List of Changes to SOL-R8-12-000003 via Amendment #000001
1. Paragraph (d) of “B.1 Fixed Rates for Services…..” is revised as follows:
(d) The rate or rates set forth in the Pricing Schedule cover all expenses, including report preparation, salaries, overhead, general and administrative expenses, and profit. The equipment rates shall apply to equipment whether it is contractor-owned or rented/leased and shall be inclusive of all costs (operation and maintenance, repair costs, depreciation and other acquisition costs and indirect costs, as applicable), except fuel. Fuel is considered an Other Direct Cost.
2. Section L, Submission of Cost or Pricing Proposal, (1) General. is revised to eliminate the reference to SF1411 as follows:
1. General. Submit cost or pricing data in accordance with FAR 15.408 and the following.
3. Section M evaluation criteria, Paragraph 5 Response to Scenarios for Scenario 2 is revised to mirror Section L, Scenario 2 as follows:
5. RESPONSE TO SCENARIOS
The offeror’s response to each scenario will be evaluated separately, as follows.
For Scenario 1, the offeror will be evaluated on the quality of a written work plan, based on the following criteria:
• Inclusion of all required elements, as described in the Scenario 1 Response Instructions in Section L
• Demonstrated understanding of the requirements specific to this scenario
• Feasibility of the project schedule and allocation of resources
• Quality of the overall project approach
The offerors will also be evaluated on its demonstrated understanding of the requirements and intent of the Site Health and Safety Plan.
For Scenario 2, the offeror will be evaluated on the quality of its response to each of the six questions provided in the Scenario 2 Response Instructions in Section L.
4. In the Statement of Work, the description for Regional Cross Over is included as part of Element 11. Element 12 Regional Cross Over of the Statement of Work is omitted. The revised Element 11 reads as follows:

1. Response Times
The contractor shall provide a management and personnel structure that will ensure that personnel are available on a 24 hour-a-day basis and that responses are conducted in accordance with the technical direction outlined in TOs or as provided by On- Scene Coordinators (OSCs). For emergency response actions requiring immediate mobilization to the site, the contractor will be required to be en route within two (2) hours and arrive at the site no later than four (4) hours from notification for the following primary mobilization cities within Region VIII:
• Denver, Colorado
• Salt Lake City, Utah
For other emergency response actions requiring immediate mobilization to the site, the contractor will be required to be en-route within two hours and arrive at the site no later than twelve (12) hours from notification.
Typically, for other than emergency removal responses (time critical and non-time critical removals), the contractor will be required to deliver all required personnel, equipment, materials and other necessary items within 72 hours of notification. The exception to this is if the task order (TO) or the ordering officer requests a specific mobilization date and time.
Regional Cross Over
Regional cross over responses to areas outside of Region 8 are a possibility.
Response times for such a cross over will be negotiated at the time the TO is issued.
5. Appendix D is revised to omit resume templates for Level I Response Manager and Corporate Safety Officer
6. Attachment 2 is replaced in its entirety. The replacement Attachment 2 also spells out the abbreviation PAS (Project Administrative Specialist) and contains the most current RCMS numbers and descriptions.
