Request for Quotations: SOL-OAA-12-000052 for the
Establishment of a Blanket Purchase Agreement (BPA) for Fleece Blankets
This is a Total Small Business Set-Aside
Issuing Date: June 25, 2012
Closing Date: July 13, 2012
Closing Time: 3:00 P.M., Eastern Standard Time
Prospective Vendors:
The United States Government, represented by the U.S. Agency for International Development
(USAID), is seeking qualified, responsible vendors to provide the supplies and/or services described in the attached statement of work that would result in the establishment of one or more Blanket
Purchase Agreements (BPAs).
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR PART 12, in conjunction with FAR PART 13, as applicable, and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and all qualified small businesses are encouraged to submit quotes.
This RFQ is issued as a total small business set-aside. The North American Industry Classification
System (“NAICS”) code for this acquisition is 313312 Textile and Fabric Finishing (except Broadwoven
Fabric) Mills. The small business size standard is 500 employees.
USAID intends to award BPAs to vendors that meet the following:
1. offer technically acceptable supplies or services
2. at low prices;
3. whose past performance has shown them to be dependable; and
4. are responsible vendors.
Page | 2
Note: Prospective vendors should also be aware that while USAID may award multiple BPAs, USAID will only award the number of BPAs we believe sufficient to meet our needs and regulatory competition requirements. In other words, even if USAID has six responsible firms that submitted technically acceptable quotes at low prices, USAID may award less than six BPAs if USAID has no need for six vendors for this supply or service. In that case, USAID will make award to the technically acceptable, responsible vendor(s) providing the lowest priced quotes. The Contracting
Officer will determine technical acceptability by ensuring that the firm is able to comply with the terms of this RFQ. Responsibility will be based on requirements of FAR 9.1 including:
adequate financial resources or the ability to obtain them;
ability to comply with the required performance period, taking into consideration all existing commercial and governmental business commitments;
satisfactory record of integrity and business ethics;
necessary organization, experience, and skills or the ability to obtain them;
necessary equipment and facilities or the ability to obtain them; and be otherwise qualified and eligible to receive an award under applicable laws and regulations.
Vendors are required to submit quotes in accordance with the attached information at the place and time specified. Vendors wishing to respond shall submit the Test Requirements stated in Section 8 along with their respective TINs and DUNS. The anticipated period of performance for each BPA awards will be for 2 year(s) with 3 option periods. For quotation preparation purposes, vendors should assume a
BPA start date of 07/31/2012.
Vendors must be registered in the Central Contractor Registration (“CCR”) database prior to award at www.ccr.gov. Vendors must present their representations and certifications, through the Online
Representation and Certifications Application (“ORCA”) (http://orca.bpn.gov).
Authorized Geographic Code: The Authorized Geographic Code for this procurement is 937—
Any area or country excluding prohibited sources. http://www.usaid.gov/policy/ads/300/310maa.pdf
Vendors should retain for their records copies of all enclosures which accompany their proposals.
All submissions should be clearly marked with the RFQ number. Please submit the required information electronically to Renee Reed who may be contacted via e-mail at rreed@usaid.gov.

STATEMENT OF WORK
Establishment of a Blanket Purchase Agreement
Fleece Blankets
A. DESCRIPTION/SPECIFICATIONS/STATEMENT OF WORK
The United States Agency for International Development, Office of Foreign Disaster Assistance
(OFDA) is seeking to establish a Blanket Purchase Agreement (BPA) with one or more firms that will allow USAID to contract simply and quickly with these firms for immediate delivery of Fleece Blankets.
This competition for the BPA allows Vendors to submit their quotes to USAID describing how their firm is capable and qualified to provide the commodities and services outlined in this Request for Quotations
(RFQ). The selected vendor(s) would then have a signed agreement with USAID (which includes a minimum order guarantee of zero fleece blankets), through which future purchase orders could easily be issued. The selected BPA holders must be able to perform all activities specified in the scope of work in support of USAID programs.
1. BACKGROUND: The United States’ emergency response to current deteriorating humanitarian conditions due to natural disasters and complex emergencies around the world require USAID/OFDA to immediately purchase commodities to increase warehouse inventory to levels to both support ongoing responses and field operations and provide lifesaving timely responses to unpredictable future disasters. The capacity of the United States’ Government to quickly respond to international emergencies is dependent on having access to the requested commodities in a timely manner. To accomplish this, USAID/OFDA maintains stockpiles of commonly requested materials. As these commodities are drawn down to fulfill emergency requests, they need to be replenished in preparation for the next emergency situation.
2. SCOPE: This RFQ is issued to establish arrangements for ordering, production, and billing to cover the fleece blankets during the blanket purchase agreement period of performance. A separate contract will be awarded for transportation of the fleece blankets. The fleece blankets shall be picked up from the contractor’s facilities; no shipping costs will be included in the issues of a contract or purchase order. As the fleece blankets are standard configuration commodities, which are purchased by other governmental and non-governmental entities, any balance of fleece blankets remaining in storage at the end of this agreement shall be retained by the contractor, without obligation by USAID, at the end of the BPA agreement period of performance.
3. PACKAGING AND DIMENSIONS: Blankets are to be baled as specified below.
a. Quantity per bale: 25 pieces
b. Blankets to be covered with a polypropylene or jute woven bag, compressed and strapped with 5 straps (2 lengthwise, 3 crosswise).
c. Height of the bales to be compressed by maximum 40% from free state to final compressed and strapped state.
Page | 5
i. Ex: if the bale is 1m high at free state, it should be compressed to a height of 0.6m at final and strapped state.
d. Bale dimensions: Length approx. 0.8m, width approx. 0.5m
The contractor must provide separate pricing for the listed options.
a. Pricing Option A: Blankets to be individually wrapped in a re-sealable clear plastic bag no less than 2 Mil. = 50.8 Microns thickness and then baled as specified.
b. Pricing Option B: Blankets to be baled as specified, without being individually wrapped.
4. BLANKETS MARKING: The USAID Logo must be used on commodities and boxes per technical specifications provided. Logo is to conform to USAID Branding Specifications, Full
Color Vertical logo on a white background. Minimum diameter of 14” (35.56 cm). The logo shall appear in the center of the blanket. An electronic copy of USAID logo design will be provided upon request. Please see the following web-link for USAID Branding instructions.
http://www.usaid.gov/work-usaid/branding/acquisition-awards.
5. BALE MARKING:
a. Each bale shall be marked in with a label or tag identifying its contents.
b. Each bale shall also indicate the name of manufacturer (NO ADDRESS), and date of the order or date of manufacturer.
c. Marking example: Blanket, Synthetic, 1.5x2m, medium Thermal-25 pieces.
d. Print on each bale the USAID Logo. Logo is to conform to USAID Branding
Specifications, Full Color Vertical logo on a white background. Please see the following web-link for USAID Branding instructions. http://www.usaid.gov/work-usaid/branding/acquisition-awards.
e. The printing on each outer bale shall be indelible or of a non-smudge nature and in a contrasting color.
6. WARRANTIES: The contractor must provide full manufacturers warranties which shall be stated in the proposal.
7. TECHNICAL REQUIREMENTS AND SPECIFICATIONS: The contractor must provide weight and dimensions of all cargo, both individual bales and palletized.

