DAM SPILLWAY FUSEGATE LIFTING & MAINTENANCE

U.S. ARMY CORPS OF ENGINEERS

OTTER BROOK LAKE

KEENE, NH

PERFORMANCE WORK STATEMENT

1. SUMMARY

Contractor shall provide lifting and maintenance services of two (2) spillway Fusegates (Fusegates 1 and 2 only) at Otter Brook Dam. Fusegates 1 and 2 shall be lifted individually off the concrete sill to break any bond that may have developed, and contractor shall perform any maintenance deemed necessary in the “Fusegates Operation and Maintenance Manual Rev. C” (Reference 2) to Fuesgates 1 and 2. Maintenance includes clearing debris from the downstream drain holes, the inlet well, and debris from the uplift chamber located at the base of the Fusegates. The Fusegates are designed to overtop and travel downstream during a high flow event such as a PMF (Probable maximum flood), reducing the spillway crest elevation and effectively increasing the amount of discharge that can be released from the spillway without causing overtopping of the dam.

Lifting shall be performed with four (4) jacking shoes and other lifting equipment (hydraulic pump, hydraulic cylinders, etc.) present at the Otter Brook Field Office in Keene, NH.

Calibration and testing of all lifting equipment will be required prior to lifting of the Fusegates.

The joint seal system consisting of rubber seals and steel plates between each Fusegate will have to be removed prior to lifting and re-installed after lifting. A report documenting the work performed including the condition/testing of the equipment, procedure for lifting, removal/re-installation of the joint seal system, maintenance performed, and the loads needed to raise each Fusegate shall be produced.

2. SCHEDULE

Work shall be completed no later than 130 days from the contract award date (Period of Performance). Work shall be performed Monday through Friday between the hours of 7:00 AM and 4:00 PM unless otherwise approved by the Technical Point of Contact. Any proposed changes to an approved schedule must be submitted to the TPOC in writing and are not finalized until approved. No work shall be done on weekends or Government holidays. The Pre-Work Conference shall be completed within 14 days of Contract Award.

3. PROJECT DESCRIPTION

3.1. Otter Brook Dam

Otter Brook Dam (480 Branch Road, Keene, NH 03431-9801) is located in Roxbury and Keene, New Hampshire in Cheshire County on Otter Brook, the principal tributary of the Branch River, which in turn is a tributary of the Ashuelot River. The dam site is situated 4.9 miles above the confluence of the Branch with the Ashuelot River and about 31 miles upstream from the Ashuelot River’s confluence with the Connecticut River at Hinsdale, New Hampshire. The project consists of an embankment, an outlet works, and a spillway with an uncontrolled Fusegate weir.

3.1.1. Spillway Fusegates

The six (6) Fusegates were installed as a result of a1990 hydrologic review which concluded that Otter Brook Dam could not safely discharge flows from a PMF without causing overtopping of the dam and likely dam failure. A 2003 Dam Safety Assurance study recommended that the existing spillway weir be removed, the spillway channel excavated, and a Fusegate structure consisting of six (6) labyrinth concrete overflow sections be added. These sections are designed to overtop, tip once the pool reaches a certain elevation, and travel downstream during a high flow event such as a PMF, thus reducing the spillway crest elevation and effectively increasing the amount of discharge that can be released from the spillway without causing overtopping of the dam. The mechanism by which they tip consists of water entering a vertical inlet well once the pool reaches a certain elevation, water filling the uplift chamber located at the base of the Fusegate and creating uplift that causes the Fusegate to tip. The spillway modifications were completed in the spring of 2006. See Appendix B for construction plans of the Fusegates and appurtenant work and photos of the Fusegates.

The Fusegates were fabricated off-site and placed in their intended locations once the concrete sill was complete. The Fusegates were then lifted (2006) with the jacking equipment in order to confirm their weights and then, based upon the results, concrete ballasts were added for each gate. The purpose of the ballast is to regulate the spillway flow released downstream by preventing each Fusegate from tipping until the upstream pool reaches specified elevations.

