Statement of Work

For Grenada Lake / Guard Rail Removal/Installation

Grenad Lake Field Office

Scope of Work. The contractor shall provide all labor, tools, equipment, transportation, parts, materials, and other incidentals necessary to complete the removal/installation of approximately 29,683 linear feet of guard rail at USACE Grenada Dam. This work consists of furnishing and erecting complete sections of guardrail in accordance with current MDOT specifications and in reasonably close conformity with the lines and grades shown on the plans or established by the Engineer. Steel Post shall conform to the requirements of

ASTM A36 and metal rail shall conform to the requirements of AASHTO M180. The contractor shall dispose of all old guard rail and excess waste. All of the work will be performed in the public right‐of‐way.

PART 4- SECURITY

PART 1 - GENERAL

1.1 Work Included

The work specified in this section consists of furnishing of all supplies, equipment and personnel necessary to accomplish the removal/installation of approximately 29,683 linear feet of guard rail at USACE Grenada Dam.

(a) Contractor shall provide a per foot bid based on the information contained below. (Example: $1 per foot X

3500 ft. = $3,500 Total)

(b) Total length consists of approximately 29,683 feet of GUARD RAIL AND POSTS.

1.2 Quality Assurance

The requirements set forth in this document specify a wide range of procedural precautions necessary to insure that the very basic, essential aspects of a proper guard rail installation are adequately controlled. Strict adherence shall be required under specifically covered conditions outlined in this specification. Adherence to the specifications contained herein, or the Resource Manager’s approval of any aspect of any operation covered by this specification, shall in no way relieve the Contractor of their ultimate responsibility for the satisfactory completion of the work authorized under the Contract.

1.3 Submittals

A. WORK PLAN: Prior to beginning work, the Contractor must submit to the Resource Manager a general work plan outlining the procedure and schedule to be used to execute the project. Plan should document the thoughtful planning required to successfully complete the project.

B. EQUIPMENT: In the bid process the Contractor will submit specifications for equipment to be used to ensure that the equipment will be adequate to complete the project. Spares inventory or procurement method for parts shall be included.

C. PERSONNEL: Prior to the beginning of work, documentation of training and relevant experience of personnel shall be submitted to the Resource Manager.

D. Contractor shall submit a weekly report containing the number of linear feet completed each week.

PART 2 - EQUIPMENT REQUIREMENTS

2.1 General

The equipment shall be able to sufficiently perform the removal/install of guard rails and trained and competent personnel to operate the system must be provided. All equipment shall be in good, safe operating condition with sufficient supplies, materials and spare parts on hand to maintain the system in good working order for the duration of this project.

PART 3 – OPERATIONS

3.1 General

The Resource Manager must be notified 48 hours in advance of starting work. The Resource Manager’s approval for beginning the installation shall in no way relieve the Contractor of the ultimate responsibility for the satisfactory completion of the work as authorized under the Contract. It shall be the responsibility of

Resource Manager to provide inspection personnel at such times as appropriate.

3.2 Personnel Requirements

All personnel shall be fully trained in their respective duties as part of the contractor crew and in safety.

Training shall be provided specific to the project if any potential hazards may be encountered which has not already been included in personnel’s training.

3.3 Removal/Install Procedure

A. Site Preparation: Work site as indicated by site markings shall be graded or filled to provide a level working area. No alterations beyond what is required for operations are to be made. Contractor shall confine all activities to designated work areas.

B. Utility Locations: Contactor shall notify USACE Grenad Lake Facility Manager to obtain utility locations and site surveys. Once the utilities have been located the Contractor shall physically identify the exact location of the utilities by vacuum or hand excavation, when possible, in order to determine the actual location and path of any underground utilities. Contractor shall not commence operations until the location of all underground utilities within the work area have been verified and isolated.

C. Safety: Contractor shall adhere to USACE Safety and Health Requirements (EM 385-1-1), as well as all applicable municipal, state, and federal safety regulations. All operations shall be conducted in a safe manner.

Safety meetings shall be conducted at least weekly with a written record of attendance and topic submitted to

Resource Manager.

D. Traffic Control: Traffic control plan shall be used for the work adjacent to the traveled way. Traffic control plan shall be submitted to the Resource Manager prior to starting any work. The plan shall be prepared by qualified personnel.Traffic control shall be provided by the Contractor which shall include the personnel and signage packages for any flagging operations required.

3.4 Site Restoration

Following operations, Contractor will de-mobilize equipment and restore the work-site to original condition.

PART 4 - SECURITY

4.1 SECURITY REQUIREMENTS FOR UNCLASSIFIED CONTRACTS

Access and General Protection/Security Policy and Procedures. All contractor and all associated sub-contractors employees shall comply with applicable installation, facility and area commander installation/facility access and local security policies and procedures (provided by government representative). The contractor shall also provide all information required for background checks to meet installation/facility access requirements to be accomplished by installation Provost Marshal Office, Director of Emergency Services or Security Office.

Contractor workforce must comply with all personal identity verification requirements (FAR clause 52.204-9, Personal Identity Verification of Contractor Personnel) as directed by DOD, HQDA and/or local policy. In addition to the changes otherwise authorized by the changes clause of this contract, should the Force Protection

Condition (FPCON) at any installation or facility change, the Government may require changes in contractor security matters or processes.

Suspcious Activity Reporting Training (e.g. iWatch, CorpsWatch, or See Something Say Something). The contractor and all associated sub-contractors shall receive a brief/training (provided by the RA) on the local suspicious activity reporting program. This locally developed training will be used to inform employees of the types of behavior to watch for and instruct employees to report suspicious activity to the project manager, security representative or law enforcement entity. This training shall be completed within 30 calendar days of contract award and within 30 calendar days of new employees commencing performance with the results reported to the COR NLT 5 calendar days after the completion of the training.

For Contracts that Require OPSEC Training. All new contractor employees will complete Level I OPSEC

Training within 30 calendar days of their reporting for duty. Additionally, all contractor employees must complete annual OPSEC awareness training. The contractor shall submit certificates of completion for each affected contractor and subcontractor employee, to the COR or to the contracting officer (if a COR is not assigned), within 5 calendar days after completion of training. OPSEC awareness training is available at the following websites: https://www.iad.gov/ioss/ or http://www.cdse.edu/catalog/operations-security.html; or it can be provided by the RA OPSEC Officer in presentation form which will be documented via memorandum.

Escort requirements. All contract employees, including subcontractor employees who are not in possession of the appropriate security clearance or access privileges, will be escorted in areas where they may be exposed to classified and/or sensitive materials and/or sensitive or restricted areas.

Pre-screen candidates using E-Verify Program. The Contractor must pre-screen Candidates using the E-verify

Program (http://www.uscis.gov/e-verify) website to meet the established employment eligibility requirements.

The Vendor must ensure that the Candidate has two valid forms of Government issued identification prior to enrollment to ensure the correct information is entered into the E-verify system. An initial list of verified/eligible Candidates must be provided to the COR no later than 3 business days after the initial contract award.

NOTE: When contracts are with individuals, the individuals will be required to complete a (Form I-9), Employment Eligibility Verification, with the designated Government representative. This Form will be provided to the Contracting Officer and shall become part of the official contract file.
