Contractor shall be responsible for identifying variances greater than 5% and shall forfeit any claim for equitable adjustment should it fail to notify the COR within 60 days from date the additional equipment (that exceeded the 5% limit) was put in service.

5.2 Basic Services. The contractor shall provide fully qualified and accredited SCOTT Service technicians to provide PM and CM services as required in this SOW. PM is performed as scheduled, while CM may be performed during PM performance, it may also be performed during separate visit as requested by the COR. PM and CM shall be performed with the main objective of having ALL equipment to be in operational status when needed.

5.2.1 Preventive Maintenance (PM). PM shall be performed as scheduled for each piece of equipment listed in Attachment 1 in accordance with prescribed Government manuals/instructions/ directives and individual equipment manufacturer manuals to keep them in good working order.

5.2.1.1 PM Frequency. PM Schedule will be provided to the COR within 30 days after award to include all Installations.

Modifications to this schedule may be negotiated with the COR and the Installation Fire Chief. Any changes to the schedule must take into consideration the expiration of SCBA equipment and Air Cylinder certifications for each installation. Annual servicing of all components listed on contract must be completed.

NOTE: Any equipment that is unavailable for PM but is listed as an asset to receive PM will be noted on the W/O and the COR notified of said deficiency so it may be addressed with the Installation Fire Chief.

5.2.1.2 Equipment Operating and Maintenance Parameters. PM consisting of on-site inspection, testing, maintenance, and recertification of SCBA units and its components within Navy Region Southwest shall maintain strict compliance with NFPA, Manufacturers maintenance requirements, and NIOSH air regulations as laid out in section 1.6.8 to preserve the integrity of the SCBA Equipment and Air Cylinders to avoid unforeseen equipment failure that could cause damage to life and limb.

5.2.1.3 Service shall be conducted on-site and include an inventory of each unit by serial number, model number, location/station number and current service date. PM shall include but is not limited to the following:

- Each unit will be inspected visually.

- The SCBA unit shall be flow tested annually on a Posichek machine, and/or after any modifications requiring a new flow test as outlined in the current SCOTT Technician manuals and Posichek manual. No SCBA shall be returned to service unless accompanied by a Posichek flow test sheet indicating that it has passed all tests 100% and Flow Test sheet is signed by the technician. All completed Flow Test sheets will be provided to the Installation Fire Chief and COR within 7 business days.

- The regulator shall be visually and functionally tested as outlined in the Scott Technician manual. The pressure reducer shall be visually and functionally tested as outlined in the Scott Technician manual.

- The harness and backpack assembly shall be visually and functionally tested as outlined in the Scott Technician manual.

- All flow test reports shall include all serial numbers of the SCBA/SAR/RIT/AIRLINE REGULATORS/ SCOTT Mobile air carts being tested. A copy of all flow test reports must be provided to the COR and Installation Fire Chief within 7 business days of completion of service for each installation.

- All adjustments or repairs made shall be clearly stated in the comments section on the POSICHEK form before being printed and signed. If no adjustments/repairs were made to the unit being tested it shall be stated as such. A copy of all flow test reports must be provided to the COR and Installation Fire Chief within 7 business days of completion of service for each installation.

- All annual certifications, required maintenance and warranty repairs shall be performed on site.

- AA & AAA (voicecomm's) Batteries shall be replaced annually by Contractor at annual servicing. Batteries shall be supplied by the contractor as a component of PM. All batteries being replaced shall be of same type as authorized by SCOTT Technician manual.

All batteries shall be changed in PASS device, Voice Comm, RIT PAK, Chest gauge, SARPAK Gauge assembly, and no mixing of used and new batteries are authorized. The date shall be written on batteries with month / year. All batteries shall be in compliance with manufacturer recommended specifications.

- Full electronic testing of PASS (Personal Alert Safety system), shall be visually and functionally tested as outlined in the Scott Technician Manual.

- Mobile Air Cart Pressure gauges are tested and calibrated. If gauges are not within factory tolerance they shall be replaced and identified in the Work Summary Report.

- Mobile air cart Hand Valves are operationally tested and serviced. If the valve no longer meets minimum operational requirements it will be replaced.

- High Pressure (HP) hoses are checked for damage and replaced if necessary.

- Airline Regulators shall be visually and functionally tested as outlined in the Scott Technician manual.