Once the ballasts were installed, the Fusegates were lifted once again in order to confirm that their total weights matched up with their design weights. See the “Addendum to Fusegates Design Report Based on As-Built Configuration” (Appendix A) for design details, tipping order, and final 2006 weights (Appendix A, page 25) of the Fusegates. See Appendix E for the “Work Performed Report” by George Carins and Sons, Inc. (2015) documenting the 2015 Fusegate lifting where all Fusegates were lifted for final 2015 weights.

According to the “Fusegates Operation and Maintenance Manual Rev. C” (Reference 2, Appendix A), the Fusegates shall be lifted one-eighth (1/8) of an inch to a maximum one-quarter (1/4) of an inch above the spillway sill in order to break any bond that may have developed between the concrete Fusegates and concrete sill through calcification or siltation. Any bonding would inhibit the Fusegates from tipping and the safety of the dam could be impacted.

All of the Fusegates have been lifted during installation in 2006 and during the 2015 maintenance lifting. See Appendix E for the “Work Performed Report” by George Carins and Sons, Inc. (2015) documenting the 2015 Fusegate lifting where all Fusegates were lifted.

4. SCOPE OF WORK

Contractor shall submit an overall project schedule to incorporate the submittals and key dates, relative to the Period of Performance, as listed below. U.S. Army Corps of Engineers personnel will be on site to perform gate inspection work and to familiarize project staff with lifting methods.

The services to be performed under this scope include:

Task 1: Work Plan and Accident Prevention Plan

Work shall not proceed until the Work Plan and Accident Prevention Plan are reviewed and accepted by the Government. Work Plan and Accident Prevention Plan shall be submitted at least fifteen days prior to anticipated work start date.

Work Plan. The Contractor shall prepare a Work Plan for the work to be performed under this Task Order. The Work Plan shall describe all procedures, personnel, including any proposed subcontractors, proposed methods, equipment, and sequence of work.

The Contractor shall be responsible for obtaining all required clearances and state and local permits.

Accident Prevention Plan. The Contractor shall prepare an Accident Prevention Plan (APP) specific to the activities being performed as detailed in Part 8 below. The accident prevention plan must be approved prior to commencement of fieldwork.

Task 2: Review Background Information and Site Visit

Review “Fusegates Operation and Maintenance Manual Rev. C” (Reference 2, Appendix A) and the “Addendum to Fusegates Design Report Based on As-Built Configuration” (Reference 3, Appendix A); record drawings of the spillway Fusegates and Otter Brook Dam (Appendix B);

review photos of the completed Fusegates; and review operation and maintenance manuals of the lifting equipment present at the Otter Brook Field Office (Appendix C). In addition to the equipment at Otter Brook Dam list in Appendix C, a lifting hoist fabricated by the 2015 lifting Contractor is available. See Appendices E, F, and G for more information regarding the hoist.

This hoist mounts to the top of the Fusegate to allow the transferal of equipment from the upstream to downstream side of the Fusegates.

Any missing information regarding the operation and maintenance of the lifting equipment shall be obtained from the manufacturer.

The Contractor shall visit the project site to evaluate project logistics, to assess access to the upstream and downstream sides of the Fusegates, and to examine available lifting equipment (ie.

mechanical equipment, jacking shoes, jacking shoe bolts, etc.). There is no vehicle access to the downstream side of the Fusegates. Note that a portion of the upstream side of Fusegates contains wetlands (see construction drawings in Appendix B of this document) and areas immediately upstream from Fusegates are marshy at certain times of the year. The Contractor shall ensure no impact occurs to the wetlands due to Contractor operations; in this regard, coordination with the Government will be required. See Section 6 of this document for coordination details.

Task 3: Testing Lifting Equipment

See the “Appendix D” of the “Fusegates Operation and Maintenance Manual Rev. C” (Reference

2) for methods and equipment necessary to lift the Fusegates. The Fusegates are to be lifted via four (4) total jacking shoes attached to the upstream and downstream sides of the Fusegates and four (4) total hydraulic cylinders.

All parts of Task 3 shall be completed prior to lifting of the Fusegates.

Confirm that all lifting equipment necessary for lifting the Fusegates is present at the Otter Brook Field Office in Keene, NH. See Section 6 of this document for coordination details.

