Scope of Work:
1. Background The Great Lakes District REPFAC (Representation Facility) is a 3-story colonial with a two-car garage attached built in 1950. Recent observations by residents have identified sagging in the floors on both the first and second levels, as well as cracking in the drywall on some walls and ceilings. These issues raise concerns about the structural integrity of the building, and an evaluation is required to determine the cause of these issues and recommend corrective actions, if necessary.
2. Objective The objective of this SOW is to engage a licensed structural engineer to conduct a comprehensive assessment of the REPFAC to determine the structural condition of the building, identify the root causes of the observed issues, and provide recommendations for repair or remediation if needed. All inspections shall be non-destructive in nature.
3. Scope of Services The contractor shall provide the following services:
3.1 Initial Site Visit and Inspection
Conduct an on-site inspection of the REPFAC, including all accessible areas of the structure (e.g., foundation, framing, floors, walls, ceilings, attic, and crawl spaces).
Document the observed sagging in the floors and cracking in the drywall, including measurements, photographs, and notes.
Identify any visible signs of structural distress, material degradation, or other contributing factors.
3.2 Structural Analysis
Perform a structural analysis of the building with all readily available information, including load-bearing elements, to assess whether the observed issues pose a risk to the building’s safety or stability.
Evaluate the adequacy of the existing structural components, including the foundation, framing, and load paths.
3.3 Recommendations
Provide a detailed report summarizing the findings of the inspection and analysis.
Identify the root causes of the observed issues (e.g., settlement, material fatigue, moisture intrusion, etc.).
Recommend corrective actions or repairs, including prioritization of repairs if multiple issues are identified.
If applicable, provide preliminary cost estimates for recommended repairs.
4. Deliverables The contractor shall provide the following deliverables:
A comprehensive inspection report, including:
o Description of observed conditions.
o Analysis of structural integrity.
o Root cause analysis of identified issues.
o Recommendations for corrective actions and repairs.
Photographs and diagrams documenting findings.
5. Schedule
The contractor shall complete the on-site inspection(s) within 1 month of contract award.
The final report shall be submitted within 2 weeks of the on-site inspection(s).
6. Qualifications The contractor must meet the following qualifications:
Be a licensed professional structural engineer (P.E.) in the state of Ohio.
Have a minimum of 3 years of experience in residential structural assessments and repairs.
Provide references for similar projects upon request.
7. Government-Furnished Information (GFI) The government will provide the contractor with the following:
