup of Spills or Leaks: The contractor is solely responsible for any and all spills or leaks during the performance of this contract that occur as a result of or are contributed to by the actions of its agents, employees, or subcontractors. The contractor shall clean up such spills or leaks in a manner that complies with applicable environmental and Safety laws and regulations, Federal, and local laws and regulations. This clean-up shall be at no cost to the Government.

7.0 PROTECTION OF PUBLIC AND PROPERTY

The contractor shall fully comply with all Federal, state, local, Fort Sam Houston, and DLA Disposition Services San Antonio fire, safety, security, and environmental protection laws, regulations, and policies. The contractor, its employees, and any subcontractors shall perform all operations in a prudent, conscientious, safe, and professional manner. The contractor shall insure that its employees, while on Fort Sam Houston, are provided with and utilize all personal protective equipment such as safety shoes, goggles, etc., as may be appropriate to the work to be performed. The contractor shall be responsible for obtaining all necessary permits and licenses that may be required to perform the work under this procurement at no additional cost to the Government. The contractor shall take all necessary precautions to insure that its employees and subcontractors prevent any injury to the public, building occupants, and property while performing on Fort Sam Houston.

8.0 AWARD EVALUATION CRITERIA

The Government will award a contract resulting from this solicitation to the responsible offeror, whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following criteria will be used to evaluate offers:

9.0 OFFEROR PROPOSAL SUBMISSION

1. In addition to their proposed price, each offeror shall also provide picture(s)/ brochure(s) of their proposed items and the relevant specifications for each item in order for the Government to determine that they are equal to the requested make/ model/part numbers set forth in the bid schedule.

NOTE: These must be less than 5 MB. If larger, they must be e-mailed in parts of not more than 5 MB each.

2. All proposals shall be e-mailed only, to Erin.Clore@dla.mil not later than 10:00 AM Eastern Time, Wednesday, 27 September 2017.

10.0 INVOICING AND PAYMENT

DFARS 252-232-7006 WIDE AREA WORKFLOW PAYMENT INSTRUCTIONS

(MAY 2013)

(a) Definitions. As used in this clause—

“Department of Defense Activity Address Code (DoDAAC)” is a six position code that uniquely identifies a unit, activity, or organization.

“Document type” means the type of payment request or receiving report available for creation in Wide Area WorkFlow (WAWF).

“Local processing office (LPO)” is the office responsible for payment certification when payment certification is done external to the entitlement system.

(b) Electronic invoicing. The WAWF system is the method to electronically process vendor payment requests and receiving reports, as authorized by DFARS 252.232-7003, Electronic Submission of Payment Requests and Receiving Reports.

(c) WAWF access. To access WAWF, the Contractor shall—

(1) Have a designated electronic business point of contact in the System for Award Management at https://www.acquisition.gov; and

(2) Be registered to use WAWF at https://wawf.eb.mil/ following the step-by-step procedures for self-registration available at this web site.

(d) WAWF training. The Contractor should follow the training instructions of the WAWF Web-Based Training Course and use the Practice Training Site before submitting payment http://www.acq.osd.mil/dpap/dars/dfars/html/current/252232.htm https://www.acquisition.gov/ https://wawf.eb.mil/ requests through WAWF. Both can be accessed by selecting the “Web Based Training” link on the WAWF home page at https://wawf.eb.mil/

(e) WAWF methods of document submission. Document submissions may be via web entry, Electronic Data Interchange, or File Transfer Protocol.

(f) WAWF payment instructions. The Contractor must use the following information when submitting payment requests and receiving reports in WAWF for this contract/order:

(4) Payment request and supporting documentation. The Contractor shall ensure a payment request includes appropriate contract line item and subline item descriptions of the work performed or supplies delivered, unit price/cost per unit, fee (if applicable), and all relevant back-up documentation, as defined in DFARS Appendix F, (e.g. timesheets) in support of each payment request.

(1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor’s directly pertinent records involving transactions related to this contract.

If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. (e) WAWF methods of document submission. Document submissions may be via web entry, Electronic Data Interchange, or File Transfer Protocol.

(4) Payment request and supporting documentation. The Contractor shall ensure a payment request includes appropriate contract line item and subline item descriptions of the work performed or supplies delivered, unit price/cost per unit, fee (if applicable), and all relevant back-up documentation, as defined in DFARS Appendix F, (e.g. timesheets) in support of each payment request.

(5) WAWF email notifications. The Contractor shall enter the e-mail address identified below in the “Send Additional Email Notifications” field of WAWF once a document is submitted in the system.

“Department of Defense Activity Address Code (DoDAAC)” is a six position code that uniquely identifies a unit, activity, or organization.
“Document type” means the type of payment request or receiving report available for creation in Wide Area WorkFlow (WAWF).
“Local processing office (LPO)” is the office responsible for payment certification when payment certification is done external to the entitlement system.
