PERFORMANCE WORK STATEMENT

LABORATORY SERVICES

OHIO ARMY NATIONAL GUARD

OFFICE OF THE STATE SURGEON

The Ohio Army National Guard Medical Detachment, located at 3990 East Broad Street, Building 11, Section 11, Columbus, Ohio 43213 requires vendor to process laboratory specimens to assist medical providers in determining medical readiness status. The OHARNG has qualified personnel to collect the required specimens, but does not have the required equipment to process the specimens.

Summary of Requirements Table 1 identifies the required laboratory tests that need to be conducted on a regular basis. All tests must be priced. “Per Month Average” is an estimate of the number of tests conducted per month. Tests listed as zero is a test that is conducted on an irregular basis (less than monthly), but must remain on the testing matrix in case the test is needed. There will be no additional requirements for testing beyond those listed in Table 1.

Administrative Requirements The Office of the State Surgeon (OTSS) will input a Purchase Request (PR) monthly prior to the specimens being collected. The OTSS will make a best value determination on individual and cumulative cost for the purchase request based on operational tempo and probably need. The USPFO will issue a CALL each month and distribute to the vendor. Specimens will not be collected until the call is issued to the vendor.

Collection and Specimen Processing Requirements Labs will provide supplies and instructions to the Ohio Army National Guard regarding their procedure for shipment of specimens. Shipping cost must be inclusive, as there will not be separate funding CLIN to cover shipping. These requirements must be included in the response to this quote. Specimens will be collected and processed for testing using the requirements provided by the testing lab. Qualified members of the Ohio Army National Guard will collect the specimens for testing. Supplies and equipment will be provided by the lab. The testing lab will provide detail results of each individual test within 5 days of receipt of specimens.

Payment of Services The vendor shall submit an invoice detailing the test conducted, date of test and name of examinee. The vendor must submit invoices through Wide Area Work Flow in accordance with DFARS 252.232-7006. Payment will be made monthly via a BPA Call.

Each CALL will have a roll-up of all test conducted that month. Prices will be based on the price sheet outlined in Table 1.
