About this file
This file is a Short Term Contractors Template and CBP Contractor Information Sheet used by U.S. Customs and Border Protection (CBP) to manage visitor access and security clearance for contractors visiting CBP facilities.
The form collects comprehensive contractor and visitor information including company details, points of contact, visit dates and times, and detailed personal information for each visitor (names, dates of birth, citizenship status, passport/visa data, and identification numbers). A mandatory submission requirement of 72 hours prior to visitor arrival must be sent to the Security Management Division via email to SMDSECURITYSERVICESBRANCH@cbp.dhs.gov or fax to 202-344-1860. All visitors must present U.S. Government-issued photo identification and check in/out at the security desk. The form establishes escort requirements, mandating that one escort accompany every four visitors or fewer, with potential one-to-one escort requirements based on vetting status. Visitors found unescorted in CBP space will be immediately removed. The form is submitted under the Privacy Act of 1974 for security vetting purposes, with failure to provide required information potentially resulting in access denial.
