STATEMENT OF WORK

FOR

REPAIRS TO WATER WELL #3

Dated 13 October 2022

1. GENERAL

DESCRIPTION AND SCOPE OF WORK. The contractor shall provide all labor, equipment, tools, and supplies necessary to accomplish the work specified below. Contractor is responsible for keeping construction area(s) neat, secure, orderly, and in compliance with OSHA and MO DNR standards.

Contractor will be responsible for cleanup upon project completion. Contractor shall comply with all OSHA, MO 10 CSR 60, and American Water Works Association (AWWA) guidelines. Contractor will be responsible for the storage, transportation, and disposal of all demolition and waste materials at an off-base location. The Contractor shall become familiar with all details of the work, verify all dimensions in the field and specifications stated herein, and shall advise Contracting Officer of any discrepancy before performing the work. All work to be accomplished during regularly established Whiteman AFB duty hours (0700-1630). The Contractor must be a certified well puller who is recognized by the state of Missouri. All Contract personnel are required to have a valid state issued photo identification card, be a US Citizen, and no criminal record to enter the installation. All work shall be coordinated with Base Civil Engineering and Water Treatment Plant personnel.

2. PRINCIPLE ITEMS OF WORK.

− This work is located at Raw Water Well #3, facility 2000, located at 719 Mitchell Ave.

− Contractor shall disconnect well piping and power to well motor. Using roof access, the contractor will remove 462 ft of 4” steel well piping to reach the well pump and motor. (Each section of 4” steel piping is 21 ft in length)

− Once the well pump and motor is removed, the contractor will install new well pump and motor, (50 hp, 460V, 3 ph motor and 5 stage Grundfos well pump or comparable to deliver the requirements below), 462 ft of 21’L x 4”D galvanized well piping, two 4” stainless steel Flomatic check valves, or comparable, with stainless steel nipples and couplings. Install new 480’ of 2/3 w/g flat jacketed wiring and ¼” air line with gauge for performing monthly static and pumping levels. New well pump will be required to provide a minimum of 300 gpm at 448 total dynamic head.

− Once the well pump is reinstalled the contractor will perform a pump test and provide the results to the government on a Pumping Test Data Sheet to insure it meets the pumping requirements desired

− Contractor shall ensure no contaminants are introduced into the well during their repairs.

− Contractor will remove and discard old piping, pump/motor and any other debris from site at an off-base location.

3. PERMITS/REPORTING

Contractor shall file all required reports in accordance with MO DNR and AWWA where applicable.
