Attachment 1 RFQ SP3100-8043-1001
PART 1 – SCOPE OF WORK - SCALE MAINTENANCE
1.1 SPECIFIC REQUIREMENTS. The Defense Distribution Depot Warner Robins, Georgia has a requirement to acquire scheduled and unscheduled maintenance for various types of scales used to perform distribution functions (e.g., receiving, storage, and issue).
The Contractor shall provide all supervision, qualified labor, materials and supplies, equipment, and transportation to perform the requirements in this SOW that include, but are not limited to, calibration and maintenance of Government-owned scales. All scales are located at Defense Distribution Depot, Warner Robins, GA (DDWG).
1.1.1 Scheduled Maintenance. The Contractor shall perform scheduled (preventative maintenance (PM)) in accordance with (IAW) Original Equipment Manufacturer (OEM) specifications to optimize equipment service life. The Contractor shall coordinate scheduling of equipment due for PM at least one week in advance of the scheduled PM date. The Contractor shall perform PM on the equipment so that mission, security, and safety are not impacted. The Contractor shall perform an annual calibration and certification test according to the OEM specifications and maintain records on completed calibration to include results, and will make available to the Government on request. If any repairs are identified during PM the Contractor shall request authorization from the KO or designee to repair the scale.
1.1.2 Unscheduled Maintenance. The Contractor shall perform miscellaneous service repairs to Government-owned scales as needed. The Contractor shall respond to a “work stoppage” trouble call to repair equipment within 24 hours of receipt of notification by the COR or Technical Representative and a “non-work stoppage” within 48 hours of notification by the COR or Technical Representative. The Contractor shall diagnose needed repairs within one hour of arrival at the site and notify the Government to determine if it is economical to repair the scale.
1.1.3 The Contractor shall provide scheduled and unscheduled maintenance on the following types and numbers of scales. (See Attachment 2 for additional information)

1.2 INSPECTION AND ACCEPTANCE
Upon completion of the work, the COR and Technical Representative will inspect and test all services performed under this SOW to the extent practicable.
PART 2 GENERAL
2.1 INSTALLATION AND DDWG ACCESS
a. Normal operating hours for DDWG and Robins AFB are 0700 to 1600 hours, Monday through Friday except Federal Holidays. In the event an Executive Order (EO) issued by the President of the United States declares Agencies of the Federal Government closed for a regularly scheduled workday, the Contracting Officer’s Representative (COR) will determine and advise the contractor on whether services are required for that day.
b. The Contractor shall enter and register at the main gate located at Gate 3. The Contractor shall provide a valid Drivers License, vehicle registration, and proof of insurance to receive an ID badge and vehicle pass to the Installation.
c. The Pass and ID Office hours of operation are 0730-1530 hours, Monday through Friday, closed Saturday, Sunday, and all Federal Holidays.
d. The Contractor’s vehicles shall post the vehicle pass in the front vehicle window.
2.1.1 Searches
All vehicles and personnel are subject to search and seizure of contraband and/or unauthorized Government property. Contractor vehicles (Contractor-owned and personal), Contractor employees, and their personal property shall be subject to searches upon entering or leaving the Installation.
2.2 CONTRACTOR PERSONNEL
For the purpose of this contract, the term “Contractor Personnel” applies to all Contractor employees, subcontractor employees, or any other person(s) acting for or on behalf of the Contractor to perform work on this contract.
2.2.1 Employee Qualifications
The Contractor shall ensure all employees performing work are qualified. Contractor personnel shall be physically capable of performing work under all climatic conditions.
2.2.2 Employee Conduct
Contractor employees shall be appropriately dressed, including safety shoes. No employee having possession of, or under the influence of alcohol or controlled substances shall be permitted access to the Government installation. Government rules, regulations, laws, directives, and requirements which are in place or issued during the contract term relating to law and order, Installation administration, and security on the Installation shall be applicable to all Contractor employees or representatives who enter the Installation. Violation of such rules, regulations, laws, directives, or requirements shall be grounds for removal (permanently or temporarily as the Government determines) from the work site. Individuals who violate such rules, regulations, laws, directives, or requirements may be denied access to the Installation, either temporarily or permanently.
2.2.3 Personnel Constraints
2.2.3.1 Conflicts of Interest
The Contractor shall not hire for this contract any person whose employment would result in a conflict of interest, or employment, which is prohibited by DoD Joint Ethics Regulation 5500.7-R.
