Statement of Work

For

Boom Crane Inspection, Maintenance, and Repairs

1. Description of Services

1.1. This Statement of Work (SOW) is for a one time effort for a five (5) year boom crane inspection for two (2) each National Crane Boom Truck(s) 1100 Series for the 583rd MMXS, Minot Air Force Base (AFB). Inspection is required In Accordance with Air Force Manual (AFMAN) 91- 203 Air Force Occupational Safety, Fire and Health Standards and Crane Manufacturer’s recommendations. Contractor shall accomplish full boom inspection, replace cables, wear pads, load tested to manufacturers specifications at Contractors facility by a certified National Crain Inspector/Mechanic.

1.2.The Government will deliver crane to be repaired to Contractor within a 30 mile radius of Minot AFB, ND. Upon completion of inspection and repair the Contractor shall notify the 583rd MMXS, MAFB designated point of contact to make arrangements for pick up.

1.3.Any Contractor beyond that radius will be required to pick up the vehicle to be repaired at Minot AFB, ND and return the vehicle to Minot AFB, ND after repairs are made.

Contractor will have the option to drive or use Contractor provided drop deck trailer for pickup and delivery.

1.3.1. One (1) crane will be provided to the Contractor for services at a time, upon completion and returned to Minot AFB, ND second crane will be provided.

1.4.Contractor shall provide all management, parts, materials, tools, supplies, equipment, facility and labor necessary for inspection, cable replacement, wear pads and load test.

1.4.1. Contractor shall complete inspection and repair within 30 calendar days upon delivery to Contractor’s facility or Contractor pickup at MAFB.

2. Provided Service:

2.1. Contractor shall accomplish full boom disassembly and perform a thorough inspection of two (2) each 583rd MMXS (Missile Maintenance Squadron) cranes consisting of (2) each National Cranes 1100 Series (Registration #s 03D00252 and 08D01021). This will include:

2.1.1. Remove boom from crane and remove telescope cylinder, Telescope cables and inspect boom wear components

2.1.2. Replace all cable (Retainer and Assembly) and wear parts in boom IAW manufacturers specifications to include: Wear pads, bearings, plug, cable retainer, nuts, sheave, plugs, blocks, or any other items appropriate for wear and tear replacement.

2.1.3. Boom will be inspected, reassembled, lubed and load tested to manufacturers specifications at end of service.

2.1.4. Any additional items found that need attention, the Contractor shall notify the end user point of contact and 5th Contracting Squadron as soon as practicable.

Quote must be specifically laid out as why it’s needed and have OSHA, ANSI or Manufactures Specifications references.

2.1.5. No additional work shall be performed until permission is granted by the 5th Contracting Squadron. The Contractor shall provide an itemized list of costs to include labor rates, hours, and categories for work to be performed, as well as supporting documentation for the price of any materials (such as catalog prices, supplier invoices, or other information as required). The Contractor shall not proceed with any additional work until the final price is accepted by the Contracting Officer.

2.2. All service; parts and lubrications MUST meet all local, state, federal laws / regulations and applicable American Society of Mechanical Engineers / American National Standards Institute (ASME / ANSI) codes, OSHA standards and manufacturer’s requirements in a manner that shall ensure continuous and safe operation. Contractor shall provide proof of certification and qualification of inspectors and repair personnel according to American Society of Mechanical Engineers / American National Standards Institute (ASME / ANSI) codes, OSHA standards and manufacturer’s requirements within 5 business days if requested by the Contract Officer.

2.3.Contractor shall provide records of any preventative maintenance, inspection / testing, repair, load testing, certification conducted for each Boom Crane inspected to include corresponding Reg. Number.

2.3.1. Label/Sticker showing when inspection was completed and next inspection due date shall be placed somewhere accessible and be able to withstand ND weather.

3. Contractor shall provide a one year warranty for all parts and services provided.

4. Prior to acceptance, the vehicle(s) will be inspected and full function check will be performed by the Government.