Equipment present at the Otter Brook Field Office consists of the original equipment used during the initial lifting in 2006 and the subsequent lifting in 2015. Information regarding of equipment present at the Otter Brook Field Office, including an inventory list, recent photos (2015), cut-sheets, and available manuals, is included in Appendices C, F, and G of this document. 61 bolts and 52 washers for use with the jacking shoes are also available. Equipment includes a lifting hoist fabricated by the 2015 lifting Contractor that is available. This hoist mounts to the top of the Fusegate to allow the transferal of equipment from the upstream to downstream side of the Fusegate. See Section C-1 in Appendix C of this document for an example of the lifting equipment configuration. See Section B-3 of Appendix B of this document for photos from the 2006 lifting. See Appendix E for the “Work Performed Report” by George Cairns and Sons, Inc.

(2015) documenting the 2015 Fusegate lifting where all Fusegates were lifted. See Appendix F for the “Working plan Supplemental Information” by George Cairns and Sons, Inc. (2015) for information regarding the hoist used to transport equipment from the upstream to downstream side of the Fusegates. Additional information regarding use of the hoist during the 2015 lifting can also be found in Appendix E.

Contractor shall confirm that there is enough length of hose to operate upstream and downstream jacks.

Ten (10) jacking shoes (125 lbs each) and bolts are located in the Otter Brook Field Office in Keene, NH. Jacking shoes are to be attached to the Fusegates in their designated location with proper bolts as specified on sheet SF-6 of the Fusegates construction plans (Figure 18, Appendix B of this document). See Appendix C of this document for summary of jacking shoes labels and install locations. These labels are assumed to be correct, however, adjustments to shoe locations may have to be made. See Appendix E for the “Work Performed Report” by George Cairns and Sons, Inc. (2015) documenting the 2015 Fusegate lifting for further information regarding jacking shoe use.

Contractor shall confirm that all bolts necessary to secure the jacking shoes are available.

Any missing information regarding the operation and maintenance of the lifting equipment shall be obtained from the manufacturer.

Contractor shall confirm that all lifting equipment to be used in lifting the Fusegates is working properly. This includes performing any maintenance recommended by the manufacturer, testing the equipment to ensure it will perform properly according to the operation and maintenance manual, and correcting any problems found with the equipment (including replacement of parts).

This includes confirming that the hoist that allows the transferal of equipment from the upstream to downstream side of the Fusegate is in working order and performing any necessary maintenance.

Part of the testing shall be connecting the full lifting system including the hydraulic pump, valves, gauges, connections, hoses, and hydraulic cylinders intended for use in the Fusegate lifting and loading each cylinder to a minimum of 80,000 lbs. This load shall be maintained by each hydraulic cylinder for 5 minutes. Each component of the lifting system must be shown to be working properly in accordance with their respective manufacturer operation and maintenance manuals (i.e. no leaks, loss of pressure, erratic behavior).

The gauges shall be calibrated during the testing to ensure that they provide accurate readings during the lifting.

Prior to lifting the Fusegates, contractor shall provide documentation describing the testing of all equipment intended for use in the lifting and confirmation that it is working properly.

Task 4: Remove and Re-Install Joint Sealing System

The joint sealing system consists of rubber seals secured with steel plates and nuts that prevent water from seeping under and around each Fusegate during a high pool. The system is essential in preventing early tipping of the Fusegates by stopping water from prematurely entering the uplift chamber below each Fusegate.

Scheduling the execution of this task shall be coordinated with the project personnel so that anticipated pool levels will not interfere with site access and execution. Joint sealing system shall only be removed if no large pool events are anticipated during the full duration of time that they are not in place.

Prior to lifting the Fusegates, remove all horizontal and vertical joint seals located between each gate, the concrete sill, and the abutments according to methods specified in the “Fusegates Operation and Maintenance Manual Rev. C” (Reference 2). This includes removal of rubber seals, steel plates, nuts, and washers. Documenting from where materials were removed may aid in re-installing the sealing system. Protect seals and hardware from damage during work. The Contractor shall be responsible for replacing any seals or hardware damaged by work activities or improper storage.

After lifting the Fusegates, re-install all horizontal and vertical joint seals located between each gate, the concrete sill, and the abutments according to methods specified in the “Fusegates Operation and Maintenance Manual Rev. C” (Reference 2). Re-use seals that were initially removed. This includes replacing any damaged or missing hardware with equivalent products that meet requirements specified in the “Fusegates Operation and Maintenance Manual Rev. C” (Reference 2). The system shall be re-installed so that it prevents water leakage from the upstream pool into and around the Fusegate uplift chambers and prevents premature flipping of the Fusegates.

