FEDERAL EMERGENCY MANAGEMENT AGENCY (FEMA)

STATEMENT OF WORK (SOW)

Caribbean Survivor Response Resource - Rice

1.0 Introduction/Background

Federal Emergency Management Agency (FEMA) Logistics Management Directorate has a national emergency response meal requirement to support 20% of Puerto Rico’s population of 3.4 million habitants for 10 consecutive days during presidentially declared national disasters. This represents a total of 680,000 impacted individuals. Due to the massive amounts of commodities that are required to feed the population, courses of action have been identified to ensure options are considered to address the feeding needs of the impacted individuals.

2.0 Objective

FEMA seeks vendors that possess the capability to purchase or manufacture 3 lbs. white medium grain rice to support disaster survivors during emergency responses. In the recent years, FEMA expanded its footprint of warehouses in Puerto Rico to store initial response resources (IRR). In addition to the 7 million meals FEMA staged in the Caribbean, as part of supporting the feeding plan, FEMA will procure approximately 75,000 lbs. of rice (25,000 bags of 3 lbs. rice).

3.0 Period of Performance

All commodities (3lbs packaged rice) shall be delivered within 30 days of contract Award.

4.0 Requirements

a. The contractor/manufacturer shall provide 25,000 bags of 3lbs White Medium Grain Rice.

b. The contractor shall deliver the pallets of rice to the delivery location within 30 days of contract award.

c. Labeling/Packaging/Markings

Note: ALL LABELING SHALL BE IN ACCORDANCE WITH FDA AND USDA

REQUIREMENETS INCLUDING NUTRITIONAL FACTS LABELING IN

ACCORDANCE WITH THE NUTRITIONAL LABELING AND EDUCATION

ACT (NLEA). MAXIMUM SIZE OF PRINT FOR SUCH REQUIRED

USDA/FDA LABELING SHALL BE THE MINIMUM SIZE PERMITTED BY

THOSE REQUIREMENTS AND WITH THE REQUIREMENTS STATED

BELOW.

All packing shall list production date, lot number and expiration date. Expiration date shall be no earlier than 12 months after production date.

Individual packages shall be labeled with the Nutrition Facts’ in accordance with the Nutrition Labeling and Education Act (NLEA) and all other applicable FDA/USDA regulations.

All items shall be packaged in the contractor’s standard commercial casing which shall be shipped on industry standard size pallets (48 inches long by 40 inches wide) with 3lbs bags of rice.

Shrink wrap shall be fully over all sides (excluding the top and bottom) with a minimum of (10) layer of shrink-wrap to enforce the sturdiness of load and security measurements to prevent loads from being restacked prior to being issued to survivors or placed in the warehouse. The finished product will be placed onto 48 inches long by 40 inches wide 4-way wooden pallet. The over-all height of the pallet (with the products) will not exceed 55 inches.

The contractor shall label each wrapped pallet in accordance with the following :

• A statement identifying the commodity.

• The name and place of business of the manufacturer, packer, or distributor.

• Net quantity of contents in terms of weight, measure, or numerical count (measurement must be in both metric and inch/pound units).

• Manufacture date and expiration date

d. Transportation Requirements

In-Transit Visibility (ITV) and Tracking:

The Vendor shall use Electronic Data Interchange (EDI) when conducting business with the FEMA. If EDI is unavailable during emergency situations, the Vendor shall log into LSCMS (e.g., Vendor Portal/Partner Gateway) directly and enter the ASN information.

Vendor must receive consent from the COR to temporarily use the alternative to EDI.

COR will contact vendor concerning training and access to the Vendor Portal/Partner Gateway. The Vendor shall provide the below-listed mandatory data to be transmitted immediately upon dispatching a vehicle to a designated FEMA location. The tracking information shall be submitted to FEMA upon commodity departure from contractor origin and prior to arrival at FEMA destination.

• FEMA LSCMS Distribution Order (DO)

• Partner Reference Number

• Trailer Number or Container Number

• Trailer License Plate State

• Trailer License Plate Number

• Carrier Standard Carrier Alpha Code (SCAC)

• Origin Facility Identification sent on DO

• Destination Facility Identification sent on DO

• Estimated Delivery Date Time

• LSCMS Item Name (sent on DO)

• Shipped Quantity (Units)

• For items that contain a lot the Lot Number is required (such as rice)

• For items that contain an expiration date the Expiration Date is required (such as rice)

• For items that contain a manufacturer, the manufacturer is required (such as rice)

4.0 Delivery Location

Vendor shall deliver rice FOB destination to the delivery location identified below. The vendor must have the rice palletized and must use the required transportation vehicle as deemed necessary for transfer of products (20’- 53’ containers, lifted box trucks, pallet jacks, if needed).

It is the responsibility of the vendor to attest that truck drivers have a proof of either completed COVID-19 vaccination status or negative COVID test result within the last 72 hours. The truck driver shall comply with CDC and/or local guidelines on Personal Protective Equipment (PPE) usage while on federal facilities.
