STATEMENT OF WORK
Install Road Shark Spike Strips Project WHAY230020
Stewart Air National Guard

PART 1
GENERAL INFORMATION
1. GENERAL: This is a non-personal services contract to renovate the main gate entrance facility with the installation of Road Shark spike strips.
1.1 Description of Services/Introduction: The contractor shall provide all personnel, equipment, supplies, facilities, transportation, tools, materials, supervision, and other items and non-personal services necessary to complete this as defined in this Statement of Work:
1.1.1. Background: The 105th Stewart Air National Guard Base main entrance requires a deterrent as a safety measure to protect unauthorized entry to vehicles in the exit lane(s). The government has purchased a Road Shark LP-48 with heat tracers which will be installed across the exit lane to prohibit vehicles from entering the facility through the exit lane.
1.2. Phase 1 – Site Preparation:
1.2.1. This work at a minimum shall include:
1.2.1.1. The contractor shall provide markout of the work area, including the adjacent shoulder and side slope. See plans provided for approximate existing utility locations to be verified by markout.
1.2.1.2. The contractor shall sawcut pavement to meet manufacturers specificcations. See Attachment #3 for manufacturers specifications.
1.2.1.3. The trench for the new road shark shall span the width of the exit lane starting a terminating 1 foot from each guiderail. See Attachment #4
1.2.1.4. The contractor shall remove existing subgrade to a depth required to install new subgrade materials as shown on Foundation Option #2 (Attachment #3). In addition the contractor shall install 6” diamiter perforated pipe within the drain rock layer directly below the drain holes on the Road Shark unit(s). This perforated pipe shall transition to solid 6” PVC pipe in the shoulder and daylight at the nearest point in the adjacent slope to avoid interference with existing utilities. The bottom of the trench shall be stable.
1.3. Phase 2 - Install Road Shark:
1.3.1. Install government furnished Road Shark LPC-48 with heat tracer per manufacturers specifications.
Barrier shall be installed to prevent vehicle and motorcycle circumvention of the device when driving in the wrong direction. All Road Shark units shall be flush mounted. Roadway is 32 feet wide at install location.
1.3.2. All concrete shall be 4,000 psi compressive strength.
1.3.3. All concrete shall be tested for compressive strength in accordance with ASTM 39.
1.3.4. The contractor shall install heat tracer:
1.3.4.1. The Contractor is responsible for designing and installing heat tracer along the inside walls of each road shark panel and extend to the shoulder at a junction box. The junction box shall be connected to the existing junction box near the guard shack by a 1” conduit, in a trench (see attachment #4 for layout) trenching for new conduit shall be in accordance with all local governing codes.
1.3.4.2. All equipment shall be wired and powered by the existing panel located within the guard shack.
There are sufficient spares to achieve the work described in this SOW. All wires shall be labeled on both ends and clearly labeled in the panel.
1.3.4.3. There is an existing 1” conduit with pull strings located in the guard shack that connects to the junction box across the exit lane from the guard shack. This pull string shall be utilized for the electrical connection of the heat tracer.
1.3.5. The contractor shall purchase and install two sets (four signs total) highly reflective roadway wrong way signs reading “STOP DO NOT ENTER severe tire damage will occur”. Purchase and install one set (two signs total) highly reflective roadway exit signs reading “EXIT DO NOT BACK UP severe tire damage will occur”. The contractor shall install one Max speed limit sign. (See attachment #5 for sign specifications). Sign locations will be coordinated with the KO.
1.3.6. The contractor shall purchase stencil “DO NOT ENTER” and paint roadway marking in white reflective paint 10 feet before reaching the barrier in the wrong direction. Stencil shall be large enough to span most or all of the entire two lane roadway.
1.3.7. The contractor shall install bollards are each end of the spike strip between the guiderail and the last Road Shark unit. (See attachment #6 for bollard detail).
1.4. Misc:
1.4.1. The work described in this scope will take place at an active DoD Installation. This will require the contractor to ensure the exit lane capability remains throughout the duration of construction.
1.4.2. The contractor shall submit a work plan and schedule to the KO and the COR at the preconstruction meeting which describes the means and measures to ensure at least one lane of the dual exit lanes will remain open throughout construction.
