STATEMENT OF WORK

The contractor shall furnish all requisite services, labor, travel, transportation, materials, shipping, parts, connections, and equipment for the provision and on-site installation of a minimum of three vaporized hydrogen peroxide Low Temperature Sterilizers. These units are to be located at 4801 Linwood Blvd, Kansas City MO 64128. These low temperature sterilizers are essential for the Sterile Processing Service (SPS) in reprocessing reusable medical devices (RMDs) employed in invasive procedures. needed to support Sterile Processing Service (SPS) for reprocessing reuseable medical devices (RMD) used in invasive procedures. Furthermore, these units are critical for supporting medical procedures and surgeries and surgeries conducted within the Operating Room, as well as in various ancillary clinics throughout the KCVA.

The contractor shall additionally furnish training to clinical personnel prior to the utilization of the equipment. This training is to encompass all tours of duty for the sterile processing department and necessitate annual retraining on all associated on all associated equipment. Moreover, the contractor bears the responsibility for the removal and disposal of all debris and packaging material resulting from the installation of the VPRO MaX2 Low Temp Sterilizers.

2. SPECIFICATIONS

Minimum of three low temperature sterilizers shall have a minimum individual dimensions of Cabinet (DD): 33 x 40-15/16 x 72” (838 x 1040 x 1829 mm) with Recessed/Cabinet (SD): 33 x 38- 5/16 x 72” (838 x 973 x 1829 mm).

o Mounting single door, cabinet o System installation requires no plumbing, ventilation or air o supply – only a dedicated electrical connection is needed. A power cord is supplied for this connection o The facility space in which the vehicle can occupy is approx. 35’ x 110’.

Vaporized hydrogen peroxide sterilization processes are particularly suited for sterilizing heat-sensitive materials because temperatures within the load currently do not exceed 55ºC (131ºF) and sterilization occurs in a low-moisture environment. Heat tolerant devices and materials can also be indicated for vaporized hydrogen peroxide sterilization processes. FDA-cleared vaporized hydrogen peroxide sterilizers, either with or without plasma, are currently available in the United States.

Per ANSI/AAMI ST58:2013 or subsequent, the sterilizer shall be installed and operated in an area with a ventilation system capable of at least 10 air exchanges per hour.

VPRO MaX2 must support utility requirements:

o Current 16 AMP o Minimum Circuit Breaker 20 AMPS o Use NEMA L15-20R Plug o Provide grouped or ganged circuit protection and disconnect for phase 3 power as required by NEC and local codes.

o Individual power shutoffs near each machine for servicing.

Shall integrate with the CensiTrac Instrument Tracking System.

3. Requirements

The assigned Contracting Officer Representative (COR) is the sole point of contact for all administrative and construction requirements.

The contractor is responsible for conducting site visits, verifying existing conditions, determining materials quantities, and reviewing installation instructions and any discrepancies or conflicting requirements in the Statement of Work prior to submitting the Final Proposal.

The contractor shall provide and install three low temperature sterilizers at the VA Medical Center located at 4801 Linwood, Kansas City, MO 64128.

All repairs, preventative maintenance, and remedial maintenance provided under this contract must meet or exceed the Original Equipment Manufacturers (OEM) specification.

Only fully qualified technicians and engineers shall be employed by the Contractor for the performance of all work under this contract.

The contractor shall maintain the highest standards of professional capability and workmanship for the entire duration of this contract.

4. EQUIPMENT MAINTENANCE & RESPONSE TIMES

The vendor is required to furnish comprehensive full-service maintenance via service agreement for a minimum duration of one year following installation. This includes the provision of all necessary recurring and preventative maintenance activities.

1. Emergency response for equipment and MSPS failures. The vendor shall respond to the facility for any equipment failure resulting in a total or atrial loss of the installed equipment or any component of the mobile sterile processing system. This response must occur within four hours from the time of VAMC contract. Should a permanent resolution not be achievable within 24hours of the initial failure, a temporary solution shall be made operational within the same 24-hour period. Subsequently, an agreed permanent solution shall be planned, and necessary replacements ordered within five business days.

5. SAFETY

The contractor/vendor shall always, ensure that all work provided complies with all local, State, and Federal rules and regulations pertaining to workplace safety. The contractor shall take all proper safety and health precautions to protect the work, the workers, the public and the property (OSHA). Installs and construction work will be in accordance with all applicable codes to include the most current codes National Fire Protection Association (NFPA), and the VA Design Guide Manuals that are applicable to the work. If the design guide is equivalent or more stringent then the mentioned codes, the VA Design Guide takes precedence.

6. DOCUMENTATION

The contractor shall furnish to the KCVA Medical Center Biomedical Engineering department legible hard copies and electronic copies of service reports of the maintenance indicating details of the maintenance performed upon completion of all work.

8. PERFORMANCE MONITORING

The contractor’s performance will be monitored by the Contracting Officer (CO), Contracting Officer Representative (COR), Infection Control Risk Assessment Office, and Safety Office during the rental agreement period.

Coordinate all administrative and construction requirements exclusively with or through assigned Contracting Officer Representative (COR).

The Construction Safety Officer, Construction Safety Committee, and COR will review the operations and site construction conditions of the project to confirm that all Safety regulations relating to construction and healthcare facilities are being adhered to. Daily and weekly safety rounds will be conducted. For non-compliance safety related issues, a safety report will be sent to the Contractor to correct within 14 days. Non-compliance may result in job-site suspense until compliance is met at the contractor’s expense.