2.2.3.2 Restricted Personnel
The Contractor shall not employ persons for work on this contract if such employee is considered by the Installation Commander to be a potential threat to the health, safety, security, general well being, or operational mission of the Installation and its population.
2.3 PERMITS, TAXES, LICENSES, ORDINANCES, AND REGULATIONS
The Contractor shall, at his own expense, obtain all necessary permits, give all notices, pay all license fees and applicable taxes, comply with municipal, state, and federal laws, ordinances, rules, and regulations applicable to the business carried on under this contract.
2.4 ENVIRONMENTAL REGULATIONS
a. The Contractor shall be knowledgeable of and fully comply with all Federal, State, and local environmental laws and regulations. In case of a conflict among these laws and regulations, the most stringent law or regulation shall apply. In the event environmental laws, regulations or requirements change during the term of this contract, the contractor shall comply with such law or regulation as changed.
b. If as a result of Contractor noncompliance with any applicable environmental law or requirement, the Government incurs any cost (e.g., the cost of cleaning up hazardous waste spill); the Contractor shall indemnify the Government and provide reimbursement for these costs. If a Federal, State, or regulatory agency assesses any fine, penalty, or fee against the Government that is attributable to Contractor noncompliance with any environmental law or requirement, the Contractor shall indemnify the Government for the amount of the fine and other Government costs attributable to the violation. In the event of Contract noncompliance with any applicable environmental law or regulation, the KO will determine whether the Government or the Contractor is responsible for abating or correcting the violation. This provision is not intended in any way to waive the Government’s rights under any provision of the contract.
c. If the Contractor spills or releases any hazardous chemicals/substances into the environment, the Contractor shall immediately report the incident to the COR. The Contractor shall be liable for containment and environmental clean-up of the spill or release of hazardous chemicals/substances.
d. The Contractor shall be responsible to acquire and to maintain Material Safety Data Sheets (MSDS) on all materials required by State and Federal Laws and/or Regulations.
2.5 SERIOUS ACCIDENTS AND INCIDENT REPORTING
The Contractor (to include subcontractors) shall report all accidents and incidents, including occupational illnesses and injuries, encountered while performing work under this contract. Reports shall be made to the COR or technical representative not more than one hour following the incident or accident.
2.6 DAMAGES TO GOVERNMENT FACILITIES AND/OR EQUIPMENT
All damages to Government property as determined by the COR/Technical Representative caused by the negligence of the Contractor or his employees during contract performance shall be repaired by the Contractor or by the Government at the Contractor’s expense, as determined by the Government. If repaired by the Government, any money due the Government will be deducted from the Contractor’s payments. The Contractor will be advised in advance of any such deductions.
2.7 EMPLOYEE CONFIDENTIALITY AND DISCLOSURE STATEMENT
In the event that performance under subsequent award requires that the contractor gain access to proprietary information of other companies, the contractor shall be required to execute written agreements with those companies to protect the information from unauthorized disclosure and refrain from using it for any purposes other than for which it was intended, as prescribed by FAR 9.505-4. A copy of all executed agreements shall be delivered to the Contracting Officer of the award. All information disclosed by the Government to the contractor under the award shall be deemed to be “sensitive and/or proprietary”, wither or not designated or marked as such. Any employee of the contractor working on this award shall be required to read and sign the “Employee Confidentiality and Disclosure Agreement” which is included in this contract.
PART 3 DEFINITIONS AND ACRONYMS/ABBREVIATIONS
3.1 Definitions
Contracting Officer (KO): A person with the authority to enter into, administer and/or terminate contracts, and make related determinations and findings.
Contracting Officer’s Representative (COR): A person who monitors the contract and reports to the Contracting Officer with updates on there findings.
Quality Assurance and Acceptance: A term or condition used in contracts that outlines the Governments rights to inspect and accept supplies and/or services.

PART 4 – GOVERNMENT FURNISHED PROPERTY AND/OR SERVICES
The Government will not provide facilities to store Contractor owned supplies and equipment.
PART 5 – CONTRACTOR FURNISHED PROPERTY AND/OR SERVICES
5.1 Contractor-owned or Leased Vehicles. The Contractor shall maintain contractor-owned or leased vehicles used in the performance of these requirements in a safe, operable condition. Any contractor vehicles used in the performance of this contract shall have the company name prominently displayed on both sides of the vehicle. The Contractor shall ensure that all contractor-owned vehicles used in the performance of this contract to meet the Federal, State, and local safety requirements.
