About this file

This is a Short Term Contractors Template and Visitor Management Information Sheet for U.S. Customs and Border Protection (CBP). The form is designed to document and track contractor and visitor access to CBP facilities by requiring submission of comprehensive information including contractor company details, on-site personnel points of contact, visitor identification data, and visit logistics. The document serves as a security clearance tool that captures personal information for up to 84 visitors, including names, dates of birth, citizenship status, passport and visa information, driver's license details, and social security numbers.

Key submission requirements include mandatory notification to the Office of Professional Responsibility Security Management Division at least 72 hours prior to visitor arrival, with submission directed to SMDSECURITYSERVICESBRANCH@cbp.dhs.gov or fax to 202-344-1860. All visitors must present a U.S. Government-issued photo identification and check in and out at the security desk upon arrival and departure. Visitor escorts, who are required CBP employees, must maintain constant supervision of visitors at all times, with a minimum ratio of one escort per four visitors, though certain vetting statuses may require one-to-one escort ratios. Any unescorted visitors found in CBP space will be immediately removed. The form is submitted under the Privacy Act of 1974 (PL 93-579) and failure to provide required information may result in access denial.
