Statement of Work
Cataract and Vitreoretinal Virtual Reality Surgical Simulator for Naval Medical Center San Diego
SECTION 1 – BACKGROUND
Ophthalmology and Refractive Surgery Department in Naval Medical Center San Diego (NMCSD) offers ophthalmic residency training programs that requires Ophthalmology residents to perform cataract and vitreoretinal surgeries using virtual reality (VR) surgical simulator provided by Eyesi prior to performing any intraocular surgeries on actual patients. Currently, the Eyesi VR surgical simulator onsite non-functional and has reached end-of-life (EOL). Eyesi is crucial to ensure residents get necessary hands-on training/experience as well as supplements medical readiness/skills sustainment of staff surgeons. The only other way residents can get hands on experience and develop proficiency would be operating on actual patients, which is not ideal in terms of patient safety.
Eyesi is utilized by Graduate Medical Education (GME) and Ophthalmology. The Eyesi provides ophthalmologists with a virtual environment to hone their skills in delicate eye surgeries such as cataract removal and retinal procedures. Surgeons can practice intricate maneuvers and develop precision without additional risk to patients, ensuring optimal patient safety during actual procedures. The Eyesi allows for the opportunity to practice procedures on demand and allows for continual remediation until proficiency is achieved. On average, NMRTC SD has 9 residents per academic year completing a 3+ year training program. Each resident completes approximately 75 cataract surgeries per year as primary surgeons and 30 per year as assistants to staff surgeons. Total number of cataract surgeries annually per trainee is a little more than 100, totaling 900 cumulatively.
The Eyesi is the most prevalent of the commercially available VR surgical simulators in U.S. ophthalmic residency training programs. The Eyesi simulator is the only system that has the curriculum that allows residents to become proficient prior to operating on an on an actual patient and is part of the GME Surgical Foundations Curriculum, which residents complete prior to any intraocular surgical experience. It is the only one that has been validated in peer-reviewed publications and available on the market for the training of both cataract and vitreoretinal surgeries.
In addition to the built-in curriculum that a trainee must work through in a step-wise fashion, the Eyesi simulator is also the only simulator available that comes with handheld instruments that mimic the instruments surgeons use inside the eye. Residents are required to practice and pass a specific syllabus on the simulator itself before beginning any intraocular surgeries. The Eyesi also supports achieving competency throughout the last two years of intraocular surgical training with simulation of more advanced maneuvers. Additionally, the Eyesi is an excellent tool used in recruiting residents to the training program as they are very interested in gaining surgical experience early in their training.
