SCOPE OF WORK
Laboratory Countertop Replacement
Project # OK8CAS05H6
August 20, 2021
I. Objective:
The objective of this project is to replace damaged laminated countertops in the laboratory department at the US PHS Indian Hospital located in Claremore, Oklahoma. The hospital is a
110,000+ ft2 hospital building that consists of outpatient services such as OB/GYN, Behavioral
Health, Pediatrics and Adult Medical Clinics, Contract Health, Dental, WIC, Radiology, Pharmacy, Diabetes Education, and Public Health, as well as in-patient services that includes ER, OB, in-patient, OR, and ICU.
II. Background:
The countertops in the laboratory department are laminate on wood-based particleboard on plywood base 1-1/8” thick and were installed during the late 1990’s Laboratory Department
Renovation. Countertops are installed on lower cabinetry used as work surfaces, pony walls, windowsills, and office workstations. Countertops for standing work surfaces are at standard height of 36” and countertops used as sitting office work surfaces are at the standard height of 30”. As-built prints from this renovation are available for reference. The countertops are damaged, worn, require constant repairs, are infection control risks with exposed wood surfaces, and pose a general safety risk with sharp edges and exposed wood for patients, staff, and visitors. The west counter in Staff Office 387 is sagging and does not have enough clearance underneath for staff to work adequately when sitting. Cracks have also developed and laminate has separated from the side trim due to old glue or impact.
Countertops in the Lab Department shall be replaced with infection control-friendly materials typically used in laboratory environments that are durable, easy to clean and maintain, and are scratch/chemical resistant.
Lab Department rooms to have countertops replaced are as follows:
Lab Reception/Check-in Room 370
Phlebotomy Room 372
Urinalysis Room 373
Hematology Room 374
Clean Up Area/Workstation Area 377
Processing Area 378
Hallway 379
Chemistry 380
Microbiology Room 381
Chemistry Work Area 382
Blood Bank 385
Lab Staff Office 387
Lab Supervisor’s Office 389
Lab Staff Breakroom 390
Laboratory cabinetry with countertops are identified on Attachment A. Lab Countertop Locations w/Dimensions.

2. Contractor shall replace countertops with similar dimensions, profiles, and thicknesses.
Countertop material used shall conform to use in laboratory environments that are non-porous, easy to clean, long lasting, resists scratching, chemical resistant, temperature resistant, etc. such as phenolic resin, stainless steel, epoxy resin, and solid surface.
3. Contractor shall provide all labor, materials, supplies, tools and equipment, transportation, supervision and all other necessary items for successful completion of the countertop replacement and of related supporting work.

7. As-built prints for architectural building construction are available for review.
8. Signing in/out and scheduling of contractors and sub-contractors shall be coordinated with the Facility Manager or his designee in his absence in order to maintain efficient use of traffic-flow, parking, and access to building and locked areas.
9. Contractor shall use zero or low VOC sealants, adhesives, and other chemicals when available while applied inside the facility. For use of other items that emit VOCs, the
Contractor shall coordinate the work with the Lab Department and Facility Manager to minimize or avoid impact to hospital occupants.
10. The Contractor shall, at all times on the hospital campus including storage areas and work sites, keep the work area and public access areas free from accumulations of waste materials. The Contractor shall remove from the work and premises any rubbish, tools, equipment, and materials that are not the property of the Government. Upon completing the work, the Contractor shall leave the work area in a clean, neat, and orderly condition satisfactory according to IHS Facilities Management.

Contractor shall abide by all following codes:
a. ANSI
b. ANZI

EPA regulations. Recyclable materials shall be recycled where possible.

B. Countertop Replacement:
1. Countertops located on lower cabinetry workstations and reception area (including vertical face) shall be replaced with anti-bacterial solid-surface material (ex: copper-infused) in rooms 370, 372, and 390. The vertical reception area counter face may be replaced with a separate material approved during the submittal process by the Facility Manager.
Countertop material shall be selected from standard color patterns during the submittal process and countertop profiles shall be coordinated with the Facility Manager. There is approximately 161 square feet of existing horizontal countertop (not including the 6” vertical laminated counter extension at the outside of the Receptionist/Check-in area on the waiting room side) and approximately 80 linear feet of 4”H backsplash on top of the existing countertops in these areas.
2. Countertops located on lower cabinetry, pony walls (3), and window sills (2) with bottom 2”
H strip shall be replaced with phenolic (light gray preferred) material in rooms 373, 374, 377, 378, 379, 380, 381, 382, and 385. The window sill ledge may be replaced with solid surface in #3 below. Use of stainless steel where applicable is acceptable by Facility
Manager’s approval. There is approximately 454 square feet of existing horizontal countertop and approximately 243 linear feet of 4”H backsplash on top of the existing countertops in these areas.
3. Countertops located in Lab Staff Offices 387 and 389 shall be replaced with solid-surface material, standard color patterns. There is approximately 61 square feet of existing horizontal countertop and approximately 39 linear feet of 4”H backsplash on top of the existing countertops in these areas.
4. All replacement countertops and original lower cabinetry in Lab Staff office 387 shall be raised 1” by installing taller bases (2 total, each base is 14” wide x 21-1/2” deep) or spacer between the cabinet and base, and moving all wall brackets up to accommodate the raised height. The existing counter top surface is 29” and is too low with the 4” drawers underneath to adequately accommodate staff. Replace cove base as needed on 3 sides of each base to accommodate for the rise in base height. Contractor shall also install additional wall bracing where needed to properly support the long countertop span on the west wall that has no lower cabinetry to support the weight. The existing long span is sagging.
C. Countertop Removal and Installation:
1. Contractor shall verify all measurements and existing conditions. Countertop profile edges to be coordinated with the Facility Manager.
2. Contractor shall coordinate countertop replacement with the Lab Department and Facility
Manager (affects relocation of lab equipment, and office equipment in a 24/7 operated
Lab).
3. Install counter tops according to manufacturer recommendations and standard industry practices. Only use recommended/approved hardware by countertop manufacturer.
4. Ensure a tight fit with countertop, backsplash and the back wall using methods prescribed by product manufacturer. Install one-piece backsplash with countertops where possible.
5. Contractor shall minimize seams as much as possible and apply an approved low or zero
VOC sealant to all seams (stainless steel seams to be welded as well) and around countertops and backsplashes as applicable to prevent liquids spilling into crevices and gaps.
6. Contractor is responsible for disconnecting all plumbing, removal of sinks (6) and eyewash stations (4), reinstallation of sinks and eyewash stations, and reconnecting all plumbing in all areas where plumbing is affected during countertop replacement. Plumbing includes equipment drain lines where applicable.
7. Contractor shall remove, reinstall, or repair other building components and cabinetry altered to accomplish this work, including windows at the Lab Receptionist/Check-in counter, cabinetry, hardware, wall brackets, etc.
8. Contractor shall ensure new countertops are adequately supported underneath and shall be properly attached to the cabinetry or wall supports.
