Statement of Work: Pour Concrete Slab and Install Aluminum Canopy.
Project Objective: The objective of this Project is to excavate existing soil, furnish/install compacted Crushed Limestone, pour 14’ X15’ X 12” Concrete Slab @ 4000 PSI with welded wire fabric, and Furnish/Install Aluminum Canopy. Drawings are provided below. This project is in on the South Side of Bldg. #28 at USDA Cotton Ginning Laboratory, located at 111 Experiment Station Road in Stoneville, MS.
Site Visit:
Contractors are encouraged to visit the site and be escorted by a government representative to confirm locations, dimensions, and conditions of the project before a quote is submitted. The site visit shall be for the purpose of observing firsthand any conditions relevant to the completion of this project. No consideration will be given to claims based on lack of knowledge of existing conditions, except where Contract Documents make definite provisions for adjustment in cost or extension due to existing conditions not readily foreseeable.
Contractor must be registered in System for Award Management, (SAM), prior to submitting a quote.

Utilization of Bio-Based Products: The contractor shall utilize products and material based from bio- based materials as applicable, (e.g. bio-based greases; bio-based hydraulic fluids, bio-based absorbents), to the maximum extent possible without jeopardizing the intended end use of detracting from the overall quality delivered to the end user. All supplies and materials shall be of a type and quality that conform to the applicable Federal Specifications and Standards. All supplies and materials to be used in the performance of work described herein are subject to the approval of the Contracting Officer Representative (COR). Products can be found at:

Safety: The contractor will submit a Site Safety Specific Plan/Accident Prevention Plan in industry recognizable format to the Contracting Officer within 14 days of contract award. Contractor shall furnish a Site-Specific Safety Plan (SSSP) and the Government shall review the SSSP prior to the start any construction activities. SSSP shall be in typically accepted industry format and shall contain maps, addresses, phone numbers to the nearest local Dr. Office and Hospital.
Base: Contractor shall provide all materials, labor, and equipment in order to: Furnish and install concrete slab and aluminum canopy at the location specified by site personnel in a length of 14’ x 15’. Contractor will undercut soil 6” below natural ground, remove soil, and refill with 6” of Compacted Crushed Limestone with Class A Compaction 95% Standard Density, to serve as a base for the concrete slab, with the intent for the slab to be 6” above natural ground elevation. Contractor will provide concrete design mix for Minimum 4000 psi within 28 Days to USDA for approval.
Contractor will pour Concrete Slab 14’ x 15’ x 12”, 6”x 6” x 6 ga. welded wire fabric with metal chairs, placed in center of the slab, to meet ACI and ASTM Standards. New concrete slab shall be tied to the existing slab by means of 30” #6 rebar tie bars at mid slab spaced 24” on center and imbedded into existing slab a minimum of 12” and epoxied in place with Hilti HIT-HY 100 Adhesive Anchor. This project will require on-site testing of the concrete to include, air content, slump, and temperature. White curing compound shall be sprayed on all exposed concrete after pour to prevent moisture loss. This project will require four (4) concrete cylinders taken for compressive strength http://www.sam.gov/ https://www.biopreferred.gov/ from on-site concrete and broken two (2) by 7th day and two (2) on the 28th day. All testing shall be performed by a Certified Concrete Testing Lab and all Test Results shall be submitted to USDA.
Contractor will furnish and install a new white aluminum standard rolled canopy on the concrete pad structure with the dimensions of 14’W x 15L’ x 11’-6” H (minimum to underside). The canopy shall have 3’ vertical skirts on (3) sides of the canopy. Roof shall be 2:12 pitch. Canopy will have gutter facias with minimum 3” x 3” x .125” aluminum white downspout columns from the facias. Columns will be securely mounted to new concrete slab. The canopy will be free standing with minimum of (7) supporting steel columns to provide for a minimum of 120 mph wind loading rating. Contractor will re-locate the existing lightening protection cable to accommodate new canopy and shall comply will National Electric Code for splicing, final location, and grounding. Contractor Responsibility:
The project has a base to be performed per the following Specifications and Base:
1. The Contractor's approved laboratory shall collect samples of fresh concrete in accordance with ASTM
C172/C172M during each concrete pour as required to perform tests specified herein. Make test specimens in accordance with ASTM C31/C31.