1.4.3. Any changes to the work plan or schedule shall be coordinated ahead of time with the COR.
1.4.4. The contractor shall be responsible for any damage to existing utilities. Any utilities damaged during construction shall be replaced and restored by the contractor.
1.4.5. Any grass/soil disturbed during construction shall be seeded and restored by the contractor.
1.4.6. Upon completion of work, the contractor shall perform an acceptance test with the base ISS NCIOC present to verify all associated equipment is operating and functioning properly.
1.5. General Information
1.5.1. Management: The project is being managed from the 105th Civil Engineer and 105th
Contracting offices located at Stewart Air National Guard Base in Newburgh, New York. A Contracting Officer’s Representative (COR) will be designated within the 105th CES and will be the on-sight representative for daily coordination.
1.5.2. Period of Performance: The period of performance shall be for the duration of the time it takes to survey, design and renovate the project area as described in detail in Part 5 herein but not to exceed 150 calendar days.
1.5.3. Quality Control: The contractor shall develop and maintain an effective quality control program (QCP) to ensure services are performed in accordance with this SOW. The contractor shall develop and implement procedures to identify, prevent, and ensure non-recurrence of defective services.
The contractor’s quality control program is the means by which he assures themselves that their work complies with the requirement of the contract. Two copies of a comprehensive QCP will be submitted to the KO and COR within 10 days after the contract is awarded or with the contractor’s proposal if it is an evaluation factor. After acceptance of the quality control plan the contractor shall receive the contracting officer’s acceptance in writing of any proposed change to his QC system.
1.5.4. Quality Assurance: The government shall evaluate the contractor’s performance under this contract in accordance with the Quality Assurance Surveillance Plan. This plan is primarily focused on what the Government must do to ensure that the contractor has performed in accordance with the performance standards. It defines how the performance standards will be applied, the frequency of surveillance, and the minimum acceptable defect rate(s), see Technical Exhibit One.
1.5.5. Recognized Holidays:

1.5.6. Place and hours or Performance: The work to be performed under this contract will be performed in building 107, located at the 105th Airlift Wing, Stewart Air National Guard Base, Newburgh, New York. The contractor is responsible for conducting business between the hours of 0700-1600 hours Monday thru Friday except Federal holidays or when the Government facility is closed due to local or national emergencies, administrative closings, or similar Government directed facility closings. For other than firm fixed price contracts, the contractor will not be reimbursed when the government facility is closed for the above reasons. The Contractor must at all times maintain an adequate workforce for the uninterrupted performance of all tasks defined within this SOW when the Government facility is not closed for the above reasons. When hiring personnel, the Contractor shall keep in mind that the stability and continuity of the workforce are essential.
1.5.7. Disposal Requirements: The Contractor is responsible for the collection and disposal of all debris, rubble, residue and waste material generated in the performance of work under this contract. All such materials shall be disposed of in accordance with all Federal, State and local laws and regulations off the installation. The CONTRACTOR is not permitted to deposit any such materials in base trash collection containers or at any on base location without prior approval of the Contracting Officer.
1.5.7.1.1. Hazardous Materials: Materials classified as hazardous by US environmental rules, regulations, or laws, shall be handled and processed for disposal in accordance with those rules, regulations, or laws. The CONTRACTOR is responsible for all such materials that are residue from CONTRACTOR furnished supplies and materials which were brought to the job site by the CONTRACTOR, and for any such materials identified herein or by the drawings.
1.5.8. Security:
1.5.8.1. Physical Security: The contractor shall be responsible for safeguarding all government equipment, information and property provided for contractor use. At the close of each work period, government facilities, equipment, and materials shall be secured. Failure to do so can result in Government compensation.
1.5.8.2. Military Security: Military procedures and regulations are strictly enforced. Contractor is responsible for identifying necessary procedures to complete this project and ensure all personnel under this contract is familiar and abides by those policies and procedures.
1.5.8.3. Base Access: Contractor is responsible for attaining all permits, licensing and passes as required by Stewart Air National Guard Base. Coordination with project COR needs to be given 1 week in advance to properly submit for a Stewart Air National Guard Base pass. Once all paperwork is submitted to Stewart Air National Guard Base pass and ID, a period of 2 working days will be given for processing. The contractor is responsible for maintaining an accurate Base Pass List of all individuals they have submitted for base access. This Base Pass List is to be turned into the COR for proper documentation whenever new individuals are added. Once the project is closed out all passes will be turned into the COR who will reference the Base Pass List to account for all passes issued and for proper disposal.
