About this file

This is a Contractor Information Sheet template used by U.S. Customs and Border Protection (CBP) for visitor access control and security vetting purposes. The form serves as an exhibit to a Statement of Work and is not directly related to the procurement of support canines referenced in the associated federal contract opportunity.

The template is designed for short-term contractors and visitors requiring access to CBP facilities. It requires submission 72 hours prior to visitor arrival and must be submitted to the Office of Professional Responsibility via email (SMDSECURITYSERVICESBRANCH@cbp.dhs.gov) or fax (202-344-1860). The form collects comprehensive personal identification information including full name, date of birth, U.S. citizenship status, passport and visa details, social security number, and driver's license information for up to 80 individuals. Key operational requirements include mandatory presentation of U.S. Government-issued photo identification upon arrival, visitor check-in and check-out at the Security Desk, and constant escort by CBP employees at a ratio of one escort per four visitors (or one-to-one depending on vetting status). Any unescorted visitors found in CBP space will be immediately removed. The submission is governed under the Privacy Act of 1974 and serves as a prerequisite security clearance mechanism for limited authorized facility access.
