Statement of Work
1. DESCRIPTION OF THE SERVICES REQUIRED:
2. 1. Provide project Insurance, Supervision and Mobilization.
3. 2. Provide construction surveying, layout and staking.
4. 3. Provide performance and payment bonds.
5. 4. Provide, prior to construction, synthetic turf shop drawings.
5. Provide final punch-out and clean-up of the completed project.
2. LOCATION OF WORK:
13th ASOS 8420 SkyRaider LN Bldg. 9093 Colorado Springs, CO 80913-4102
3. DESCRIPTION OF SUPPLIES NEEDED:
Synthetic Turf Field
Scope of work:
1. Shock Pad – Install approximately 3,480 square feet of 10 mm shock pad at the synthetic turf area.
2. Provide and install approximately 3,480 square feet of Performance Turf.
100% Polyethylene Extruded Monofilament synthetic turf system with the noted installation options listed below.
Black Turf Football markings in white turf, per attached rendering SBR rubber infill Rubber transition strips at exposed edges Provide 8-year manufacturer warranty After synthetic turf installation is complete, will provide an operation and maintenance orientation for care of the turf field.
4. SCHEDULING OF WORK & ACCESS TO GOVERNMENT FACILITIES:
4.1 Contractor personnel shall present a neat appearance, have proper identification, and be easily identified as contractor employees.
4.2 Contractor shall ensure each employee obtains required pass and identification items for access to the base. The Pass and Registration Section will issue these items. The contractor and employees shall return identification cards if issued to Fort Carson Visitor Center within 24 hours of completion of work.
4.3 If at any time during performance of this contract the commander finds it in the best interest of the Government, the commander may order the contractor to remove any of his personnel from further performance under this contract for security reasons or for violation of installation regulations. In the event it becomes necessary to replace any contracted personnel for any of the above reasons, the contractor shall bear all costs, including relocation costs, associated with such removal and replacement.
4.4 Contractor shall not bring any hazardous material onto Fort Carson nor use hazardous materials in any Fort Carson facility not related to the installation of the ball returns and its system.
4.5 Weapons, Firearms, and Ammunition: Contract employees are prohibited from possessing weapons, firearms, or ammunition on themselves or within their contractor-owned or privately-owned vehicles.
4.6 Temporary Visitor Pass. Temporary Visitor Pass will be issued versus contract badge due to contract services will be less than 30-days to complete. Contractor personnel and Contractor representatives, while on duty, shall have on their possession Contractor provided temporary visitor pass. The visitor pass shall display at a minimum: company name, employee name, employee photo, and expiration date. The visitor pass shall be on the possession of the contractor or displayed on the dashboard of contractor vehicle while on Fort Carson.
4.7 Contractor tools and pilferable supplies must be removed from the facility and/or secured by the contractor at the end of every business day unless coordinated with 13th ASOS. The13th ASOS assumes no responsibility for lost or stolen tools, supplies, or equipment.
9. HOURS OF OPERATION.
5.1 The scope of installation should be scheduled between the hours 0700 and 1700, Monday through Friday.
5.2 Federal Holidays. Contractor performance shall not be required on the holidays listed below unless otherwise notified by the Contracting Officer.

When one of the above designated legal holidays falls on a Sunday, the following Monday will be observed as a legal holiday. When a legal holiday falls on a Saturday, the preceding Friday is observed as a holiday. This list of holidays relates to Government duty days, and is not intended to supplement or otherwise alter the provisions of the Wage Determination regarding paid holidays.
5. SUPPORT & INSTALLATION REQUIREMENTS:
· Products must be delivered NLT 60 days after award
· Installation must occur in 60 business days or less
· Must include all necessary parts and full installation
6. AVAILABILITY OF UTILITIES:
All reasonable quantities of water and electricity will be made available to the contractor without charge. The contractor shall be responsible for all connections necessary to hook up to the utilities and removing same upon termination of work.
7. RESPONSIBLITY OF THE CONTRACTOR:
The contractor assumes full responsibility for any and all government property damaged in whole or part from negligent acts or omissions by the contractor, subcontractor, or any employee, agent or representative of the contractor. The contractor also agrees to reimburse the government for any and all damage of whatever kind to any and all government property associated with the performance of this service.
8. DISPOSAL OF MATERIALS:
The contractor shall contract a container or dumpster to dispose of all debris generated by this contract at an off-base location in accordance with state, local and federal ordinances. All hazardous materials shall be disposed of off base in such a manner that meets federal, state and local environmental regulations.
