STATEMENT OF WORK
SNOW REMOVAL SERVICES
This Statement of Work sets forth the general terms for Snow Removal Services to be provided at the Little Shell Service Unit, Great Falls, Montana.
Background:
The Little Shell Tribe was federally recognized in 2021 and subsequently entered in to a lease on a healthcare clinic to provide outpatient healthcare services to the population it serves. The clinic provides routine outpatient care to tribal members.
Scope of Work:
Contractor shall provide snow removal services described herein at:
Little Shell Tribal Health Clinic, 425 Smelter Ave, Great Falls, MT 59404
Period of Performance:
December 1, 2022 to June 30, 2023
Specific Tasks:
A. Common Terms:
i. CO – Contracting Officer
ii. COR – Contracting Officer’s Representative
iii. IHS – Indian Health Service
B. Areas to be Serviced:
All service areas below are assumed to be inclusive of, but not limited to the areas listed
i. Pre-Storm Preparation: Sidewalks, walkways, parking lots
ii. Post-storm Snow Removal: Sidewalks, walkways, parking lots
iii. De-icing:
C. Work Schedule
i. All work to be completed before business operations which start at 8:00 (Monday through Friday); when weather conditions allow.
a) NOTE: If work cannot be completed prior to the facility opening, the contractor will inform the IHS no later than 7am the same day and provide an updated anticipated completion time.
b) NOTE: The CO, COR, & IHS reserves the right to limit, or otherwise reschedule access to the site to allow for meetings and/or other previously unscheduled events. Forty-eight (48) hours’ notice will be given to the contractor for all unscheduled events.
D. Job Specifications:
i. Sidewalks, walkways:
a) Clear snow from entire width of sidewalk/walkway
b) Apply product to prevent accumulation of ice
ii. Driveways, Parking Lots
a) Clear snow from all parking spaces
b) Apply product to prevent accumulation of ice
iii. Pre-Storm Preparation Services:
a) Prior to a snow event, apply environmentally friendly de-ice product and/or materials to the sidewalks, walkways, driveways and parking lots in areas that may form ice in an effort to prevent ice from forming and bonding to the surfaces
iv. De-icing Services:
a) Prior to and after a weather event that may cause an accumulation of ice, environmentally friendly de-ice product and/or materials shall be applied in areas that may form ice in an effort to prevent ice from forming and bonding to the surfaces
E. Materials:
i. The contractor shall provide all labor, tools, materials and equipment necessary to complete work as outlined.
ii. All products and materials used shall be environmentally friendly and abide by all EPA and OSHA guidelines.
Damages:
Contractor agrees to hold IHS harmless for any damages caused by negligence of the contractor’s employees. Damages will only be repaired or reimbursed upon notification of damage(s) to contractors account manager and inspection by account manager or representative.
Notification must be given within 72 hours of said damage.
