About this file

This file is a CBP Contractor Information Sheet template used for short-term visitor access to U.S. Customs and Border Protection facilities. The form serves as a security vetting document that must be submitted 72 hours prior to any visitor arrival to the Office of Professional Responsibility via email to SMDSECURITYSERVICESBRANCH@cbp.dhs.gov or fax to 202-344-1860. The template collects contractor company information, CBP location details, points of contact for both the contracting company and on-site personnel, and comprehensive visitor information including names, dates of birth, citizenship status, place of birth, occupation, address, and identification documents (visa, passport, driver's license, and Social Security number).

The form accommodates up to 80 visitors and establishes mandatory security protocols requiring all visitors to present U.S. Government-issued photo identification, check in and out at the Security Desk, and maintain constant escort by a designated DHS/CBP employee. Escort requirements are established at a ratio of one escort per four visitors (1-4 visitors) or one-to-one escort depending on vetting status. The form includes a Privacy Act Notice citing Public Law 93-579, indicating that failure to provide required information may result in access denial. Any unescorted visitors found in CBP space will be immediately reported to security and removed from the facility.
