STATEMENT OF WORK (SOW)

A. GENERAL INFORMATION

1. Title of Project: Service Contract for the BioTek Synergy H1 Hybrid Multi-Mode Reader currently in use by the Biomarkers and Genetics Core in Waco in support of multiple ongoing research studies conducted on-site.

2. Scope of Work: The VISN 17 Center of Excellence, Biomarkers and Genetics Core, located at 4800 Memorial Drive, Building 93, room 2A113, Waco, TX 76711 has requested a service contract for the existing Microplate Reader. This contract needs to include preventative maintenance, labor & parts discounts, and unlimited access to remote diagnostic and telephone support as well as field service as needed.

3. Background: The BioTek Synergy H1 Hybrid Multi-Mode Reader (S/N 1710302E) is a critical piece of equipment used by researchers at the CoE and must be fully operational and up to quality control standards to support multiple funded research studies at this location.

4. SERVICE COVERAGE DESCRIPTIONS: Below are requirements for service and maintenance of the BioTek Synergy H1 Hybrid Multi-Mode Reader:

· Provide support for all components of our current instrument configuration including:

· Monochromator Fluorescence

· Monochromator Absorbance

· Full-Light Luminescence

· Time Resolved Fluorescence

· Temperature Control to 45°C

· Gas Controller

REPORTING PROCEDURES: The contractor will provide at least 24 hours’ notice prior to performing servicing of the instrument. For service calls, repair/service technicians must report to the Center of Excellence and sign-in before proceeding to work location. A copy of the service report will either be directly given or emailed to the research point of contact within two weeks after service.

5. Performance Period:

Base year: 11/03/2021 to 11/02/2022 Option year 1: 11/03/2022 to 11/02/2023 Option year 2: 11/03/2023 to 11/02/2024 Option year 3: 11/03/2024 to 11/02/2025 Option year 4: 11/03/2025 to 11/02/2026

B. TRAVEL: Travel and per diem will not be reimbursed, in addition to contract.
