Statement of Work

1.0 Introduction

This Statement of Work (SOW) defines the requirements of the Department of Veterans Affairs (VA) in establishing a fixed-price Brand name or equal purchase order for ultrasound tables and exam tables.

1.1 Background

VA Jonathan M Wainwright Medical Center (JMWMC) requires ultrasound tables and exam tables supply to outfit the facility.

1.2 Scope

The scope of this purchase order is to provide JMWMC with a brand name item or equal for delivery to the Walla Walla VA location. Contractors shall furnish ultrasound tables and exam tables. Estimated delivery dates are November 1st, 2024 to November 15th, 2024.

The delivery schedule is based upon the current construction schedule, which is subject to change. The vendor shall not increase prices for a change in delivery date for up to 60 days due to construction delays. The awardees' actual delivery dates will be confirmed by VA upon award.

2.0 General Requirement

Ultrasound tables and exam tables are required to outfit radiology.

All equipment to be supplied under the specifications shall be the newest and the most current model of a standard product of a manufacturer of record. A manufacturer of record is defined as a company whose main occupation is the manufacture for sale of the items of equipment and which:

a. Maintains a factory production line.

b. Maintains a stock of replacement parts for the item.

c. Maintains engineering drawings, specifications, operating manuals, and maintenance manuals.

d. Has published and distributed descriptive literature and equipment specifications on the equipment.

Key Features:

1. M9005 - Table, Imaging, Ultrasound

a. Three section ultrasound imaging table.

b. Torso section with Fowler back positioning, center section remains fixed and leg section drops down to 40° and 80° for stirrup access.

c. Motorized height range: 23"-39”

d. Must include Hand control, two body straps, Side rails and stirrups.

e. 500 lb. patient capacity.

2. M9035 - Chair/Table, Exam/Treatment, Combination Unit

a. A dual purpose examination and treatment/procedure chair/table combination unit.

b. Unit consists of an upholstered chair with foot and head sections.

c. Unit is designed for use in clinics, doctor's office or treatment rooms.

d. Table allows for patient positioning that results in more accurate vital sign collection.

e. Table/chair provides back support to patient while sitting upright

f. Barrier free exam table.

g. Allows for easy transition from wheelchair/scooter to exam chair

h. Adjustments to table are high low and back raise lower.

i. Table shall be able to lower to 15" Above floor to up to 37" above floor.

j. Table/Chair upholstered top shall be approximately 28"w x 60" long (76" long with footrest extended)

k. Weight capacity 650 lb.

l. Stirrups

m. Pelvic tilt

n. Accessory receiver

o. Tamper resistant hospital grade receptacles on both sides of table

p. Heated drawer

q. Must include Patient support arms/rails that can have blood pressure taken on.

r. Must include Detachable gooseneck LED exam light

s. Dimensions: 28" wide x 76" long 15"-37" high

t. Electrical 120V cord and plug

u. Finish Options: Upholstery: Dark Linen 856

3.0 Work Hours:

Normal business hours are 8:00AM to 4:30PM Monday through Friday excluding Federal Holidays. Work completed outside this time shall be requested through the Point of Contact.

The Government recognized US holidays are: New Year's Day, Martin Luther King, Jr. Day, Presidents Day, Juneteenth, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, and Christmas Day. If the holiday falls on Sunday, it is observed on Monday.

4.0 Delivery and Receiving:

Delivery and receipt of the proposed items is anticipated to be directed to the location identified below. Confirmation of delivery location will be provided by the VA POC post-award. Delivery must be coordinated with VA POC Kati Stocke 509-525-5200 x26751, kati.stocke@va.gov or Joshua Freer at 509-525-5200 x26132, Joshua.freer@va.gov prior to any attempts at delivery. Any item that has not been coordinated will be turned away and any additional charges will be the responsibility of the sender/transportation company.

Delivery shall be freight on board (FOB) – Destination.

The vendor is required to define the need for a staging area to accommodate item delivery, if needed. Post-award, this information will be confirmed with the vendor.

6.0 Warranty and Support:

Warranty of the equipment shall be provided for at least one year upon receipt and acceptance of the equipment.

VA Engineering will have full access to the hardware that constitutes the system. The Contractor shall provide one (1) set of user manuals and technical manuals to VA Engineering.

A copy of the service report shall be provided to both the using Service and Biomedical Engineering.

The contractor shall verify to Engineering that the system meets all requirements stated in this contract through demonstration and validation.

7.0 Use of Premises

If during the performance of this contract, work will be carried out on VA JMWMC premises, the vendor shall perform all work in such a manner that will cause a minimum interference with VA JMWMC operations and the operations of other contractors on the premises.

The vendor shall communicate through the VA POC any required coordination requirements with the General Construction contractor prior to and during delivery and installation activities.

The project may be an active construction site during delivery and installation. The vendor shall take all necessary precautions to protect the premises and all persons and property therein from damage or injury. The vendor shall assume responsibility for taking precautions for the vendor's (and associated subcontractors’) employees, agents, licensees, and permittees. The vendor shall abide by any safety requirements imposed by the GC (e.g., hard hats, safety vests).