Task 5: Perform Lifting of Each Fusegate and Maintenance

Lifting of each Fusegate is performed to eliminate any bonding with the sill that would inhibit the Fusegates from tipping and prevent any impact to the safety of the Dam and also to clear any debris present in the chamber or that could inhibit function of the Fusegate.

Scheduling the execution of this task shall be coordinated with the project personnel so that anticipated pool levels will not interfere with site access and execution.

Prior to lifting each Fusegate, the downstream drain hole and vertical inlet well pipe shall be inspected and water jetting shall be performed to clear any debris that is present or any potential debris that is unobserved.

Prior to lifting each Fusegate, the threaded anchor slots for the jacking shoes and the bolts for the jacking shoes shall be carefully cleaned with a wire brush and air jetting so that they are free of all sand and debris. Care shall be taken to ensure that each bolt is carefully and properly attached and that damage to the threaded anchor slots is prevented. Care shall be taken to ensure that cross-threading does not occur.

Perform lifting of two (2) spillway Fusegates (Gates 1 and 2 only) according to the “Fusegates Operation and Maintenance Manual Rev. C” (Reference 2, Appendix A). It should be noted that theoretical forces noted in “Appendix D” of the “Fusegates Operation and Maintenance Manual Rev. C” (Reference 2, Appendix A) are no longer applicable as they were for the initial 2006 lifting prior to ballast installation; Fusegates now weigh more than this (see tables on page 25 of the “Addendum to Fusegates Design Report Based on As-Built Configuration” (Reference 3, Appendix A) for 2006 weights. The latest weights can be found in Appendix E, “Work Performed Report” by George Carins and Sons, Inc. (2015), which documented the 2015 Fusegate lifting.

Contractor shall document the pressure and load (in pounds) applied to each individual jack with the Fusegate in the full lifted position (1/8 inch to a maximum of 1/4 inch above sill).

While in the lifted position, the uplift chamber located at the base of each Fusegates shall be inspected and if debris is present, water jetting shall be performed.

As noted on page 14 of the “Fusegates Operation and Maintenance Manual Rev. C” (Reference 2, Appendix A), “Care must be taken to ensure the Fusegate is lifted and lowered in a controlled, horizontal manner (see Appendix D for recommendations for Fusegate Lifting).”

Also noted on page 14, “it is essential to replace the Fusegate exactly in the same position as prior to the lifting. It must be ensured that the contact between the downstream toe abutments and the downstream face of the Fusegate chamber is maintained after lowering each unit.”

Care shall be taken not to impact adjacent Fusegates during the lifting.

Jacking shoes must be secured with proper bolts as indicated on the Fusegate construction drawings.

After the lifting of the two (2) Fusegates has been performed, all lifting equipment shall be placed into storage on-site in a location determined by the Otter Brook Project personnel.

Contractor is responsible for any damage that occurs as a result of their operations (ie. to the spillway Fusegates, appurtenant structures, etc.).

Task 6: Prepare Report Documenting Work Performed

Prepare a report documenting:

Initial condition of each piece of lifting equipment (including high resolution photos) All testing performed on the lifting equipment All maintenance and problems that were corrected with the lifting equipment Method of transporting the lifting equipment to the upstream and downstream sides of the Fusegates Physical configuration of the lifting equipment at each Fusegate Complete method of removal of the joint sealing system (note any issues that existed) Condition of each Fusegate prior to lifting (including photos) (ie. debris present, large cracks in concrete) Distance between each Fusegate and, if applicable, between each Fusegate the abutment prior to lifting Step-by-step procedure used to lift each Fusegate Pressure and Load applied to each jack necessary when each Fusegate is one-eighth of an

(1/8) of an inch to a maximum of one-quarter (1/4) inch off the sill Maintenance performed on each gate (ie. debris clearing) Condition of each gate once fully lowered (any changes from initial condition) Final distance between each Fusegate and, if applicable, between each Fusegate and the abutment after lifting Complete re-installation of the joint sealing system
