STATEMENT OF WORK

“Maintain RO/DI Water Treatment System”

1. Introduction:

a. The Minneapolis VA Health Care System (MVAHCS) requires a continuous supply of Reverse Osmosis and Deionized water supply for the Sterilizing Processing (SPS) and Laboratory (LAB) departments.

2. Objectives:

a. Establish a base plus four (4) year Service and Maintenance contract for the RO/DI water treatment systems located at Minneapolis VA.

3. Statement of Work:

a. One on-site assessment is required to evaluate the work to be performed and provide the most accurate price quote. Assessment must be completed prior to the closing date of the solicitation.

b. Contractor will remove and install 8 Carbon tanks and 8 Mixed Bed tanks on the first business day of the base year contract unless contractor is the incumbent.

c. Contractor is responsible for all materials, tools, equipment, and labor to service the water treatment system in the following areas:

· Bldg 70 Room BA101 – Lab/SPS (RO/DI)

· Bldg 70 Room 3P127 – Research (DI)

· Bldg 70 Room 4Q127 – Research (DI)

· Bldg 49 Room 228A – Research (DI)

d. Contractor shall perform services outlined in the table below:

e. Replace carbon and/or DI tanks ahead of schedule if they become depleted due to increased water usage.

f. As part of the monthly system inspections, inspect/replace filters, O-rings, UV lamps, and quartz sleeves on a periodic basis or when items fail under normal use.

g. Provide loop disinfection on a semi-annual basis for system located in BA101 that serves SPS and Lab. (June and December).

h. Critical water testing: Samples will be drawn on the first workday of each month and provide the results to the COR & SPS Supervisor no later than the 15th of each month. Water samples will be drawn from the critical water loop system that is located in BA101 and points of use in SPS. Monthly water samples will be drawn from the following locations:

· BA101- Existing sampling valve.

· BM 107- DI Water - Sink 1, 2, 3 and 4.

· BM 107C – DI Water - Sink 1, 2 and 3.

i. Provide emergency services after normal hours listed in Section 4, on weekends, and on holidays. Respond within four (4) hours of notification.

4. Invoicing:

a. Invoices will be submitted monthly. Invoice will be itemized and list the services performed and/or the materials that were used for that month.

5. Working hours:

a. Anticipated hours of work will be Monday through Friday from 7:00am – 5:00 pm. Additional hours must be coordinated through the COR. Work will not be performed on weekends or federal holidays unless there is an emergency.

6. Deliverables:

a. Service Reports due after the performance of each service listed in the chart in Section 3.b.

b. Monthly Critical Water Test Results: Results due on the 15th of each month.

7. References:

ANSI/AAMI ST108 (2023 or most current edition)

8. Safety:

a. Contractors are responsible for maintaining a safe working environment for their employees as well as the MVAHCS Staff. Work areas will be kept clean throughout the day, and all trash and demolished materials are to be properly disposed of daily.

b. Additional PPE will be required when entering the SPS Department. This PPE will be provided by the MVAHCS.

9. Security:

a. Contract personnel need to obtain a vendor’s badge form VA Security located at the outpatient entrance security desk. A valid government issued form of identification will be required to receive a badge.

b. Contractor’s Technicians are required to check in with the COR upon arrival at facility to obtain access and any additional instructions, task explanations, and project location information. Daily performance log sheets are required to be submitted to the CO and COR daily. The logs will provide information about location and progress that has been made.