9. The Government will award a BPA(s) resulting from this RFQ to the responsible vendor whose quote/offer conforming to the RFQ will be lowest priced, technically acceptable quote. The following factors shall be used to evaluate quotes:
(i) technical acceptability;
(ii) price;
(iii) past performance;
Options. The Government will evaluate quotes for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the
Government to exercise the option(s).
10. EXTENT OF OBLIGATION: USAID is obligated only to the extent of authorized calls actually placed against this blanket purchase agreement and shall not be liable in any manner in the event no calls are made.
Page | 10
11. PRICING: The Vendor will furnish the Contracting Officer a price list for items it proposes to furnish under this Agreement. The listing must be identified with the BPA number, and must be for a specific period. No price changes under this agreement may be made unless approved by the Contracting
Officer. If prices are expected to change, the Contractor must provide sufficient advanced notice to the
Contracting Officer prior to the effective date of the change. BPA Calls will be made for only the items that appear on the APPROVED PRICE LIST. Negotiation or RFQ for prices and additional requirements will be conducted by the Contracting Officer only. The contractor hereby warrants that the prices offered to USAID shall be as low as, or lower than, those charged the supplier’s most favored customer (MFC) for comparable quantities under similar terms and conditions, and shall include any discounts for prompt payment. Fleece Blankets pricing shall be on the basis of FOB Origin (see U.S. Federal Acquisition
Regulation clause 52.247-29).
12. PERFORMANCE PERIOD: The BPA will be effective from the date of establishment for a period of 2 year(s) with 3 option periods unless cancelled. Work on issued purchase order may extend beyond the effective period of this BPA. However, no new purchase orders may be issued once the 2 year(s) with 3 option periods has expired.

Page | 12
(3) Acceptable evidence to establish the time of receipt at the Government installation includes the time/date stamp of that installation on the offer wrapper, other documentary evidence of receipt maintained by the installation, or oral testimony or statements of Government personnel.
(4) If an emergency or unanticipated event interrupts normal Government processes so that quotes cannot be received at the Government office designated for receipt of quotes by the exact time specified in the RFQ, and urgent Government requirements preclude amendment of the RFQ or other notice of an extension of the closing date, the time specified for receipt of quotes will be deemed to be extended to the same time of day specified in the RFQ on the first work day on which normal Government processes resume.
(5) Quotes may be withdrawn by written notice received at any time before the exact time set for receipt of quotes. Oral quotes in response to oral RFQs may be withdrawn orally. If the RFQ authorizes facsimile quotes, quotes may be withdrawn via facsimile received at any time before the exact time set for receipt of quotes, subject to the conditions specified in the RFQ concerning facsimile quotes. An offer may be withdrawn in person by an offeror or its authorized representative if, before the exact time set for receipt of quotes, the identity of the person requesting withdrawal is established and the person signs a receipt for the offer.

The DUNS+4 is the DUNS number plus a 4-character suffix that may be assigned at the discretion of the offeror to establish additional CCR records for identifying alternative
Electronic Funds Transfer (EFT) accounts (see FAR Subpart 32.11) for the same concern. If the offeror does not have a DUNS number, it should contact Dun and Bradstreet directly to obtain one. An offeror within the United States may contact Dun and Bradstreet by calling 1-866-705-5711 or via the Internet at http://fedgov.dnb.com/webform. An offeror located outside the United States must contact the local Dun and Bradstreet office for DUNS number. The offeror should indicate that it is an offeror for a Government contract when contacting the local Dun and Bradstreet office.
(g) Central Contractor Registration. Unless exempted by an addendum to this RFQ, by submission of an offer, the offeror acknowledges the requirement that a prospective awardee shall be registered in the CCR database prior to award, during performance and through final payment of any contract resulting from this
RFQ. If the Offeror does not become registered in the CCR database in the time prescribed by the
Contracting Officer, the Contracting Officer will proceed to award to the next otherwise successful registered Offeror. Offerors may obtain information on registration and annual confirmation requirements via the Internet at http://www.ccr.gov or by calling 1-888-227-2423 or 269-961-5757.