1.5.8.4. Restricted Area Access: Work in and around restricted areas to include all three hangars when aircraft are present inside of the hangar, require an escort with a current and approved restricted area badge. The restricted area badge is to be properly visible at all times. Work in the restricted area needs to be properly coordinated with 105th CE Production Control Office at least 2 days prior to the needed access. Work will not be conducted unless the escort is physically present at all times while working in the restricted area.
1.5.8.4.1. Escort: A single escort will be made available by the 105th Civil Engineer Squadron when working in restricted areas. Due to security concerns, the Government requires all contractors and associated subs to be in the presence of a military escort at all times within specific locations. Once the work schedule is approved it is the contractor's responsibility to notify the contracting officer’s representative at least 24 hours in advance of the time and location that the escorts shall meet the contractor. The contractor must make all attempts to promptly meet this appointment. The Stewart Air National Guard will not be held liable for any claim attributable to the contractor's failure to follow these guidelines for arranging and meeting escorts.
1.5.8.4.2. Escorted Personnel: All Contract personnel shall maintain on his or her person proper Federal or State photo identification at all times.
1.5.8.4.3. Upon award of the contract the Contractor shall submit a list of all personnel to be on the project and obtain Stewart ANG Contractor Base Pass ID by submitting all required documents; social security numbers, driver license, etc., to receive contractor passes. Work shall be arranged so that contractor's personnel can be escorted by the 105th Civil Engineering Squadron in areas which are considered to be restricted. No Contractor employee, associate, or other representative shall have any visual, audible, or physical access to any area marked as a "restricted area". Contractor's personnel shall not violate any security regulations pertaining to the facility.
Violators will be removed from the premises with the right to re-enter revocable. Contractor's day-to-day work schedules in the restricted areas shall be so arranged to allow for minimum escort.
1.5.8.5. Permits: Contractor is responsible for attaining all permits and license as required by the Stewart Air National Guard Base.
1.5.8.6. Contractor is responsible for verifying existing plans/conditions to ensure that they do not interfere with existing utilities.
1.5.9. Post Award Conference/Periodic Progress Meetings: The Contractor agrees to attend any post award conference convened by the contracting activity or contract administration office in accordance with Federal Acquisition Regulation Subpart 42.5. The contracting officer, Contracting Officers Representative (COR), and other Government personnel, as appropriate, will meet periodically with the contractor to review the contractor's performance. At these meetings the contracting officer will apprise the contractor of how the government views the contractor's performance and the contractor will apprise the Government of problems, if any, being experienced. Appropriate action shall be taken to resolve outstanding issues. These meetings shall be at no additional cost to the government.
1.5.10. Contracting Officer Representative (COR): The (COR) will be identified by separate letter.
The COR monitors all technical aspects of the contract and assists in contract administration The COR is authorized to perform the following functions: assure that the Contractor performs the technical requirements of the contract: perform inspections necessary in connection with contract performance:
maintain written and oral communications with the Contractor concerning technical aspects of the contract: issue written interpretations of technical requirements, including Government drawings, designs, specifications: monitor Contractor's performance and notifies both the Contracting Officer and Contractor of any deficiencies; coordinate availability of government furnished property, and provide site entry of Contractor personnel. A letter of designation issued to the COR, a copy of which is sent to the Contractor, states the responsibilities and limitations of the COR, especially with regard to changes in cost or price, estimates or changes in delivery dates. The COR is not authorized to change any of the terms and conditions of the resulting order.
1.5.11. Key Personnel: The following personnel are considered key personnel by the government:
Project Engineer, Controls Engineer/Professional Engineer, and Project Superintendent. The contractor shall provide a project engineer who shall be responsible for the performance of the work. The name of this person and an alternate who shall act for the contractor when the project engineer is absent shall be designated in writing to the contracting officer. The project engineer or alternate shall have full authority to act for the contractor on all contract matters relating to daily operation of this contract. The project engineer or alternate shall be available 0700 to 1600, Monday thru Friday except Federal holidays or when the government facility is closed for administrative reasons.
