About this file

This is a Contractor Information Sheet template used by U.S. Customs and Border Protection (CBP) for managing short-term contractor and visitor access to CBP facilities. The form serves as a security vetting document required by the Office of Professional Responsibility and must be submitted 72 hours prior to any visitor arrival. It collects comprehensive personal information for all visitors, including full names, dates of birth, citizenship status, place of birth, occupation, driver's license details, passport information, visa documentation, and social security numbers. The template accommodates up to 84 visitor entries and requires designation of a CBP employee escort, with specific escort ratios mandated (one escort per four visitors in standard cases, or one-to-one depending on vetting status).

The submission process requires emailing completed forms to SMDSECURITYSERVICESBRANCH@cbp.dhs.gov or faxing to the Office of Professional Responsibility Security Management Division at 202-344-1860. All visitors must present valid U.S. Government-issued photo identification and check in/out at the Security Desk, with designated escorts maintaining constant supervision throughout facility access. Failure to provide required information may result in denial of access. This template is a security control mechanism rather than a procurement document and does not involve pricing, contract awards, or commercial product/service requirements.
