About this file
This is a Contractor Information Sheet template used by CBP's Office of Professional Responsibility for processing visitor access to CBP facilities. The template requires detailed personal information for contractors including full name, date of birth, citizenship status, place of birth, occupation, address, and identification details (SSN, driver's license, passport, or visa information). The form must be submitted 72 hours prior to arrival via email or fax to the Security Management Division.
The template includes specific security protocols requiring all visitors to present U.S. Government-issued photo ID, check in/out at security desks, and maintain proper escort ratios (1:4 or 1:1 depending on vetting status). The form falls under the Privacy Act of 1974 and is used specifically for minimal security checks to grant limited facility access. This is purely a administrative template and does not contain any specific requirements for products or services.