2. The Contractor's approved laboratory shall perform concrete slump tests in accordance with ASTM C143/C143M. Take samples for slump determination from concrete during placement. Perform tests at the beginning of a concrete placement operation to ensure that specification requirements are met.
3. Make four test specimens for each set of tests. Test two specimens at [7] days, and the other two at [28] days. Concrete strength will be considered satisfactory when the minimum of the [28]-day test results equals or exceeds the specified [28]-day strength. Concrete which is determined to be defective shall be removed and replaced with acceptable concrete.
4. Hot Weather: Maintain required concrete temperature in accordance with Figure NRMCA NOMOGRAPH FOR ESTIMATING EVAPORATION RATE ON THE BASIS OF MENZEL FORMULA in ACI 305.1 to prevent evaporation rate from exceeding 0.2 pound of water per square foot of exposed concrete per hour. Cool ingredients before mixing or use other suitable means to control concrete temperature and prevent rapid drying of newly placed concrete. After placement, use fog spray, apply monomolecular film, or use other suitable means to reduce the evaporation rate. Start curing when surface of fresh concrete is sufficiently hard to permit curing without damage. Cool underlying material by sprinkling lightly with water before placing concrete. Follow practices found in ACI 305.1.
5. Contractor will verify all dimensions and site conditions prior to beginning work.
6. Contractor will perform the work as listed in the above outlined Base.
7. Contractor shall keep work area clean daily and take measures to control debris while work is being performed.
8. Contractor is required to manage the project and consult with the Government prior to beginning all work phases. And, to work in a manner that minimizes the impact of the work on Government operations, research, and functions.
9. All work must be performed in such a manner as to not adversely affect daily operations of the facility, its workers, and the onsite facility operations.
10. Contractor must always have a Superintendent on the job while working on this project.
11. Contractor will provide their own restroom facility.
12. Contractor will remove all demolished material and debris from the worksite and dispose off-site.
Material / Submittals:
The material for the entire work shall include but are not limited to those detailed above necessary to successfully complete the project and deliver a functioning system as required. A drawing of the proposed installation of concrete pad and canopy shall be submitted and approved prior to beginning work. Submittals on all material, including shop drawings and specifications, used on this project shall be submitted to the EPM and Contracting Specialist as noted in this document. Electronic submittals are acceptable otherwise three written submittals are required.
Installation Procedure:
Contractor will verify all dimensions and site conditions prior to beginning work. Contractor will schedule the work between working hours as listed below. Contractor is required to provide labor, specified material, equipment, and other tools required to perform the job fully and completely. No Government equipment will be available for use by the contractor. The contractor shall always be responsible for protecting his work and ensuring the safety of his crew and ARS employees. Upon completion of the project, the contractor shall remove from the premises all accumulated materials and rubbish caused by the project. Premises shall be left in a clean, orderly, and acceptable condition.
Damage to Property:
Any damage to the Government buildings, utilities, etc. caused as the result of the contractor’s activities shall be repaired or replaced to the satisfaction of the Government.
Inspection and Acceptance:
A government representative shall inspect and approve the work performed on this project. The Contractor shall schedule inspection a minimum of 10 days in advance, in writing, to the Contracting Officer.
Project Schedule:
The completion of this project shall not exceed 90 days from date of notice to proceed. A request for time extension must be submitted to the Contracting Specialist in writing. Each request must include specific and detailed reasons in support of the schedule alteration.
Work Hours:
Contractor work hours shall be Monday through Friday, seven (7) A.M. to four (4) P.M. with the exception of Government holidays. Any changes to work schedule shall be submitted to Contacting Officer for written approval prior to schedule change.