1.5.12. Workplace Safety: The contractor is responsible for the safety of those under his charge.
Care must be taken with all vehicles especially when backing (using a spotter to watch for obstructions and personnel) and in the performance of all work. Care shall be taken during the work hours by cordoning off the work site and securing the work site at the end of each work day. The Contracting Officer or their designated representative reserves the right to halt progress of work if unsafe workplace practices are found and until they are addressed to his/her satisfaction. Contractor shall adhere to applicable OSHA or DOD safety codes.
1.5.13. Contractor Manpower Reporting: The contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for the Air Force.
PART 2 DEFINITIONS
& ACRONYMS
2. DEFINITIONS AND ACRONYMS:
2.1. DEFINITIONS:
2.1.1. CONTRACTOR. A supplier or vendor awarded a contract to provide specific supplies or service to the government. The term used in this contract refers to the prime.
2.1.2. CONTRACTING OFFICER (KO). A person with authority to enter into, administer, and or terminate contracts, and make related determinations and findings on behalf of the government. Note: The only individual who can legally bind the government.
2.1.3. CONTRACTING OFFICER'S REPRESENTATIVE (COR). An employee of the U.S.
Government appointed by the contracting officer to administer the contract. Such appointment shall be in writing and shall state the scope of authority and limitations. This individual has authority to provide technical direction to the Contractor as long as that direction is within the scope of the contract, does not constitute a change, and has no funding implications. This individual does NOT have authority to change the terms and conditions of the contract.
2.1.4. DEFECTIVE SERVICE. A service output that does not meet the standard of performance associated with the Statement of Work.
2.1.5. DELIVERABLE. Anything that can be physically delivered, which will include non-manufactured things such as meeting minutes or reports.
2.1.6. KEY PERSONNEL. Contractor personnel that are evaluated in a source selection process and that can be required to be used in the performance of a contract by the Key Personnel listed in the SOW.
When key personnel are used as an evaluation factor in best value procurement, an offer can be rejected if it does not have a firm commitment from the persons that are listed in the proposal.
2.1.7. PHYSICAL SECURITY. Actions that prevent the loss or damage of Government property.
2.1.8. QUALITY ASSURANCE. The government procedures to verify that services being performed by the Contractor are performed according to acceptable standards.
2.1.9. QUALITY ASSURANCE SURVEILLANCE PLAN (QASP). An organized written document specifying the surveillance methodology to be used for surveillance of contractor performance.
2.1.10. QUALITY CONTROL. All necessary measures taken by the Contractor to assure that the quality of an end product or service shall meet contract requirements.
2.1.11. SUBCONTRACTOR. One that enters into a contract with a prime contractor.
2.1.12. WORK DAY. The number of hours per day the Contractor provides services in accordance with the contract.
2.1.13. WORK WEEK. Monday through Friday, unless specified otherwise.

GOVERNMENT FURNISHED PROPERTY, EQUIPMENT, AND SERVICES
3. GOVERNMENT FURNISHED EQUIPMENT AND SERVICES:
3.1. Facilities: The Government will provide access to the necessary facilities to ensure proper installation and set-up of said systems.
3.2. Utilities: The Government will provide the Contractor with the basic utilities for general use. The Contractor shall instruct employees in utilities conservation practices. The contractor shall be responsible for operating under conditions that preclude the waste of utilities, which include turning off the water faucets or valves after use.
3.3. Equipment: The Government will not be providing any kind of equipment for use by the Contractor.
3.4. Materials: The Government will provide the Road Shark LPC-48 (X sections totaling 30 feet) and heat tracer to be placed within the units along with an additional 5 feet of heat tracer for extension into the junction box.

4.1. General: The Contractor shall furnish all supplies, equipment, facilities and services required to perform work under this contract.
PART 5 SPECIFIC
TASKS
5. Specific Tasks:
5.1 Basic Services. The contractor shall provide services for design, demolition and installation of Road
Shark spike strips at the main gate exit lane.
5.1.1 Basis of Design:
5.1.1.1 Design a complete and usable installation plan for Road Shark LPC-48 to span both exit lanes with heat tracer.
5.1.1.2 Complete set of as-built drawings with all applicable mark ups in AutoCAD 2014 or newer. One hard copy and two
CD’s.

PERFORMANCE REQUIREMENTS
SUMMARY
The contractor service requirements are summarized into performance objectives that relate directly to mission essential items. The performance threshold briefly describes the minimum acceptable levels of service required for each requirement. These thresholds are critical to mission success.
