About this file
This document is a Statement of Work (SOW) for the Corporal Michael J. Crescenz VA Medical Center (CMCVAMC) to join the HealthShare Exchange of Southeastern Pennsylvania (HSX). The purpose is to enable CMCVAMC leadership to make informed decisions regarding Veteran healthcare by decreasing community care costs and improving continuity of care through health information exchange. The SOW requires CMCVAMC to provide HSX with a patient panel list, keep the list updated, and receive encounter notifications from HSX based on admission, discharge, and transfer data. HSX will match the patient panel list against its participating facilities and deliver the notifications to CMCVAMC through standard methods. HSX will also provide member services support for implementation and post go-live.
The related federal contract opportunity is a sole source notice for CMCVAMC to contract with HSX for access to their network of electronic patient health information. The solicitation number is 36C24424Q0749, and the contracting office is the Department of Veterans Affairs Network Contracting Office 4. Interested parties may submit capability information by June 13, 2024 to establish their ability to meet the requirement, though this is not a request for competitive quotations.