- The wheeled frame unit, pneumatic unit HP system, LP system, mainframe unit, and electrical unit will be inspected according to manufacturer requirements.

- The following final performance tests will be performed and documented leak, static LP manifold pressure, flow rate, and relief valve set point.

- Testing, maintenance, and recertification of confined space hoses, SAR hoses, air cart hoses or low pressure supplied air line systems will be tested and marked in accordance with NFPA 1852, 1981, 1982, 1404.

5.2.2 Breathing Air Bottles/Cylinders Testing and Recertification.

5.2.2.1 PM will include annual certification of all SCBA Breathing Air Bottles/Cylinder 1,016 - 45 minute cylinders, 548 - 60 minute cylinders and four (4) - 15 minute cylinders.

5.2.2.2 If a cylinder is found to be damaged or at the end of its 15-year life cycle, it shall be noted on the W/O, reported to the COR and the Installation Fire Chief, pulled out of service and tagged as such. (See attachment 1 for equipment list)

5.2.3 Corrective Maintenance (CM). CM is repair service required to restore equipment to its working condition and operating within NFPA, Government and Manufacturer predetermined tolerances and limitations. Services shall be completed within 3 working days, unless, as determined by COR, it is not feasible due to event or situation beyond the control of the contractor. In such a case, contractor shall provide a reasonable target date of completion for COR approval. If in the COR's opinion that provided target date will create an emergency situation (see definition), and the contractor refuses to change proposed target date to an earlier date the COR deemed necessary to prevent emergency situation, the Government reserves the right to enter into a contract with another contractor to complete the CM. CM performance shall be performed on equipment when triggered by one of the following:

5.2.3.1 Routine COR request. Contractor shall provide a written cost estimate for all repairs within 2 business days from the date request is submitted by COR. The cost estimate shall include equipment description, number of hours required to effect the repair and the associated labor cost, replacement parts name and cost, and the grand total cost of the CM. CM shall be performed no more than 3 working days from date cost estimate is approved by the COR. All cost estimates for CM or repairs must be approved by the COR in advance of the work being performed and must be in writing. Communication will be by Email for routine requests.

5.2.3.2 CM requirement identified during PM. When CM requirement is identified on an equipment undergoing PM service, CM shall be performed by the contractor to the equipment as long as: (a) it can be performed during normal working hours (see section 1.6.4), and (b) can be accomplished without deviating from the original scheduled equipment PM (i.e. all equipment originally scheduled are serviced). When CM cannot be completed during the PM day for whatever reason, additional time for a separate visit must be communicated to the COR for approval. Otherwise, the CM may be continued during the next scheduled PM day (where the equipment is located). COR approval is not required when CM is discovered during performance of scheduled PM and ONLY when there is no associated additional cost to the Government. All other CM requiring parts, travel and/or labor cost must be approved by the COR.

5.2.3.3 Emergency COR request. Under an emergency situation (see paragraph 2 - Definitions and Acronyms), CM shall be performed within 1 working day (i.e. 24 hours) from date request is communicated by the COR and received by the contractor. Cost estimate as described in 5.2.3.1 above may be performed via phone conversation between COR and contractor, but confirmed and approved via Email before CM performance.

5.2.2.4 The contractor shall provide a CM report of performed CM services to the COR within 2 business days following CM of inoperable or malfunctioning equipment. A CM equipment log will be utilized and maintained by the contractor to record all services rendered.

5.2.3 Parts. The contractor shall acquire brand new OEM only parts needed to complete performance of CM using the most economic means to ensure equipment is repaired with minimal downtime. Fair and reasonable pricing shall be demonstrated to the COR by showing that price is within plus or minus 5% of Commercial item pricing. Proof of Commercial item pricing can be from: Internet sources to include manufacturer, distributor and other reseller website, or any other sources where price is shown to Government buyers and/or the general public; manufacturer pricing schedule. All procurement of replacement parts must be approved by the COR in advance of purchase.

5.2.3.1 Sales Tax Exemption: The Contractor is hereby advised that the United States Government and Agencies are exempt from State and Local Government Tax by virtue of Article 6 of the United States Constitution. No exemption certificate is required.

5.2.4 Reports and Deliverables. The contractor shall provide written documentation of any performed PM and CM service in a written report to the COR within 2 business days following performance. A repair equipment log will be utilized and maintained by the contractor to indicate all services rendered. See Technical Exhibit 2.
